回阳生肌膏治疗糖尿病大鼠阴证疮面的研究*

Treatment of yin-pattern wound with Huiyang Shengji Ointment in diabetic rats*

摘要: 目的 探讨回阳生肌膏促进糖尿病大鼠阴证疮面愈合的机理。方法 66只雄性Wistar大鼠剔除不符合要求的大鼠12只,将剩下大鼠随机分为:实验组、模型组、对照组各18只,除对照组直接进行皮肤切除—塑料环埋置外,其他各组采用“腹腔注射链脲佐菌素(STZ)—肌注激素干预—皮肤手术切除—塑料环埋置”制备糖尿病大鼠阴证疮面的模型。实验组给予回阳生肌膏纱条,模型组和对照组给予凡士林纱条。每天给药1次,分别给药3、7、10 d后每组各处死6只大鼠,观察疮面愈合及形态变化,并取病理组织进行切片,观察HE染色及CD34免疫组化染色情况。结果 给药3、7、10 d后,模型组愈合率均低于对照组(P<0.01),给药3 d实验组和模型组之间的愈合率差异不显著,但在给药7、10 d时愈合率有明显的差异,实验组愈合率均高于模型组(P<0.05);通过HE染色结果显示给药7、10 d后实验组和对照组较模型组肉芽组织生长旺盛,成纤维细胞、毛细血管丰富,胶原纤维加粗,炎性细胞较少,而模型组在给药7 d后炎性细胞达到最高值,给药10 d后有了明显的下降,且毛细血管开始增加,胶原纤维不断增多;给药3、7 d,实验组和对照组微血管生成标志CD34的IOD值升高显著(P<0.01),给药10 d略有下降,实验组和对照组之间的差异不显著;模型组在实验期间CD34的含量一直缓慢增加,与实验组比较差异有统计学意义(P<0.01)。结论 回阳生肌膏可动态调节糖尿病大鼠阴证疮面从而有效促进疮面愈合。

Abstract: Objective To investigate the mechanism of Huiyang Shengji (Restoring Yang and Promoting Tissue Regeneration) Ointment on healing of yin-pattern wound in diabetic rats. Methods Male Wistar rats (n=66) were chosen and 12 rats (non-conformity) were excluded, and then divided randomly into test group, model group and control group (each n=18). The control group was directly given skin resection-plastic ring embedding, and other groups were given intraperitoneal injection of STZ-intramuscular injection of hormone-skin resection-plastic ring embedding for preparing model of yin-pattern wound. The test group was treated with HSO gauzes and model group and control group were treated with Vaseline gauzes once a day. After 3 d, 7 d and 10 d, 6 rats were executed in each group, and wound healing and morphologic changes were observed. The wound tissue were collected and prepared pathological sections for observing results of HE staining and immunohistochemical staining of CD34 (microangiogenesis marker). Results The healing rate was lower in model group than that in control group (P<0.01) after 3 d, 7 d and 10 d. The difference in healing rate was not significant between test group and model group after 3 d, but was significant after 7 d and 10 d. The healing rate was higher in test group than that in model group (P<0.05). The results of HE staining showed that granulation tissue grew vigorously, fibroblasts and capillaries were abundant, collagen fibers were thickened and inflammatory cells were less in test group and control group compared with model group after 7 d and 10 d. Inflammatory cells reached the highest value after 7 d and decreased significantly after 10 d, and capillaries began to increase and collagen fibers increased continuously in model group. The IOD value of CD34 increased significantly in test group and control group after 3 d and 7 d (P<0.01), and decreased after 10 d. There was statistical difference between test group and control group but was no significant. The content of CD34 increased slowly in model group during experimental stage, and there were significant statistical difference between model group and test group (P<0.01). Conclusion Huiyang Shengji Ointment can dynamically regulate yin-pattern wound to improve wound healing in diabetic rats based on theory of yang transforming qi and yin transforming body constituent.
