    The origin and clinical practice of image thinking

    The roots of TCM extend back to antiquity — agricultural civilization, until now TCM has been working on clinical practice to ensure national health. TCM, an important branch of Sinology, is a capsule of spirit of Sinology, conforming to Chinese thought. Therefore, carrying forward the original thinking is the premise of development of TCM. This paper which traced and summarized “image thinking” based on the theory of Sinology, tried to explore again the thinking way of “judging the inside by observation of the outside” enlightended by image thinking. This paper aimed to interpret the origin of TCM pattern theory of “taking manifestations as factors, factors as symptoms and symptoms as syndrome” , not to compare TCM with other medicines. At the same time, the clinical practice of image thinking was explained from the psychosomatic medicine model of “inseparability of the body and spirit, mutual induction of sovereign fire and ministerial fire” .

    View of “Mean and Harmony” and medical concepts in Huangdi’s Canon of Medicine

    “Mean” (Chinese:中) stemmed from the hieroglyphs character of oracle “banner”, and “harmony” (Chinese: 和) was originated from the hieroglyphs character of musical instruments. “Mean” and “harmony” had the same extending meaning of “moderate”. “Mean and Harmony” view, interpreted and applied fully in Book of changes, the Analects of Confucius and the Doctrine of the Mean, was the core idea of confucianists’thought to run the emperor and supervise themselves. When the view was expanded into Huangdi's Canon of Medicine, it underlay the concept of life, disease, treatment and health maintenance, which played an indispensable role in the formation of the theories of Huangdi's Canon of Medicine.

    Study of liver fibrosis rat model with pattern of liver-qi stagnation and validation of pattern time window by TCM formulas treatment

    Objective To explore the pathogenesis and TCM patterns related to liver fibrosis , as well as to establish the rat model of liver fibrosis of combination of disease and pattern. Methods The rat liver fibrosis model with pattern of liver-qi stagnation was induced by subcutaneous injection of carbon tetrachloride. The patterns were distinguished by body weight, open field test, grip strength test, and form collection of symptoms and signs, and the pathological stage was confirmed by HE and Masson staining. Then Chaihu Shugan Powder, Xiaoyao Powder and Sini Powder were used to direct at TCM patterns, and colchicine was used to direct at the disease. Results Four weeks after modeling, pattern of liver-qi stagnation occured on the majority of liver fibrosis rats and lasted to the twelfth week. On the other hand, the main pathological stage was S2 to S3 at the fourth week after modeling, S3 at the eighth week and S4 at the twelfth week. The treatment of TCM formulas validated the improvement of symptoms and pathological stages. Conclusion The time window of TCM pattern of liver fibrosis rat model with pattern of liver-qi stagnation was from 4 to 10 weeks, which was proved by TCM formulas. And the model of combination of pattern and disease was reliable.

    Effect of Jianpi Huashi Tongluo Formula on synovial angiogenesis in rats with adjuvant arthritis

    Objective To observe the mechanism of Jianpi (invigorating the spleen) Huashi (resovling dampness) Tongluo (removing obstruction in collaterals) Formula (JHTF) on PI3K/AKT/mTOR signaling pathway, hypoxia inducible factor (HIF-1α), vascular endothelial growth factor (VEGF-A) ,microvessel density (MVD) and endostatin (ES) of synovial tissue in rats with adjuvant arthritis (AA). Methods Fifty rats were randomly divided into five groups — normal control group, model group, methotrexate group, tripterygium glycosides tablet group and JHTF group. Rat AA model was induced by using Freund's complete adjuvant. After modeling, each treatment group were i.g. corresponding medication for consecutive 30 days, and then the expression of microvascular density (MVD) was detected using immunohistochemical, interleukin (IL)-6, IL-10, HIF-1α and VEGF were measured by enzyme-linked immunosorbent assay, and PI3K, AKT1, p-AKT1, mTOR and ES proteins were detected by Western blotting. Results Compared with normal control group, footpad swelling, arthritic index increased, serum IL-6, VEGF and HIF-1α and MVD, PI3K, AKT1, p-AKT1, mTOR, ES in synovial vascular significantly increased in the model group(P<0.05 or P<0.01). Compared with the model group, the MVD count, serum VEGF-A, HIF-1α and IL-6 decreased in JHTF group. The expressions of PI3K, p-AKT1, mTOR, ES in synovial vascular in JHTF group were lower ,and serum IL-10 was higher than those of the model control group. Conclusion Jianpi Huashi Tongluo Formula can improve angiogenesis by regulating PI3K/AKT/mTOR pathway, expression of HIF-1α and ES in rat AA model.

