    On clinical application of classic formulas from the formula-pattern and formula-pattern element correspondence*

    Formula-pattern correspondence is a principle for prescribing formula for a specific mechanism. Correspondence between formula and pattern elements (formula-pattern element correspondence is a new principle developed on the basis of formula-pattern correspondence for formulating prescriptions. They are both under the guidance of "treatment according to pattern identification". Exploring the meanings of formula-pattern and formula-pattern element correspondence can help improve flexibility of practitioners and promote standardization of clinical prescription, expand the modern clinical application of classical formulae and improve the therapeutic effects in the treatment of complicated diseases.

    Inspecting and conjecturing the exterior and interior: interpretation and diagnostic significance

    Inspecting and conjecturing the interior and exterior of diseases is a key concept implied in the chapter titled External Conjecture in The Spiritual Pivot (Líng Shū, 灵枢). It can be interpreted with a new connotation. In the process of understanding diseases, we can conjecture the nature of diseases by inspecting the external manifestations or the other way around, in this dual direction of inspecting and conjecturing of the interior and exterior, we could have a more comprehensive and accurate knowledge of the disorder. This is a scientific idea of the The Inner Classic (Nèi Jīng, 内经) which matches both the scientific principle and development of TCM theory. This is supplementary to the traditional one direction of conjecturing the interior nature of disease by observing the external manifestations. Therefore a novel idea of inspecting and conjecturing both the interior and exterior could serve as a fundamental principle for TCM diagnosis.

    Comparing textual variance in three versions of Basic Questions*

    This paper selects three important versions of Basic Questions (Sù Wèn, 素问). They are Gu Congde version in Ming Dynasty, Dushutang version in Yuan Dynasty, and Gulinshutang version in Yuan Dynasty. A comparative study was conducted to analyze the variance of texts in the different versions. Textual variance was categorized into three types and explained with examples from the texts, This study could promote better understanding of text meaning, and provide more reference for the further study on textual variance in Suwen.

    Effects of kidney-tonifying qi-regulating formula on time-course expression of bFGF in rat limbic system with cerebral ischemia*

    Objective To investigate the time-course expressions of the basic fibroblast growth factor (bFGF) in limbic system of cerebral ischemic rats and explore the effects of Yishen Tiaoqi (kidney-tonifying qi-regulating) Formula. Methods Male Wistar rats were randomly divided into normal group ,sham-operation group, MCI group, treatment group and control group according to the scores of Open-Field test. The normal group received no intervention. The sham-operation group received sham operation, and intragastric administration of distilled water. Homogenous microthrombus were injected into internal carotid artery through the external carotid artery of those rats in other groups to duplicate MCI model first. The treatment group and control group received intragastric administration of Yishen Tiaoqi Formula (6.2g/kg) and Nimodipine (9.45mg/kg) respectively. The changes of limbic bFGF and intervention efficacy of Chinese medicinal were observed by using immunohistochemistry technique in the 1st Week,2nd Week and 4th Week.Results Immunohistochemistry test showed that positive cells of bFGF increased significantly in MCI group.The expression of bFGF was significantly higher in the first week than in other time points in MCI group, reached peak at day 7 and then decreased gradually. The positive cells of bFGF increased significantly in Chinese medicinal group compared with MCI group and normal group in the 2nd Week and 4th Week.Conclusion There are changes of limbic bFGF in rats after cerebrovascular disease,and Yishen Tiaoqi Formula can block mental disorder after stroke with improved vascular repair and regeneration.

    Effects of Zuogui Jiangtang Yishen formula medicated serum on the expression of miRNA-30d and GRP78 in podocyte injured model*

