    Professor Wang Yongyan’s theory of “yang(qi) deficiency generating wind” for the mechanism of stroke*

    This paper elaborated on “yang(qi) deficiency generating wind” as the stroke pathogenesis, proposed by Professor Wang Yongyan, from the following aspects: diagnosis & treatment, previous research, interpretation of wind in TCM, and understanding of this disease in Western medicine.From long-term clinical practice,Professor Wang Yongyan found that at the beginning of the stroke onset, qi deficiency even yang deficiency symptoms were commonly present in some patients, including flaccid limbs, lassitude,and pale complexion, which were aggravated upon physical exertion.Boosting qi and supplementing yang appears to be more effective than usual treatment methods. Professor Wang proposed this new pathogenesis theory from an imaging diagnostic process of “identifying causes from manifestations, identifying symptoms from causes, and identifying pattern from symptoms”. Differing from the previous views of “qi deficiency or yang deficiency inducing stroke” held by ancient TCM practitioners, Professor Wang’s theory is based on both clinical practice and TCM theory with reference to corresponding changes of hemodynamics and pathogenesis of ischemic cerebrovascular disease in Western medicine. The theory, yang(qi) deficiency generating wind, was developed as a new pathogenesis of stroke from the disease, symptoms, treatment, and clinical validation, so as to supplement and improve the core pathogenesis, “wind, fire, phlegm and blood stasis” for the acute phrase of stroke.

    Interpreting configuration of ancient Chinese characters—a new method to understand terms in Huangdi Neijing*

    Interpreting configuration of ancient Chinese characters means to divide characters into several parts, and infer the meaning of characters from the meanings of each part,the relationship between parts and the context. This method,characterized by tracing the original source and visualizing the characters, can be used to explain the meaning of Huangdi Neijing(Huangdi’s Internal Classic) text and its TCM terms vividly.It may contribute to a more accurate and profound understanding of the medical theory in Huangdi Neijing, and building a deep understanding of the thinking pattern of classification according to manifestation by ancient people.

    Revisiting the nomenclature of diaphragm qi deficiency by Zhang Zhongjing*

    The nomenclature of diaphragm qi deficiency was firstly mentioned in Shanghan Zabing Lun(Treatise on Cold-Induced and Miscellaneous Diseases) by Zhang Zhongjing. Most ancient and present scholars interpret the concept from the view of pectoral qi or stomach qi deficiency, which distorts the original meaning to some extent.Textual research and analysis of the origin of diaphragm qi indicates that this term is often used as a name or symptom of diseases.We analyzed the meaning of diaphragm qi deficiency with reference to diaphragm qi excess, then we reached a final conclusion that the concept of diaphragm qi deficiency refers to a pathological state that invisible heat stagnates in the diaphragm.Therefore,deficiency is not used to describe the insufficiency of healthy qi,but the description of the invisible heat.The major reason why diaphragm qi deficiency was interpreted as stomach qi deficiency or pectoral qi deficiency is because the general meaning of deficiency is often understood as insufficiency in TCM language.

    Effects of Bushen Tiaojing Fang and Xiaoyao Wan on the expression of oocyte secreted factors in mouse ovary before ovulation*

    Objective To investigate the possible mechanisms of ovulation induced by supplementing kidney and smoothing liver by observing the effects of Bushen Tiaojing Fang(Kidney-tonifying Menstruation-regulating Formula, BSTJF) and Xiaoyao Wan(Free Wanderer Pill, XYW) on the expression of growth differentiation factor-9(GDF-9), bone morphogenetic protein(BMP-15)mRNA and its protein expression.Methods Immature female mice were randomly divided into normal control group, kidney-tonifying(BSTJF) group and liver-soothing(XYW) group.All groups received intraperitoneal injection of 5 IU pregnant mare serum gonadotrophin(PMSG) at day 3 and 5 IU human chorionic gonadotropin(hCG) at day 5; mice in kidney-tonifying group and liver-soothing group received oral gavage of BSTJF (18.70 g/kg BSTJFⅡ from day 1 to day 3; 28.57 g/kg BSTJFⅢ from day 4 to day 5) or XYW (2.34 g/kg XYW from day 1 to day 3; 1.17 g/kg XYW from day 4 to day 5) respectively. GDF-9, BMP-15mRNA and its protein expression in ovary were measured with real-time fluorescence quantitative PCR, Western blot, and immunohistochemical analysis at the 0,8,12 hours after hCG injection. Results Compared with the normal control group, BSTJF and XYW increased the expressions of GDF-9, BMP-15mRNA and its protein at 8 and 12 hours after hCG injection(P<0.05). Conclusion Bushen Tiaojing Fang and Xiaoyao Wan may induce ovulation by promoting proliferation of ovarian granular cells, expansion of cumulus oophorus, and maturity of oocyte through increasing the expression of GDF-9, BMP-15mRNA and its protein.

