    Theoretical framework changes of Life Gate Doctrine*

    To study the influence of TCM clinical practice on the development of TCM theories by the way of studying “Life Gate (mingmen)Doctrine”, including its history, its theoretical framework and the reason for its vicissitude . The rise was in Ming Dynasty and fall was in Qing Dynasty, and decentralization in Foundamental Theory of Traditional Chinese Medicine, a unified compiled teaching book. In Ming Dynasty, Life Gate Doctrine originated from the requirement of interpreting the life-propelling force and the force mechanism, and was established with Neo-Confucianism as its central foundation. But for the reason of lacking practical ground, the doctrine declined in Qing Dynasty. In Foundamental Theory of Traditional Chinese Medicine, the theoretical framework of Life Gate Doctrine was not original, but a new one based on parts of the original framework of “life-propelling force-liver, heart, spleen, lung, kidney viscera-organ”. The changes reflected the theoretical construction of “liver controlling dipersion” due to two causes: the changes of mainstream thoughts induced the difference of central structure and central theories, which were mutually exclusive in Life Gate Doctrine whose inherent problems were magnified;throughout, the interpretation of “life-propelling force” was indispensable for TCM theory. In a word, it is undoubtedly significant to study TCM Viscera-Organ Theory and improve clinical practice based on understanding the changes of Life Gate Doctrine.

    Reflecting upon sixty-year-cycle in Huangdi Neijing*

    That sixty-year-cycle was an inherent cycle similar to cycle of days, months and years was put forward based on the astronomy background of sixty-year-cycle, relationship between cycles and inspiration by the theoretical structure of Huangdi Neijing. A broad scope of astronomy determined the birth of sixty-year-cycle which was less stable due to the variation of larger cycle. So, if we want to forecast by using sixty-year-cycle, we should face and take advantage of its irregularity.

    Study of TCM thinking based on TCM language: a novel way from cognitive neuroscience

    TCM language is a language based on metaphorical thought, describing a metaphorical world of human physiology, pathology and treatment of diseases. Metaphor is not only a linguistic phenomenon, but more importantly, a cognitive or thinking mode. Various cognitive modes will produce diverse cerebral electromagnetic and metabolic information. Understanding Chinese medicine language will lead to different brain nerve activities from those Western medicine leading to, if there were differences between the two medicines. Event-related potential (ERPs) technology or the functional magnetic resonance imaging (fMRI) technology can be used to capture the different brain activities. This article provided a new way for exploration the proposition“TCM has a unique way of thinking”, from cognitive neuroscience. Consequently, the application of cognitive neuroscience Methods and theories in metaphorical cognition study and the research status of TCM language cognition were analyzed. Accordingly, the theoretical hypothesis of TCM thinking and the scheme of linguistic contrastive study of Chinese and Western medicine by application of ERPs technology and fMRI technology were put forward. It would be of great significance that ERPs and fMRI provide a novel way for empirical study of TCM cognition.

    Modern anatomy proves the structure of muscular interspace

    Muscular interspace (Chinese: 谿谷; pinyin:xīgǔ), a TCM term used to explain the occurrence and development of almost all bone diseases, was a very important human structural concept in Huangdi Neijing. In Neijing, the muscular interspace was clearly described as a depression that muscles interdigitated, which was adjacent to minute collaterals and belonging to bones. However, these features were rarely mentioned in later literature, leading to modern TCM researchers’ evasive explanation for the term. In 1987, a scientist from Australia, Taylor, put forward “angiosome” based on anatomical findings in his published article which mentioned a structure analogous to muscular interspace. Angiosome was described as “the cutaneous perforators pierce the outer layer of the deep fascia near where it anchored to bone and they are easily seen as grooves and valleys around the perimeter of muscles-espescially where they interdigitate”. The validity of the muscular interspace and angiosome was verified by the highly accordance between the anatomical structures. It is beneficial to deeper understand and explore muscular interspace by the way of modern medical technologies, so as to guide TCM clinical practice.