    Objective To investigate the expression levels of miRNA-30d and glucose-regulated protein 78kD (GRP78) in podocyte injured model induced by D-glucose (50mmo/L) plus Palmitic acid (2mmol/L), and to observe the changes in miRNA-30d and GRP78 expression levels with Zuogui Jiangtang Yishen (left-restoring sugar-lowering kidney-tonifying) formula in medicated serum 48h after treatment.Methods Cultured podocyte cells were divided into four groups: control group (25mmoL/L glucose, 10% blank serum), model group (D-glucose 50mmo/L combining Palmitic acid 2mmol/L, 10% blank serum), Zuogui Jiangtang Yishen formula medicated serum group (ZJYD, 10% medicated serum, D-glucose 50mmo/L combining Palmitic acid 2mmol/L), and 4-Phenylbutyric acid group (4-PBA, 10% medicated serum, D-glucose 50mmo/L combining Palmitic acid 2mmol/L). After incubation for 48h, the miRNA-30d expression level was analyzed with real-time PCR, and the protein and mRNA expression levels of GRP78 were measured with RT-PCR or western blot respectively. Results The expression level of miRNA-30d was significantly down-regulated in model group, while expression levels of GRP78 protein and mRNA were up-regulated, the differences were statistically significant, as compared with control group (P<0.01), and the expression level of miRNA-30d was inversely correlated with that of GRP78(P<0.05). As compared with model group, the expression level of miRNA-30d was significantly up-regulated in ZJYD group , while down-regulated of GRP78 protein and mRNA(P<0.01), and the effects were partly as good as 4-PBA group. Conclusion 10% ZJYD serum can improve podocyte injury induced by D-glucose 50mmo/L combining Palmitic acid 2mmol/L through up-regulating miRNA-30d expression level, while down-regulating GRP78 mRNA and protein expression levels.

    Anti-allergy mechanism of fangfeng-wumei pair based on expressions of PAR-2*

    Objective By observing the effects fangfeng-wumei pair on degranulation of mast cells and PAR-2 expression before and after the pairing to provide experimental basis for the research on its anti-allergy mechanism. Methods A mast cell degranulation mouse model was established by stimulating mouse mast cell line P815 with trysin in vitro, and the levels of histamine as well as PAR-2mRNA and protein expression of P815 cells affected by fangfeng-wumei pair were measured using RT-PCR, Western blotting and ELISA. Results Fangfeng-wumei pair in vitro significantly inhibited the secretion of histamine and the expression of PAR-2 by mast cells in comparison to cell model group (P <0.05 or P <0.01). Conclusion The results demonstrated that inhibition of degranulation of mast cells was significantly enhanced by fangfeng-wumei pair, and that the expression of PAR-2 by mast cells might be involved in the anti-allergy mechanism of fangfeng-wumei pair.

    Effects of Yiqi Huoxue Jiedu Fang on gene expression of cytokines involving NF-κB signal pathway in collagen II-induced arthritic rats*

    Objective To study the gene expressions of key cytokines involving NF-κB signaling pathway in collagen II-induced arthritic (CIA) rats treated with Yiqi Huoxue Jiedu Fang (qi-tonifying blood-activating and toxin-removing formula). Methods We used collagen II-induced arthritic (CIA) rats by bovine II collagen as an animal model and Leflunomide as controls. CIA rats received treatment with Yiqi Huoxue Jiedu Fang. The levels of inflammatory cytokines in serum were measured with protein chip assay, and hematoxylin dyeing was used to observe the histopathological changes. The gene expressions of the key cytokines in NF-κB signal pathway were detected by real-time PCR. Results This formula inhibited the proliferation of synovial tissues, reduced inflammatory exudation and narrowed the range of fibroid necrosis. It also down-regulated inflammatory cytokines (TNF, IL-1, IL-5, IL-6 and IL-10) in serum of CIA rats. The gene expressions of NF-κB and IKK decreased in CIA rats treated with this formula. Conclusion Yiqi Huoxue Jiedu Fang could regulate the expressions of inflammatory cytokines and the key factor of NF-κB signal pathway for inhibiting inflammatory reaction in injured tissues, restoring the joint injury. Those could be the key active mechanism of RA treated with this formula.

    Antioxidant effects of Gehua JieCheng Fang on orthotopic allograft HCC mice*

    Objective To observe the antioxidant effects of Gehua JieCheng Fang (Kudzuvine Flower Formula for hang-over symptoms) on the orthotopic allograft mice. Methods Orthotopic allograft models were established using H22 ascites tumor cells. Cancerous mice were then treated. Pathological changes of the hepatic lobe were microscopically observed after HE staining. Levels of superoxide dismutase (SOD), Glutathione peroxidase (GSH-Px), and Malondialdehyde (MDA) in the serum were determined by using ELISA methods. Expression levels of SOD, GSH-Px, and MDA in hepatic tissues were compared. Results Liver of the orthotopic allograft mice induced by H22 ascites tumor cells presented with pathological changes of unlimited proliferation and necrosis. Gehua JieCheng Fang affected the expression of SOD, MDA, and GSH-Px, which demonstrated the antioxidant effects. It could slow down carcinogenesis in these mice. Conclusion Gehua JieCheng Fang seems to have therapeutic effects on mice with hepatocarcinoma induced by H22 ascites cells. Its mechanism is related to the activation of antioxidation.