    Experimental study on the different day-night expression of hypothalamus SCN Clock mRNA in mice under different light systems and Wendan Tang’s intervention*

    Objective To observe the characteristics of the hypothalamus SCN Clock mRNA expression in mice under four timing points in a day and light condition and the effects of Wendan Tang (Gallbladder-Warming Decoction, WDT). Methods 160 male KM mice of 2~3 months were randomly divided into normal group (6:00—18:00 light), dark group (24 h dark), model group (24 h light),WDT group, and melatonin group(n=32 in each group). Zeitgeber time (ZT) and Circadian time (CT) were used to indicate natural light and dark timing points respectively At the last day of the 12th week,8 mice from each group were randomly selected and sacrificed at natural light timing points of 6 a.m. (ZT0 / CT0), 12 (ZT6 / CT6), 18 (ZT12 / CT12), and 24 (ZT18 / CT18). The concentration of melatonin in serum was measured by using enzyme-linked immunosorbent assay (ELISA). The expression of SCN Clock mRNA in mouse hypothalamus was detected by using polymerase chain reaction (PCR) method. Results Serum melatonin: The melatonin level at ZT18 of mice in the normal group was the highest (P<0.05) among all groups. Serum melatonin level in mice of the dark group at CT0 was lower than those at CT18 and CT12 (P<0.05). There were no differences in the melatonin levels between four timing points in mice of the model group (P>0.05). Serum melatonin levels in mice of the WDT group were lower at ZT0 and ZT6 than that at ZT18 (P<0.05) while these values at ZT0, ZT6 and ZT12 were lower than that at ZT18 in mice of the melatonin group (P<0.05). Expression of SCN Clock mRNA in mice hypothalamus: The expression of SCN Clock mRNA was lower at ZT18 than that at ZT6 (P<0.05). No differences were found at the four timing points in mice of the dark group (P>0.05). The expression levels in mice of the model group were lower at ZT0 and ZT12 compared with ZT18 (P<0.05); also lower at ZT0 and ZT12 compared with ZT 6 (P<0.05). The expression levels in mice of WDT and melatonin groups at ZT18 were lower than those at ZT6 (P<0.05). Conclusion WDT is rich in melatonin and could adjust biological clock rhythm disorder and reduction of melatonin due to continuous light.

    Effects of Yiqi Huoxue Fang on necrosis and apoptosis of myocardial cell in rats with myocardial infarction*