    Academic ideology of moxibustion methods in Bian Que Xin Shu written by DOU Cai*

    To summarize and explore the academic ideology and characteristics of DOU Cai’s experiences of moxibustion by studying Bian Que Xin Shu written by DOU, a traditional Chinese physician in the Southern Song Dynasty. Before moxibustion, DOU was sure to differentiate meridians and yin-yang. Tonifying was sure to be applied and strengthening yang was paid much attention, and spleen and kidney were also regarded as important in critical diseases. DOU’s moxibustion worked wonder on most patients, which was characterized by direct moxibustion with less acupoints, more amount and longer duration. DOU’s moxibustion had a wide range of application in internal medicine, surgery, gynecology, pediatrics and ENT. In this book, most of diseases were followed by proved cases which provided important reference to clinical practice .

    Interpretation and application of theory of yin and yang by physicians of Epidemic Febrile Diseases in Ming and Qing dynasty

    Theory of yin and yang is the foundation of TCM theoretical system. In Ming and Qing dynasty, school of Epidemic Febrile Diseases was rising, with the characteristics of drugs with cold or cool property in prescriptions and Methods of nourishing yin. Although the arising school changed the conventional situations of “using Changes to explain TCM” and “recuperating yang should be always considered as important rather than replenishing yin”, the basic feature of theory of yin and yang undoubtedly lay in its specific differentiation and treatment. The physicians of Epidemic Febrile Diseases, with the representation of YE Tianshi, developed the theory of yin and yang of “preserving the body fluids”, “nourishing Yin and blood”and “promoting the flow of yang and qi”.

    Correlation of mRNA expressions of HSP70 and IL-8 in coating on tongue, HP infection and pathological changes of gastric mucosa in patients with different TCM patterns*

    Objective Taking patients with Helicobacter pylori (HP)-related gatrosis as the subjects, so as to study correlation of TCM patterns, HP infection, pathological changes of gastric mucosa and mRNA expression of heat shock protein70 (HSP 70) and interleukin-8 (IL-8). Methods In the First Affiliated Hospital of Guangzhou University of Chinese Medicine, total 96 patients with HP-related gastrosis were included from March to November in 2010, including 72 cases of chronic gastritis and 24 cases of peptic ulcer, with 68 cases of splenogastric hygropyrexia pattern(SGHP) and 28 cases of spleen deficiency pattern (SDP), and 14 healthy volunteers recruited were assigned into control group. Using rapid urease test, methylene blue test and HE staining to detect HP infection and gastric mucosa pathological changes; using fluorescence quantitative PCR to detect mRNA expression of HSP70 and IL-8 of tongue coating. Results HP infection rate in SGHP was higher than that in SDP, while more severe and more active than the latter; HP infection in patients was more severe and active than that of subjects without HP infection with the same TCM pattern(P<0.01). The gastric mucosal lymphoid follicles, atrophy with intestinal metaplasia and/or hyperplasia pathological changes due to HP infection were significantly higher than no-infected subjects in SGHP and SDP. HSP70, IL-8 mRNA expressions of patients were higher than no-infected ones in tongue coating. More severe HP infection was, more IL-8 mRNA expressed. HSP70 mRNA expression in SGHP was lower than SDP, but IL-8 expression was higher. HSP70 expression of HP infected patients with SDP was significantly higher than no-infected people with SGHP and SDP (P<0.05). Conclusion The correlation of HSP70, IL-8 mRNA expression in tongue coating, HP infected gastric mucosa and atrophy with intestinal metaplasia and/or hyperplasia pathological changes was proved, which showed specific features with TCM patterns. The Results would provide reference to pattern differentiation for HP-related gastrosis.

    Yiqi Yangyin Xiaozheng Tongluo Decoction on CD2AP and WT-1 in glomerular filtration barrier of rats with diabetic nephropathy at early stage*

    Objective To investigate the effect of Yiqi Yangyin Xiaozheng Tongluo (replenishing qi and nourishing yin and resolving mass and freeing collateral vessels)Decoction (here in after called "TCM"), a Chinese herbal medicine compound, on glomerular filtration barrier of rats with diabetic nephropathy (DN) at early stage. Methods Total 110 male SD rats were randomly divided into control group, model group, irbesartan group (15 mg/kg), TCM low-dose, mid-dose and high-dose group at dose of 5.29, 10.58 and 21.16 g/kg respectively. DN rat model was induced by intraperitoneal injection of streptozotozin (STZ) with left nephrectomy. Rats in control group were only performed left nephrectomy. Each medication group was intragastrically administrated (i.g.) corresponding drugs, and the control group and model group were adminstrated with the same volume of normal saline, for consecutive six weeks. Then protein expressions of CD2AP and WT-1 were detected by using PCR, and mRNA expressions were determined by Western blotting. Results Compared with model group, mRNA and protein expressions of CD2AP and WT-1 in all TCM groups and irbesartan group increased significantly(P<0.01). Conclusion Yiqi Yangyin Xiaozhou Tongluo Decoction protected the glomerular filtration barrier in the outer membrane and structure and function of podocytes, and slowed down the development of DN.