    Effects of Astragalus mongholicus injection on epithelial-mesenchymal transition of renal tubular epithelial cells in diabetic KKAy mice*

    Objective To observe the effect of Astragalus mongholicus injection on the expression of epithelial-mesenchymal transition-related proteins in KKAy mice with renal interstitial fibrosis. Methods Sixteen KKAy mice were randomly divided into model group (n=8) and Astragalus mongholicus injection treatment group (n=8),and 8 C57BL/6J mice were used as normal control. The mice in treatment group were intraperitoneally given Astragalus mongholicus injection from the 14th week. The fasting glucose levels were observed at the 16th, 20th, 24th week. The mice were sacrificed at the 24th week. The renal pathological changes were observed under light microscope. The expression of alpha-smooth muscle actin,E-cad and transforming growth factor β1 in the renal tissues were examined by immunohistochemical staining. Results Fibrosis was found in the KKAy mice of model group,while the mice in treatment group showed a slight increase in renal interstitium. Treatment with Astragalus mongholicus injection decreased the protein expression levels of alpha-smooth muscle actin,transforming growth factor β1 in the kidney tissues as compared with those in model group. Conclusion Astragalus mongholicu injection reduces the expression of alpha-smooth muscle actin and restrains the protein expression of transforming growth factor β1 in the kidney tissue of KKAy mice. These changes may inhibit and reverse the process of epithelial-mesenchymal transition,thus delaying the occurrence and development of renal interstitial fibrosis.

    Effects of wind-dispelling and/or deficiency-tonifying drugs on PI3K/Akt signal pathway in rat hippocampus with cerebral ischemia*

    Objective To explore the mechanism of Houshi Hei San (Hou's Black Powder) against cerebral ischemia damage by observing effects of wind-dispelling drugs (WDD), deficiency-tonifying drugs (DTD) and combined WDD-DTD in Houshi Hei San on the expressions of Brain-derived neurotrophic factor (BDNF), Phosphatidylinositol 3-kinase (PI3K), Protein Kinase B (PKB, also called Akt), and cyclic adenosine monophosphate response element binding protein (CREB). Methods Rats were subjected to permanent middle cerebral artery occlusion (pMCAO) surgery and randomly divided into the sham-operation group, model group, WDD group, DTD group and combined WDD-DTD group. At 72 hours after cerebral ischemia, protein of ischemic hippocampus was extracted from the fresh brain. The expression of BDNF was detected by using ABC-ELISA; PI3K, Akt and CREB were analyzed by using western blot. Results WDD had little effect on neurological deficits score and no remarkable effect on the expression levels of BDNF, PI3K, Akt and CREB; DTD up-regulated the expression of BDNF and Akt markedly (P < 0.05); combined WDD-DTD significantly up-regulated the expression level of BDNF, PI3K, and Akt as well as CREB in the ischemic hippocampus, as compared to the model group (P< 0.05). Conclusion WDD could assist DTD in Houshi Hei San against cerebral ischemic damage by regulating the key molecular signal of PI3K/Akt.

    Correlation analysis between pharmacokinetics of Kaixin San, Radix Polygalae and its monomer based on internal representations of sibiricose A5 and A6 in rats with acquired dysmnesia*

    Objective To study anti-dementia effects of Kaixin San (Joyful Powder) and Radix Polygalae's based on pharmacokinetic characteristics of sibiricose A5, sibiricose A6, 3,6'-disinapoyl sucrose, and polygala xanthone III in rats with acquired dysmnesia model from three levels: monomer, medicinal and formula. Method Serum concentrations of sibiricose A5 and sibiricose A6 after oral administration in rat model were measured by using HPLC-PDA Pharmacokinetic data were obtained. Then different forms of sibiricose A5 and A6 were compared with correlation analysis (PK-DI) . Result Concentration-time curves showed that the pharmacokinetic profiles were similar in the absorption phase when Radix Polygalae extract and Kaixin San were orally administered. A bimodal pattern of sibiricose A5 and A6 in the serum profile was revealed. However, this phenomenon was not observed in the group of monomer. Radix Polygalae extracts and Kaixin San were similar in the measurements of AUC, and levels of monomer were decreased significantly (P< 0.01). Compared to the oral administration of Radix Polygalae extract, Cmax-1 and Cmax-2 of Kaixin San were increased significantly (P< 0.05).Conclution The results showed that the absorption of sibiricose A5 and A6 in rats was superior when Kaixin San was administered. This finding supports the rationale for using the Kaixin San combination in clinical use.