    Objective To explore the protective effects of Yiqi Huoxue Fang(Qi-boosting Blood-activating Formula, YQHXF)on rat myocardium with myocardial infarction and its possible mechanism by observing the effects of YQHXF on necrosis and apoptosis of myocardial cell necrosis.Methods Male SD rats were randomly divided into the sham-surgery group, model group,YQHXF group, and perindopril group.Animal model of myocardial infarction was built by ligating coronary artery on the left anterior descending branch.Subsequently, YQHXF group received intragastric administration of Yiqi Huoxue Fang (compound medicine, 21 g/kg) once per day, and perindopril group was given perindopril (0.4 mg/kg) once a day;Sham-surgery group and model group received sterile distilled water without drug intervention.The blood in abdominal aorta was drawn, and the tissue on the peripheral area of the myocardial infarction was taken at two fixed time points (on the 7th and 28th day) after administration.Ultrasonic echocardiography was used to detect changes on cardiac morphology and function in rats: left ventricular ejection fraction (LVEF), left ventricular short axis ratio (LVFS), left ventricular end systolic diameter (LVIDs), left ventricular end-diastolic diameter (LVIDd); serum superoxide dismutase (SOD), glutathione peroxidase (gsh-px), creatine kinase (CK), lactate dehydrogenase (LDH), aspertate aminotransferase (AST), and troponin (cTnT) were measured with ELISA method. Protein imprinting method (Western Blot) was used to measure cytochrome C (CytC) and protein expression of cysteine aspartic acid and protease-3 (caspase-3) in the border zone of infarcted area. Results On the 7th day, compared with the model group, LVEF and LVFS in YQHXF group and perindopril group decreased without statistical significance(P>0.05). On the 28th day, compared with the model group, LVEF and LVFS in YQHXF group and perindopril group increased significantly(P>0.05).LVIDs and LVIDd decreased markedly(P<0.05); LVIDd in perindopril group decreased sharply (P<0.05), and decrease presented on LVIDs without statistical significace (P>0.05). On the 7th and 28th day, compared with the model group, CK-MB, LDH, AST, cTnT in YQHXF group and perindopril group decreased significantly (P<0.05), and SOD, GSH-PX increased significantly. On the 7th day,protein expression of cytC,caspase-3 decreased significantly in YQHXF group and perindopril group(P<0.05).Conclusion Administration of YQHXF at early stage can effectively improve myocardial infarction on cardiac structure and function in rats with myocardial infarction, improve oxidation resistance of myocardial cell, and inhibit myocardial apoptosis and necrosis.

    Gender specific study on psychological adjustment induced by baduanjin based on the beta band electroencephalogram power*

    Objective To analyze and explore the gender-dependent differences of the psychological adjustment induced by baduanjin(eight-trigram boxing) by comparing the total and spatial changes of β1 and β2 electroencephalogram(EEG) power among male and female college students before and after performing baduanjin.Methods College students were recruited as volunteers, and were divided into male and female groups.All volunteers received baduanjin training.β1 and β2 EEG power of two groups was measured and compared with NT9200 electroencephalograph before and after performing baduanjin. The results were collected to analyze the differences of absolute power in two groups before and after baduanjin training. Results General comparison:the absolute power of the β1 and β2 band in the female group was significantly higher than that in the male group before training (P>0.05). The absolute power of the β1 and β2 band increased significantly in the male group after training (P>0.01).There was no significant difference in the female group before training(P>0.05).In addition, there was no significant difference between male group and female group in the absolute power of the β1 and β2 band after training (P>0.05).Spatial comparison:there were significant differences in the distribution of β1 and β2 bands on the EEG between male and female groups before training. Compared with the distribution of β1 and β2 in different groups before training,the distribution of β1 on different groups tended to be consistent after training.The distribution of β2 band presented an increasing tendency on the whole brain region after training, while the female group presented decreasing tendency on most brain regions after training.Function coordination of left and right cerebral hemispheres:compared with EEG before training, the correlation between right and left cerebral hemispheres shown on β1 and β2 band increased significantly or changed from negative to positiv correlation. Conclusion Psychological adjustment may be induced by baduanjin,and it has the effects of removing differences of physiological movement between male and female volunteers. In other words, when students perform baduanjin, mental and physiological movement in brain presents with similar pattern.

    Effects of ultra-filtration extract from Danggui and Hongqi on immune function injury induced by 12C6+heavy ion beam radiationin H22 tumor-bearing mice*