    Screening succinate semialdehyde dehydrogenase inhibitors: an established model and its application in Tall Gastrodia Tuber*

    Objective To establish a model of screening succinate semialdehyde dehydrogenase (SSADH) inhibitors with anti-epilepsy effects, then using this model to find out the effective components in Tall Gastrodia Tuber (Latin: Rhizoma Gastrodiae, pinyin:Tianma) and some analogues and to study their structure-activity relationship. Methods First, SSADH enzyme system fluid was prepared, and the correlation of activity and optical density of SSADH was evaluated using UV spectrophotometry. Then, after the screening method of SSADH activity was established by optimization of reaction temperature, reaction time, concentration of NAD+, SSA and SSADH, and pH of buffer on the SSADH activity, p-hydroxybenzaldehyde (HBA), a positive drug recorded in literature, was applied to verify the model. And the structural analogues of HBA were measured and the mechanism was analyzed. Results The screening model was established successfully, proved by HBA test. The UV detective system and its buffers of SSADH activity were determined with reaction temperature at 37 ℃ for 30 min,and detect wave at 340 nm. The structure-activity of HBA on GABA-T was as the same as that of vanillic aldehyde, and —OH and —CHO of benzene ring were the essential groups for inhibition SSADH. Conclusion The model established in this paper can be used for high throughput screening SSADH inhibitors and may guide the study of analogues of SSADH inhibitors and their mechanism.

    Three-step sequential therapy on pathomorphology of bronchial lung tissues of asthmatic rat airway remodeling after hormone intervention*

    Objective To observe the effect of three-step sequential therapy on the pathomorphology of the bronchial lung tissues of rat asthmatic model with airway remodeling after hormone intervention using light microscope and electron microscope. Methods The airway remodeling of asthmatic rats model was induced by hormone intervention to simulate hormone withdrawal (HW) of steroid dependent asthma (SDA) patients. Altogether 150 rats randomly divided into five groups (each n=30) of airway remodeling group, dexamethasone intervention group, pulmicort group, three-step sequential therapy group and normal control group. Morphological changes of the bronchial lung tissues of rats of all groups were observed under light microscope and electron microscope, and thicknesses of the bronchial walls and the bronchial smooth muscle and the number of the cell nucleus in the bronchial smooth muscle were measured. Results Using HE staining, the lung tissues of rats except for those in normal control group were infiltrated by inflammation cells with different degrees, but the rats of three-step sequential therapy group were observed significantly slight inflammatory reaction. Using Masson trichromatic staining, massive collagen fiber in pulmonary interstitial of airway remodeling group deposited, while deposition increased during hormone withdrawal in dexamethasone intervention group, and significantly less deposition was observed in both three-step sequential therapy group and pulmicort group. Before HW, the thickness of the bronchial walls in three-step sequential therapy group decreased significantly compared with that in airway remodeling group(P<0.01); during and after HW, the thickness of the bronchial walls in three-step sequential therapy group decreased compared with dexamethasone intervention group and airway remodeling group(P<0.01,P<0.05). Before HW, there is no difference (P>0.05) of bronchial smooth muscle thickness observed in all the medication groups; during and after HW, bronchial smooth muscle thickness decreased compared with dexamethasone intervention group and airway remodeling group(P<0.01,P<0.05). The number of cell nucleus in the bronchial smooth muscle of three-step sequential therapy group decreased significantly, regardless of HW, compared with dexamethasone intervention group and airway remodeling group(P<0.01) . Using electron microscope, airway epithelial cilia falling off was remarkably alleviated during HW in three-step sequential therapy group and pulmicort group, while that in dexamethasone intervention group during or after HW was the same as the airway remodeling group. Conclusion Three-step sequential therapy could inhibit the airway remodeling by the way of relieving inflammatory changes of the bronchial lung pathomorphology of rat asthmatic model.