    Objective To study the effect of medicated serum of Jianpi Yiqi Huayu Jiedu Formula (JYHJF), a TCM compound formulated according to TCM principle of tonifying spleen, replenishing qi, removing blood stasis and detoxification, on proliferation, cycles, apoptosis of HCT116 colorectal cancer cells and on the protein expression of Caspase-3,Bcl-xL and Bax of HCT116 cells. Methods After drug-contained serum (10%, 15%, 20%) treated HCT116 cells for 12, 24, 48 and 72 h, the proliferation was detected by MTT method; after treated for 24 or 48 h, the cell cycle and apoptosis rate were detected by propidium iodide (PI) staining. The protein expression level of Caspase 3, Bcl-xL and Bax in HCT116 cells were detected by Western blot. The control group was treated by blank serum and 10% fetal bovine serum (FBS). Results Compared with the same concentration of blank serum group, the cell proliferation was inhibited significantly in 15% and 20% medicated serum group after 12 h (P<0.05), and the inhibitory effect showed time- and concentration- dependence with the highest at 72 h (P<0.05). In 10% and 15% medicated serum group, cells in G2 phase decreased, but the G1-phase cells and S-phase cells increased; cells in G1 phase and S phase increased and those in G2 phase decreased in 10% medicated serum group with the remarkable Results at 48 h. It was suggested that the medicated serum could blocked cell cycles, with the positive correlation with time. Compared with the blank serum groups at the same concentration, after treated 24 and 48 h, apoptosis rate of HCT116 cells increased, and had the positive correlation with time and concentration. Compared with the blank serum group, the expression of Bax protein was upregulated and that of Caspase-3 and Bcl-xL were downregulated in 15% medicated serum group (P<0.05). Conclusion The effect of Jianpi Yiqi Huayu Jiedu Formula on inhibiting proliferation and inducing apoptosis may be via the way of blocking HCT116 cells cycles and of upregulating Bax protein expression with downregulating Caspase-3 protein and Bcl-xL protein expression.

    Effect of Qilongtian on IL-β and TNF-α in hypoxic pulmonary hypertension rats

    Objective To study the effects of Qilongtian on interleukin-1β (IL-1β) and tumor necrosis factor-α (TNF-α) in hypoxic pulmonary hypertension (HPH) rats. Methods All male rats were randomly divided into six groups:normal group, model group, bosentan group, Qilongtian high-, mid- and low- group. Except the rats in normal group, those in other five groups were under atmospheric hypoxia to establish HPH model. From the first day of modeling, all medication groups were i.g corresponding drugs for consecutive three weeks. Pulmonary hemodynamic parameters were detected by right heart catheterization under direct vision, mRNA expression of IL-1β and TNF-α in lung tissue were measured using ELISA, protein expression of IL-1β and TNFα in lung tissue were measured using immunohistochemistrical methods. Results Qilongtian and bosentan significantly reduced the level of mean pulmonary arterial pressure (MPAP) , IL-1β and TNF-α expression in HPH rats, and the effect of Qilongtian high dosage was stronger with the dose-dependence. Compared with normal group, expression of IL-1β and TNF-α mRNA and protein increased, and those in model group and bosentan group did significantly(P<0.01). Compared with the model group, there was no significant difference in bosentan group (P>0.05), the expression level of IL-1β and TNF-α were significantly downregulated in all Qilongtian groups (P<0.01 or P<0.05). Compared with the bosentan group and all the Qilongtian groups, there were significant differences, and the Qilongtian high-, mid- dose group had more impact (P<0.01 or P<0.05). Conclusion High, middle and low dosage of Qilongtian reduced pulmonary hypertension, with high dosage prior to bosentan. The mechanism of lower blood pressure may be via downregulating the expression of IL-1β and TNF-α, but it would be influenced by other vascular tension factors and angiogenic factors.