    Objective To observe the effects of ultra-filtration extract from Danggui (Radix Angelicae Sinensis, Chinese Angelica) and Hongqi (Radix Hedysair, Sweetvetch Root) on immune dysfunction caused by 12C6+ heavy ion beam radiation in H22 tumor-bearing mice. Methods Tumor-bearing mice model was built by subcutaneously injecting H22 cells. Blood leukocytes count was used to determine the optimal radiation dose of immune injury induced by heavy ion beam irradiation and the optimal dose of ultra-filtration extract from Danggui and Hongqi. The mice were divided into normal control group, tumor model group, drug intervention group (6.72 g/kg), radiation group (4Gy), and drug (6.72 g/kg) plus radiation (4Gy) group. Spleen index and thymus index were measured in each group; Changes of T lymphocyte in each group was detected by using flow cytometry; Levels of serum C3, IFN-γ and IL-4 were tested with ELLSA; Spleen pathological and morphological changes were observed under optical microscope.Results Compared with the model group, blood leukocyte count, spleen index, thymus index, levels of CD3+, CD4+ and CD8+ T lymphocytes, serum C3, IFN-γ and IL-4 in radiation group decreased significantly (P<0.05), and ratio of CD4+/CD8+ and IFN-γ/IL-4 in radiation group decreased significantly (P<0.05).Pathological morphology indicated that there was no significant difference between the model group and normal control group. Spleen structure was normal; most of the splenic sinus dilated and developed blood stasis.Splenic corpuscle was clear. The lymphatic sheath around the artery was present, and macronuclear cells could be observed in the model group.Whereas in the radiation group, the spleen body and red pulp spleen cord shrank; the number of lymphocyte and megakaryocytes reduced, and fibrous tissue proliferated. Compared with the radiation group, blood leukocytes count, spleen index, thymus index, the levels of CD3+, CD4+ and CD8+ T lymphocytes, serum C3, IFN-γ and IL-4 in drug plus radiation group increased significantly (P<0.05), and the ratio of CD4+/CD8+ and IFN-γ/IL-4 in drug plus radiation group increased markedly (P<0.05). Compared with the radiation group, pathological changes of spleen in the drug plus radiation group alleviated significantly. Conclusions The ultra-filtration extract from Danggui and Hongqi can could alleviate the immune function injury caused by 12C6+ heavy ion beam radiation in H22 tumor-bearing mice.

    Protective effects of Jiedu Tongluo Fang on rats with diabetic cardiomyopathy*

    Objective To explore the possible mechanism of Jiedu Tongluo Fang(Toxin-removing collateral-unblocking Formula, JDTLF) in the treatment of diabetic cardiomyopathy by observing its effects on the expression of peroxisome proliferator activated receptor gamma(PPARγ)and cardiac function in rats with diabetic cardiomyopathy. Methods 100 Wistar rats were randomly divided into normal control group (n=8) and diabetic model group (n=92). Diabetic model was built by using intraperitoneal injection of streptozotocin (STZ, 55 mg/kg). Diabetic cardiomyopathy(DCM) model was built by feeding those rats for another 12 consecutive weeks.DCM model rats were randomly divided into JDTLF group,Qingre JieduYao(heat-clearing toxin-removing medicinal, QRJDY)group, pioglitazone group, and DCM model group.Except the DCM model group and the normal control group, the other 3 groups were orally gavaged with 10.4 g/(kg·d) JDTLF,2.7 g/(kg·d) QRJDY,and 2.7 mg/(kg·d) pioglitazone respectively. DCM group and NC group received distilled water for 6 weeks. At the end of the experiment, random blood glucose (RBG), blood lipid(TC, TG, LDL-C, and HDL-C), high sensitive C reactive protein (hs-CRP), cardiac function(LVSP,LVEDP,+dp/dtmax,-dp/dtmax and t-dp/dt) and morphological changes were evaluated. Expression of PPARγ in myocardium was measured with immunohistochemistry. Results Compared with the NC group, body weight in DCM group decreased significantly (P<0.01); Random blood glucose,hs-CRP,TC, TG and LDL-C increased significantly(P<0.01,P<0.05); HDL-C decreased significantly (P<0.05),and cardiac function decreased significantly; LVSP, +dp/dtmax,-dp/dtmax decreased significantly; LVEDP and t-dp/dt increased significantly (P<0.05). There was a significant decrease in the expression of myocardial PPARγ; Compared with DCM model group, random blood glucose in JDTLF group and QRJDY group decreased significantly;Hs-CRP,TC, TG and LDL-C decreased significantly (P<0.01,P<0.05), HDL-C increased significantly(P<0.05); Cardiac function improved in JDTLF group and QRJDY group; LVSP, +dp/dtmax and -dp/dtmax increased significantly (P<0.05); LVEDP and t-dp/dt decreased significantly (P<0.05); Expression of myocardial PPARγ presented an increasing tendency (P<0.01). Conclusion Jeidu Tongluo Fang and Qingre Jiedu Yao can significantly improve the glucose and lipid metabolism, heart function and heart tissue structure, and have protective effect on diabetic cardiomyopathy.Its mechanism may be related to the up-regulation of PPARγ.

