香港地区针灸戒烟受试者的依从性影响因素分析*

Predictors for smoking cessation: a study of compliance of cigarette smokers treated with acupuncture in Hong Kong*

摘要: 目的 探讨香港地区针灸戒烟受试者的依从性影响因素。方法 选择2010年8月到2015年6月参加中国中医科学院针灸研究所与香港博爱医院联合开展的“中医针灸戒烟先导研究”的受试者,比较脱落人群与未脱落人群的人口学、吸烟背景、戒烟意愿等资料的差异。以8周和52周是否脱落作为因变量,使用二项分类Logistic回归分析法筛选出针灸戒烟的依从性影响因素。结果 共纳入受试者4 480例,8周、26周和52周的依从率分别为68.86%、58.24%和49.51%。纳入的受试者以男性为主(62.25%),年龄(43.50±12.04)岁,文化程度以中学为主(62.46)%;烟龄(25.09±11.61)年,每日吸烟数(17.68±7.90)根,法氏烟草依赖评估量表(FTND)指数(5.35±2.29)分,大多数(81.12%)曾经戒过烟,80.96%的受试者因担心自身健康戒烟;总针灸次数(5.40±3.25)次。Logistic回归分析结果表明,影响针灸戒烟8周受试者依从性的因素有FTND(OR=1.048,95%CI 1.007~1.092);是否因他人劝说戒烟(OR=0.795,95%CI 0.708~0.892);选择中医针灸原因 (OR=1.101,95%CI 1.031~1.176);总针灸次数(OR=1.346,95%CI 1.290~1.404)和是否首月完成6次治疗(OR=2.975,95%CI 2.424~3.652)。影响针灸戒烟52周受试者依从性的因素有性别(OR=1.406,95%CI 1.207~1.637);总针灸次数(OR=1.134,95%CI 1.097~1.173)和是否首月完成6次治疗(OR=5.128,95%CI 4.242~6.220)。结论 香港地区针灸戒烟受试者依从性较好,其中高质量的针灸治疗是提高受试者依从性的最重要因素。

Abstract: Objective To explore the predictors for the compliance of cigarette smokers who were acupunctured for stopping smoking in Hong Kong. Methods Motivated daily smoking quitters were recruited from the project of Plot Study of Acupuncture for Smoking Cessation conducted by Institute of Acupuncture and Moxibustion of China Academy of Chinese Medical Sciences and Hong Kong Pok Oi Hospital from August, 2010 to June, 2015. After comparing the subjects dropped out of the study with the ones completed it, the data of demography, smoking background, desire of smoking cessation were obtained. Then the compliance data at the 8th week and the 52nd week were taken as independent variable, and the predictors were screened by using Logistic regression analysis. Results Altogether 4 480 volunteers were eligible, with the compliance rate at the 8th week, 26th week and 52nd week of 68.86%, 58.24% and 49.51% respectively. The participants were primary males (62.25%), the age was 43.50±12.04 years, and 62.46% had a secondary education. The smoking background was listed as the following: smoking history was (25.09±11.61) years, number of smoked cigarettes per day (CPD) was 17.68±7.90, Fagerstrom Test For Nicotine Dependence (FTND) score was 5.35±2.29, the percentage of previous quit attempts was 81.12% and 80.96% quit attempts concerned about health. The participants received 5.40±3.25 sessions of acupuncture. The Results of Logistic regression analysis indicated that the predictors for smoke cessation with acupuncture treatment were some different for eight weeks and for 52 weeks. At the eight-week-treatment, the predictors were “FTND score”(OR=1.048,95% CI[1.007,1.092]), “due to persuasion” (OR=0.795, 95%CI[0.708,0.892]), “reasons to select acupuncture” (OR=1.101,95%CI[1.031,1.176]), “total cessations of acupuncture received” (OR=”1.346, 95%CI[1.290, 1.404]), and “whether finished six cessations of acupuncture in the first month” (OR=2.975,95%CI[2.424,3.652]). At the 52-week-treatment, the predictors were “gender” (OR=1.406,95%CI[1.207,1.637]), “total cessations of acupuncture received” (OR=1.134,95%CI[1.097,1.173]), and “whether received six cessations of acupuncture in the first month” (OR=5.128,95%CI[4.242,6.220]). Conclusion The compliance of the smoking quitters treated with acupuncture was high, and the predominant predictor in increasing the compliance was high-quality acupuncture received.