    Objective To study the inhibitory mechanism of hydroxysafflor yellow A (HSYA) on abnormal proliferation of vascular endothelial cells. Methods Co-cultured model in vitro was established, with supernatant fraction of LS180 human colon adenocarcinoma cells tumor cells and ECV304 human umbilical vein endothelial cells. Then mRNA expressions of vascular endothelial growth factors (VEGF) and VEGF receptors (KDR), basic fibroblast growth factor (bFGF)and bFGF receptors(bFGFR), proteoglycan related to cell proliferation (HSPG2), p53 and c-myc were detected using real-time fluorescence quantitative PCR; protein expressions of VEFG, KDR, bFGF and bFGFR in supernatant fraction were measured by ELISA. Results In the cell co-cultured model, the HSYA concentration of 0.66 and 0.33 mg/L inhibited mRNA and protein expressions of VEGF,KDR, bFGF and bFGFR ,inhibited mRNA expressions of c-myc and HSPG2, while upregulated p53 mRNA expression. Conclusion HSYA inhibited angiogenesis and abnormal proliferation of vascular endothelial cells in co-cultured model by regulating expressions of VEGF,VEGFR, bFGF, bFGFR, HSPG2, c-myc, wild type p53. HSYA may be a potential tumor angiogenesis inhibitor.

    Diseases, formula and Chinese herbal medicines: a study on spleen-stomach dampness-heat pattern based on modern literature*

    Objective To analyze the distribution of diseases with TCM pattern of spleen-stomach dampness-heat (SSDHP)and the prescriptions and Chinese herbal medicines involved, so as to guide TCM clinical practice and further research on SSDHP. Methods Taking “SSDHP” (Chinese:脾胃湿热证) as subject term or title/keyword, the databases of China National Knowledge Infrastructure(CNKI),Wanfang Data Knowledge Service Platform and VIP Information were systematically retrieved, from the beginning of the databases to 31 December, 2015. The articles related to treatment on SSDHP were collected, and the database was established by using Microsoft Excel 2010, on which data were descriptively statistical analyzed. Results 109 articles were eligible from total 1 681 retrieved ones. The top two diseases with SSDHP were digestive system disorders (86.41%) and dermatoses (5.83%). The common formula directed at clearing heat and dampness including Pingwei Powder, Huopu Xialing Decoction, and Sanren Decoction, combined with specific medications for exact symptoms. The common Chinese herbal medications with frequency more than 1 percent were listed as the following: Golden Thread (Huanglian), Pinellia Tuber(Banxia), Official Magnolia Bark(Houpu), Liquorice Root (Gancao), Dried Decoctionerine Peel (Chenpi), Baical Skullcap Root (Huangqin), Indian Bread (Fuling), Dandelion (Pugongying), Agastache Rugosus (Huoxiang), Atractylodes Rhizome (Cangzhu), Coix Seed (Yiyiren), Round Cardamon Fruit (Baidoukou), Largehead Atractylodes Rhizome (Baizhu), Orange Fruit (Zhiqiao), Immature Orange Fruit (Zhizi), Corydalis Rhizoma (Yanhusuo), White Peony Root (Baishao), Medicinal Evodia Fruit (Wuzhuyu), Dried Ginger (Ganjiang), Bamboo Shavings (Zhuru), Virgate Wormwood Herb (Yinchen), Grassleaf Sweetflag Rhizome (Shichangpu), Turmeric Root Tuber (Yujin), Immature Orange Fruit (Zhishi), Chinese Thorowax Root(Chaihu), Common Bletilla Tuber (Baiji), Danshan Root (Danshen), Bitter Apricot Seed (Kuxingren), Villous Amomum Fruit (Sharen), Common Aucklandia Root (Muxiang), Finger Citron (Foshou), Talc (Huashi). Conclusion Now, the diseases with SSDHP were most common in digestive system, but also in other systems. And the prescription and Chinese herbal medicines should be applied by holistic conception, combination of disease and pattern and pattern differentiation and treatment. Classic herbal formulas in TCM ancient books are still playing the strictly key roles in clinical practice, but proved formulas of modern outstanding senior traditional Chinese practitioners and Hospital Preparations also are paid more attention by researchers.