    Ancient TCM doctors’ views on treatment of vertigo since Han Dynasty:a data mining research

    Objective To study 15 famous ancient TCM docotors’ prescribing herbal medicines for treatment of vertigo from Han Dynasty and the following periods, so as to provide basis for clinical treatment on vertigo as well as for researching and developing new medicines for treating vertigo. Methods A total of 128 traditional Chinese medicine prescriptions were collected and input into TCM Inheritance Auxiliary Platform Version 2.5. Using the method of improved mutual information entropy clustering, association rules, complex systems entropy clustering and unsupervised entropy hierarchical clustering, the pattern types, pairs of drugs ( two Chinese medicines used together), triangle drugs (three Chinese medicines used together) , association rules of drugs, evolution and extraction of combination of the core drugs, and prescriptions newfound were analyzed. Results The leading pattern of vertigo was hyperactivity of liver yang (frequency:30; rate:23.4%. Same below). The most commonly used herbal medicine was Fuling (49, 38.3%), and the most common pair of drugs was Zhigancao-Baizhu (29,22.7%), the most common triangle drugs was Zhigancao-Baizhu-Fuling (17,13.3%). The drugs combination with high correlation was Baizhu-Danggui→Zhigancao, with support of 10% and confidence of 0.937 5. Altogether 22 sets of triangle drugs were extracted, in which the core drugs were Renshen, Baizhu, Zhigancao, Chenpi, Fuling, Danggui. And 11 new prescriptions were available. Conclusion For research and development of new drugs on vertigo, we should focus on the pattern of hyperactivity of liver yang; prior compatibility rules are Baizhu, Danggui with Zhigancao and Chenpi, Baizhu with Renshen and Zhigancao, Renshen with Baizhu; when formulate a new prescrption, Fuling, Fushen, Zhigancao, Chenpi, Sangye, Juhua, Baizhu and Renshen deserve the priority.

    Effect of electro-acupuncture on Nogo/NgR signal pathway related factors in spinal cord injury rats

    Objective To observe the effect of electro-acupuncture on Nogo/NgR signal pathway related factor gene expression in rats with spinal cord injury, so as to investigate the potential mechanism of electro-acupuncture(EA) treatment for spinal cord injury. Methods The rats were randomly divided into model group, EA group, blocking agent group, EA+blocking agent group, and sham operation group. T10 spinal cord injury model was induced by the modified Allen’s impact method, and the BBB function score on day 1,7,and 14 in each group were obtained after treatment respectively. Spinal cord tissues were extracted on day 7 and 14, then Nogo-A, NgR and LINGO-1 protein expression in spinal cord tissues of rats were detected by Western blot. The mRNA expression of LINGO-1, NgR and Nogo-A were measured by real-time fluorescence quantitative PCR. Results After spinal cord injury, BBB scores of model rats was 0 on day 1; after 7 or 14 days of intervention, the BBB scores of each treatment group were significantly higher than those of model group(P<0.05), and the EA+blocking agent group was better than the EA group and blocking agent group(P<0.05). Compared with sham operation group at the same time point, the expression of each gene in model group significantly improved(P<0.05). Compared with the model group, the gene expression in each treatment group significantly decreased(P<0.05), at the same time, there was a significant difference in LINGO-1 gene expression between the EA+blocking agent group and the EA group(P<0.05). Conclusion Nogo/NgR signal pathway plays a key role in the process of nerve regeneration and repairment after spinal cord injury. Electro-acupuncture may be via inhibiting Nogo/NgR, LINGO-1 signal pathway to treat spinal cord injury in rats.

