基于关联规则和熵聚类算法的抑郁症肝郁脾虚证患者汉密尔顿抑郁/焦虑量表研究*

Characteristics of Hamilton Depression Scale and Hamilton Anxiety Scale in patients with depressive disorder-liver depression and spleen deficiency pattern: a study based on association rules and entropy clustering algorithm*

摘要: 目的 研究抑郁症肝郁脾虚证患者汉密尔顿抑郁/焦虑量表特征。方法 将200例抑郁症肝郁脾虚证病例的一般资料、病史信息、汉密尔顿抑郁量表(HAMD)、汉密尔顿焦虑量表(HAMA)、中医相关信息录入中医传承辅助平台软件(TCMISS)V2.5,使用关联规则、改进的互信息法、复杂系统熵聚类等算法,进行数据的统计分析。结果 基于关联规则提取HAMD条目组合为:忧郁情绪,工作和兴趣,精神性焦虑,躯体性焦虑,全身症状,能力减退感,入睡困难,睡眠不深,早醒,胃肠道症状;基于复杂系统熵聚类演化出7组核心组合。基于关联规则提取HAMA条目组合为:抑郁心境,焦虑心境,紧张,失眠,感觉系统症状,呼吸系统症状,植物神经症状,记忆或注意力障碍;基于复杂系统熵聚类演化出3组核心组合。结论 肝郁脾虚证为抑郁症最常见证候。抑郁症的临床表现比较复杂,可表现出多个系统的症状,与研究中200例抑郁症肝郁脾虚证患者的汉密尔顿抑郁/焦虑量表所得出的症状条目一致,即抑郁症肝郁脾虚证患者的汉密尔顿抑郁/焦虑量表得出的结果集中在植物神经系统、感觉系统、呼吸系统、胃肠道系统、肌肉系统、心血管系统等多个系统。

Abstract: Objective To study the characteristics of Hamilton Depression Scale (HAMD) and Hamilton Anxiety Scale (HAMA) in patients with depressive disorder-liver depression and spleen deficiency pattern. Methods The general data, disease history information, Results of related psychological assessment and examination scales (HAMD and HAMA) and relevant information of TCM from 200 cases were input into TCMISS V2.5 software. All data was statistically analyzed by applying association rules, improved mutual information method and entropy clustering algorithm of complex system. Results HAMD item combinations, extracted based on association rules, included depressed mood, work and interest, mental anxiety, somatic anxiety, systemic symptoms, hypoesthesia, insomnia-early, insomnia-middle, early awakening and gastrointestinal symptoms. There were 7 core combinations evaluated based on entropy clustering algorithm of complex system. HAMA item combinations, extracted based on association rules, included depressed mood, anxious mood, tension, insomnia, symptoms of sensory system, respiratory system and vegetative nervous system, and dysmnesia or attention deficits. There were 3 core combinations evaluated based on entropy clustering algorithm of complex system. Conclusion Liver qi depression is the basic pathogenesis of depressive disorder. As the disease developing, liver depression and spleen deficiency will be the total trend of pathogenesis in depressive patients, and liver depression and spleen deficiency pattern is a most common pattern of depressive disorder. The clinical manifestations of depressive disorder are complex, and symptoms of multiple systems can be displayed. The symptom items from HAMD and HAMA are consistent in 200 patients, and assessment Results of HAMD and HAMA focus on vegetative nervous system, sensory system, respiratory system, gastrointestinal system, muscular system and cardiovascular system in patients with depressive disorder-liver depression and spleen deficiency pattern.