    Clinical characteristics and long-term prognosis of type 2 cardiorenal syndrome patients with blood-stasis pattern*

    Objective To explore the clinical characteristics and the long-term prognosis of type 2 cardiorenal syndrome (CRS) patients with blood-stasis pattern. Methods 390 cases eligible were divided into CRS group and non-CRS group. After comparing TCM pattern elements between the two groups, we collected the related clinical data. Then all patients were followed-up for 1 810 days to get their final events (survive or dead). Using ROC curves plotted based on the relationship between renal function and long-term prognosis, patients were re-assigned into renal function level groups to try to find out the distribution of TCM pattern and the relationship of blood-stasis pattern with renal function or with modern medical indices. Results Blood-stasis pattern was positive correlated with renal function of chronic heart failure (CHF) patients (P=0.027), but was negative correlated with qi deficiency pattern or yang deficiency pattern, maybe due to the progression of CRS. Conclusion The important effect of blood-stasis pattern on the occurrence and development was proved, and the blood-stasis pattern may indicate the deterioration, dysfunction of heart leading to dysfunction of kidney.

    Kongsheng Zhenzhong dispensing granule on tumor-related insomnia patients with deficiency in heart and kidney*

    Objective To evaluate comprehensively therapeutic effect of Kongsheng Zhenzhong Pill (dispensing granule) (KSZZ), a classic TCM herbal formula, on tumor-relater insomnia patients with TCM pattern of deficiency in heart and kidney, so as to provide foundation for clinical application. Methods Sixty tumor-related insomnia patients with deficiency in heart and kidney who were conformed to the diagnostic criteria were randomly divided into control group and treatment group (each n=30). Patients in treatment group took orally KSZZ twice daily, one pack 30 minutes after breakfast and dinner, and those in treatment group took orally estazolam at dose of 1 mg at night, for consecutive 28 days. Pittsburgh sleep quality index scale (PSQI) and TCM Pattern Integral were used before treatment ,on Day 1, Day 14 and Day 28 after treatment, so as to record PSQI score, TCM clinical pattern and change of single symptom. Then the quality of sleep and TCM pattern curative efficacy were calculated by using SPSS 18.0 software. Results Compared with the control group, PSQI scores and TCM pattern integral decreased significantly (P<0.001).The curative efficacy of KSZZ on insomnia or TCM pattern was higher than that of estazolam(P<0.001). The scoring of individual symptoms on Day 14 or Day 28 were lower obviously than those on Day 1(P<0.001), except sleep efficiency on Day 14 and Day 28 (P>0.05) . Conclusion The curative effect of KSZZ on improving sleep quality of tumor-related insomnia patients has been proved, and KSZZ could be worthy of wide clinical application.

    Objective To explore the establishment of Chinese medical health checkup service (CMHCS)mode. Methods A retrospective study on employees of a public institution, who underwent CMHCS in Center of Preventive Treatment of Diseases, Beijing Chinese Medicine Hospital, Capital Medical University from 2014 to 2015. The effects of CMHCS was evaluated by comparison of satisfaction, life-activity style and change of TCM constitutions before (in 2013)and after (in 2014,2015) CMHCS. Results Employee satisfaction increased significantly in 2014 (91.75%) and in 2015 (93.06%), compared with that in 2013 (77.23 %)(P<0.01).After physicals in 2014, the three health risks were highlighted, including inappropriate diet, shortage of physical activity and irregular daily schedule. Then Chinese medical healthcare education directed at the three risks with TCM contitution bias. In 2015, the life and activity style improved significantly compared with that in 2014(P<0.01). Furthermore, TCM constitution of population who underwent the intervention of “differentiation of constitution and recuperation”for three months improved significantly, compared with those of people who didn’t undergo the intervention(P<0.01). Conclusion CMHCS mode, composed of differentiation of TCM constitution, TCM four examinations, TCM check of meridians and collaterals, interpretation of TCM examination report, and education of TCM healthcare, is a supplement of Western medical checkup service. CMHCS mode, which is combined with Preventive Treatment of Diseases, will meet the people’s requirement of health, and is worthy of further study.