    Absorbed constituents in rat serum after oral administration of water extract of Simiao Yong’an Decoction

    Objective To study the absorbed constituents of water extract of Simiao Yong’an Decoction (SYD) in serum of rats, so as to find out the pharmacodynamics material basis. Methods Using liquid chromatograph-mass spectrometry (LCMS)-ion trap (IT)-time of flight (TOF) method under both positive and negative ion modes of electrospray ionization, the water extract of SYD, rats’ serum after oral administration of SYD and the blank serum were analyzed. Serum samples were pretreated by using methanol. The chromatographic condition consisted of Inertsil ODS-3 column, mobile phase of acetonitrile (A) and water (0.1% formic acid contained) (B) with gradient elution. The compounds absorbed in serum was identified by multiple stage mass spectrum fragments, published reports and standard substances. Results Finally, 14 compounds were preliminary identified in rats serum, of which ten compounds were prototypes of SYD and four were metabolites. Nine peaks were validated as harpagide, chlorogenic acid, loganin, sweroside, liquiritin, hyperoside, angoroside C, isochlorogenic acid C and harpagoside. Conclusion Constituents of SYD in serum were generated from compatibility of all herbal medicines. This study would contribute to the further study of pharmacodynamics properties and metabolism of SYD.

    Separation and identification of chemical constituents of Mongolian medicine compound Shudage-4

    Objective To study the chemical constituents of Mongolian medicine compound Shudage-4, so as to find out material bases of its pharmacodynamic effects. Methods The chemical constitutes of Shudage-4 were extracted using ethanol extraction method, and were isolated and purified using technologies of silica gel, Sephadex LH-20 and reversed phase silica gel. Then the structures of the isolated compounds were identified by using different spectroscopic and chromatographic techniques including ultra violet spectroscopy(UV), infrared spectroscopy(IR), mass spectrometry(MS) and nuclear magnetic resonance(NMR). Results There were 10 compounds isolated and identified from Mongolian medicine Shudage-4—1-phenyl-7-(3-methoxy-4-hydroxy) phenyl-5-ol-3-heptanone-one(1), stigmasterol (2), kaempferol(3), daucosterol(4), p-Anisic acid(5), ursolic acid(6), oleanic acid (7), dehydrocostus lactone(8), costunolide(9) and dehydrodiisoeugenol (10). Conclusion The ten compounds, except stigmasterol, were first isolated from Shudage-4. This paper provided important scientific data for the material bases of Mongolian medicine Shudage-4.

    Evaluation and formulation of Compound Danshen Adhesive Pellet

    Objective To evaluate chitosan, hydroxypropyl methyl cellulose K100M(HPMCK100M) and carbopol as adhesion materials in this design of Compound Danshen formula. By studying the adhesion in vitro and in vivo, an appropriate formula was screened.Methods In vitro, the minimum peel strength method was performed to determine the peel force of Adhesive Pellet from swine intestine, and the tissue residue method was applied to measure the number of remaining pellets in swine intestine. In vivo, the adhesive index was obtained by direct intragastric administration by which the number of remaining pellets in intestine were detected. Results The tissue residue method showed that the remaining pellets of adhesion materials accounting for 40% were more than those of adhension materials accounting for 30% for the same amount of time.The minimum peel strength method proved that the minimum pell strength was stronger in pellets of 40% adhension materials,conformable to the Results of the tissue residue method. In vivo the adhesive index of 40% of adhesion materials was less than that of 30%, but some pellets were left in swine stomach. Conclusion The same proportion of adhesive materials showed the same adhesion. The higher the proportion was, the stronger the adhesion was and the longer retention time in the stomach which could destroy the active ingredients in Compound Danshen Adhesive Pellet.

    Influence of Fructus Chebulae water solution on quality of Mongolian medicine Aconiti Kusnezoffii Radix Praeparata

    Objective To study the influence of different concentration of Fructus Chebulae (Hezi) water solution on the quality of Aconiti Kusnezoffii Radix Praeparata (Zhicaowu), so as to optimize the processing technology and to find out the principle of detoxification of Hezi in processing. Methods Taking Hezi water solution with different concentration as the excipients, pH and content of total tannins were determined. Then content of six alkaloids, indicator of toxic effect, in Zhicaowu were measured by reversed-phase HPLC. Results With the increasing of the concentration of Hezi water solution, the content of total tannins increased, pH decreased at first, then kept stable, and the total six alkaloids increased slowly followed by irregularity. When the content ratio of Caowu to Hezi was 2∶1, consistent with Standard of Inner Mongolia Mongolian Medicines, the total alkaloids content was least. Conclusion Concentration of Hezi water solution and storage time changed the content of alkaloids in Zhicaowu. The content ratio of Hezi to Caowu recorded in Standard of Inner Mongolia Mongolian Medicines (in 1986) was scientific and valid.

