中老年人睡眠时长与气虚质的剂量-反应关系——基于限制性立方样条模型*

Dose-response relationship between sleep duration and qi-deficiency constitution of middle-aged and elderly adults-based on restricted cubic spline model*

摘要: 目的 探索中老年人群睡眠时长与气虚质的剂量-反应关系。方法 从2个已建立的数据库(2 170例和1 814例)中,筛选年龄≥45岁且问卷信息完整的数据1 021例样本纳入分析。以中医体质量表-60条目中的气虚质亚量表得分评估中老年人群的气虚质程度,用自行设计的背景情况问卷中“你平均每天睡觉时间是多少”评价睡眠时长。利用多元线性回归分析及限制性立方样条模型拟合多元线性回归模型评估睡眠时长与气虚质的关系。结果 1 021例中老年人睡眠时长平均为(7.01±1.01) h/d,气虚质平均得分为(28.50±16.00)分。控制混杂因素的多元线性回归分析显示,睡眠时长与气虚质之间的差异无统计学意义(线性分析:β=-0.950,t=-0.060,P=0.071);控制混杂因素的限制性立方样条拟合多元线性回归模型显示,睡眠时长与气虚质之间呈近似的“U”型曲线(非线性检验:P<0.01)。气虚质得分最低点的睡眠时长约为7~7.5 h/d,每天睡眠时长小于7 h和大于7.5 h均是气虚质得分增高的因素。结论 睡眠时长与气虚质呈近似的“U”型曲线,建议中老年人群保证每天7~7.5 h的睡眠,养成规律、健康的睡眠观念,可能是降低其气虚质得分的有效途径。

Abstract: Objective To explore the dose-response relationship between sleep duration and qi-deficiency constitution in the middle-aged and elderly adults. Methods From 2 established databases (with 2 170 cases and 1 814 cases respectively), 1 021 samples age 45 and above and with complete questionnaire information were selected for analysis. The scores of the qi-deficiency constitution subscale of the Constitution in Chinese Medicine Questionnaire (CCMQ) with 60 parameters were used to assess the degree of qi-deficiency constitution, and sleep duration was collected through the question “How long do you sleep on average everyday?” in the self-designed background questionnaire. Multiple linear regression analysis and restricted cubic spline model were used to establish the multiple linear regression model to evaluate the relationship between sleep duration and qi-deficiency constitution. Results Among the 1 021 middle-aged and elderly subjects, the average sleep duration was (7.01±1.01) h/d, and the average score of qi-deficiency constitution was (28.50±16.00). Multiple linear regression showed that there was no relationship with statistical significance between sleep duration and qi-deficiency constitution (linear test:β=-0.950,t=-0.060,P=0.071).After adjustment of the confounding factors, the multiple linear regression model fitted with restricted cubic spline model showed that there is a U-shaped relationship between sleep duration and qi-deficiency constitution (non-linear test:P<0.01). The sleep duration at the lowest score of qi-deficiency constitution was about 7~7.5 h/d, and sleep duration shorter than 7h/d and longer than 7.5 h/d was a factor that increases the scores of qi-deficiency constitution. Conclusion Sleep duration had an U-shaped association with qi-deficiency constitution. Therefore, it is recommended that middle-aged and elderly adults ensure 7~7.5 hours of sleep a day and develop a regular and healthy sleep style, which may help reduce their qi-deficiency level.