    Protein expression characteristics in qi-deficiency constitution and qi-deficiency pattern of nasopharyngeal cancer using springbio protein chip technology*

    Objective To investigate protein expression characteristics of qi-deficiency constitution and qi deficiency-related nasopharyngeal cancer (NPC) by using protein chip technology. Methods Four cases with nasopharyngeal carcinoma and 4 cases with nasopharyngitis, all of the typical qi-deficiency constitution were examined. Springbio protein chip technology with the capacity of detecting 722 proteins was used to detect protein expressions in qi deficiency constitution, sopharyngeal carcinoma and sopharyngeal carcinoma of qi-deficiency type, so as to explore the characteristics of the protein. Results As compared with the healthy subjects, most protein expressions were down-regulated in qi deficiency patients. There had been 19 down-regulated proteins (p< 0.05), mainly including the keratin protein and cyclin dependent kinase. As compared with nasopharyngitis, 13 proteins were up-regulated while 9 were down-regulated in nasopharyngeal carcinoma protein expressions. CD115/c-fms/CSF-1R/M-CSFR and keratin showed lower expression in nasopharyngeal carcinoma and qi deficiency type. However, Cdk1 / p34cdc2 and p170 / MDR 1 protein were down-regulated in qi deficiency type, but up-regulated in nasopharyngeal carcinoma. Conclusion There is a specific protein expression characteristic in the deficiency constitution, manifesting as lowered expression of keratin and cyclin dependent kinase. Colony stimulating factor 1 receptor and keratin protein low expression are characteristics of qi deficiency related to nasopharyngeal cancer.

    Pattern characteristics of type A influenza in Traditional Chinese Medicine*

    Objective To analyze the pattern characteristics of the type A Influenza in Beijing area.Methods Patients who were diagnosed with Type A Influenza and received treatment in Dongfang hospital, Beijing University of Chinese medicine were included into our research. First, all the included patients answered a TCM questionnaire to gather their basic data and symptoms. Then, patterns of all participants were differentiated according to ying, wei, qi and blood diagnostic principles. Results Of 151 cases with confirmed type A Influenza, 149 cases (98.7%) were identified as exogenous wind-heat patterns while only 2 cases (1.3%) were of exogenous wind-cold patterns. Of the wind-heat patterns, 37 cases (24.5%) were further differentiated as wind-heat with dampness pattern. Wei level pattern was found in 136 cases (90.1%). Qi level pattern was present in 91 cases (60.3%). Wei-Qi level pattern occurred in 78 cases (51.7%). Single Wei level pattern occurred in 59 cases (39.1%). Single Qi level pattern occurred in 14 cases (9.3%). Among the Qi level pattern, 52 cases were yangming heat pattern (34.4%), 40 cases (26.5%) were the shaoyang pattern , 30 cases (19.9%) were the excessive lung-heat pattern, 28 cases (18.5%)were the dampness-heat obstructing middle energizer pattern, and 10 cases (6.6%) were the pattern of heat in the chest and diaphragm. There were significant differences between the constituent ratios of the Qi level patterns (X2=9.73,P=0.02), and single Wei level pattern, of different durations (X2=8.78,P=0.03). The longer the duration was, the higher the proportion of the Qi pattern was accounted for, and the lower the proportion of single Wei level pattern, the lower its occurrence was. However, no significant differences were observed among the distribution of patterns between season, age or its past disease history (P>0.05). Conclusion Duration of disease is an important factor which could affect the pattern distribution of type A influenza. In Beijing, the patterns of type A influenza are mostly externally contracted wind-heat pathogens, second by wind-heat with dampness. The most common pattern is the Wei-Qi pattern, followed by the single Wei level pattern, while the single Qi level pattern is very rare.