    TCM patterns types and indicators of thyroid cancer by Delphi method:an investigation

    Objective To use Delphi method to decide the TCM pattern types and indicators of thyroid cancer, so as to provide reference and basis for TCM clinical practice. Methods Based on the literature retrieving and TCM theory, two-round Questionnaire of TCM Patterns of Thyroid Cancer were drafted, and more than 30 experts answered the questionnaires in two rounds. The questionnaires were consisted of criterion of importance and familiarity of TCM Syndromes and tables of thyroid cancer, TCM pattern classification of pre-operation and post-operation, standard of TCM pattern decision. Then the two-round questionnaires were screened and analyzed,and the experts consensus of TCM patterns of thyroid cancer was initially established.Results In the first round, 33 questionnaire were returned with the positive coefficient of 100%, and 36 were returned in the second round, with the positive coefficient of 92.31%. Cronbaeh’s α coefficient of the first-round was only 0.297. After modification and revision, Cronbaeh’s α coefficient of the first-round increased to 0.696, a higher questionnaire reliability than the first round. Kendall’s coefficient of concordance(W) of the first-round was 0.214 as well as that of the second-round 0.382, which showed the experts converged towards the results. The process was stopped. TCM patterns of thyroid cancer were decided as stagnation due to phlegm and blood, stagnation of liver qi, deficiency of yin due to stagnantion of heat, deficiency of both vital qi and yin, deficiency of both vital qi and blood, the type of yang deficiency of the spleen and kidney. Stagnation due to phlegm and blood can mostly be observed in preoperative patients,while deficiency of both vital qi and yin can mostly be observed in postoperative patients.Conclusion The establishment of experts consensus of TCM patterns of thyroid cancer has a positive significance of promoting the standardization of pattern differentiation and clinical diagnosis and treatment.

    Inheritage and development of dispensing technology on ginseng from National Medical Master JIN Shiyuan

    Objective To collect and summarize the literature related to dispensing technology on ginseng from JIN Shiyuan, a National Medical Master, so as to study his academic ideas and to inherit the technology. Methods Retrieve the records and original works of Chinese herbs based on Compendium of Material Medica, and look up Chinese pharmacopoeia (2015 version) and JIN Shiyuan’s Experience of Identifying Chinese Herbal Medicines, then the present status of dispensing technology of ginseng was analyzed. Results The dispensing technology of ginseng was first recorded in Compendium of Materia Medica, including processing, dosage form, matters need attention, incompatibility, storage, etc. For the first time, JIN Shiyuan confirmed the clinical dispensing technology of ginseng, in which recognition, processing, prescription audit, prescription coping, prescription distribution, and storage were involved. Conclusion Pofessor JIN Shiyuan inherited pharmaceutical academic thought of LI Shizhen, and carried forward the clinical dispensing technology of ginseng.

Methods 67 patients with TIA were randomly divided into treatment group and control group. The patients of the control group took orally aspirin enteric-coated tablets 100 mg per night, in addition, those of treatment group underwent acupoint catgut embedding therapy with the embedding points of Fēngchí(GB 20), Dàzhuī (GV 14), Fēnglóng (ST 40).In order to understand the cerebral blood flow perfusion and curative effect,CT perfusion imaging was performed before and one month after treatment. Results After one month of treatment, the efficacy of the treatment group with effective rate of 91.18 % (remarkable effect 18 cases, effect 13 cases and inefficacy 3 cases) was significantly better than that of the control group with effective rate of 63.64 % (remarkable effect 10 cases, effect 11 cases, and inefficacy 12 cases). And the improvement of cerebral ischemia in CTPI imaging was better than that in the control group (P<0.01). Conclusion The positive therapeutic effect of acupoint catgut embedding therapy has been proved on TIA, which can significantly improve the patients with cerebral perfusion.