    Protective effects of hedysarum polybotrys polysacchcaide on renal interstitid fibrosis in db/db mice with early diabetic nephropathy*

    Objective To investigate the influence of hedysarum polybotrys polysacchcaide (HPS) on the expression of transforming growth factor-β1 (TGF-β1) and connective tissue growth factor (CTGF) as well as its kidney-protective mechanism in early diabetic nephropathy (DN) db/db mice. Methods 60 male mice at SPF 6 were used for this study. The above 50 db/db mice were divided into 5 groups (n=10 in each group) by using the random number table method: high-dose, mid-dose and low-dose HPS group (200, 100, 50 mg/ kg, ig), telmisartan group (5 mg/kg, ig), and model group (distilled water of the same volume, ig). The remaining 10 db/m mice in the normal control group received equal volume of distilled water via intragastric administration for 8 weeks. For mice in all groups, blood glucose levels were measured before the experiment and once every two weeks after intervention. 24h urine was collected at the end of week 4 and 8 to measure urinary albumin levels by using ELISA method. Blood samples were drawn from the eyeball veins to measure the levels of serum creatinine (Scr) and blood urea nitrogen (BUN). Rat kidney tissue was dissected after sacrifice of these mice. Left kidney underwent HE staining to observe pathological changes in the renal cortex while the right kidney was used to detect the expression of TGF-β1 and CTGF mRNA and protein by using RT-PCR and Western blotting. Results Compared with the normal control group, blood glucose, Scr, BUN and 24 h urinary albumin levels increased significantly (P<0.01) in the model group; glomerular hypertrophy, mesangial widening, and capillary basement membrane thickening were observed. Expressions of TGF-β1 and CTGF mRNA and protein were significantly increased (P<0.01). Compared with the model group, blood glucose, Scr, BUN and 24 h urinary albumin levels decreased significantly (P<0.05) in all treatment groups except the low-dose HPS group (P>0.05); glomerular hypertrophy, mesangial widening and capillary basement membrane thickening presented with different degrees of improvement; expressions of TGF-β1 and CTGF mRNA and protein significantly decreased (P<0.01); curative effect of the mid-dose HPS group was superior to that of the telmisartan group. Conclusion HPS could prevent early diabetic nephropathy (DN) in db/db mice; its mechanism may be related to the inhibition of TGF-β1 and CTGF mRNA expression level in the kidney.

    Quality representation and correlation analysis of Shanzha based on phenolic characteristic spectrum of medical system*

    Objective To establish quality representation and correlation analysis method based on the spectrum of medicinal system of Shanzha(Fructus Crataegi, Chinese hawthorn fruit),so as to evaluate quality of Shanzha decocting pieces effectively and accurately. Methods HPLC-PDA method was applied to characterize quality of spectrum of Shanzha medicinal system; quality of 11 batches of Shanzha decocting pieces were characterized based on the number and chemical type of peaks on characteristic spectrum.The content of characteristic index components, protocatechuic acid, epicatechin,chlorogenicacid, phenolic acids (represented by protocatechuic acid and chlorogenic acid) and flavonoids (represented by hyperoside and epicatechin) were characterized by indexes in characteristic spectrum. Correlation analysis of quality and quantity of Shanzha was then performed with reference to the standard reference pieces.Results As the reference substance, characteristic spectrum of batch 1 contains 13 characteristic peaks, 7 peaks of flavonoids and 6 peaks of phenolic acids.All characteristic peaks appeared on the chromatograms of 11 batches of Shanzha.Batch 5,10, 8, 2,6 had more effective index ingredients. After comprehensive evaluation,there was a high correlation between batch 6, 4, 3, 7, 8, 2 and standard reference pieces. The quality of batch 5, 10, 8, 2, 7, and 3 ranked on the top among all samples.Conclusion HPLC method to characterize quality and quantity of characteristic spectrum of 11 batches of Shanzha is simple and accurate. The evaluation mode of quantity and quality and their correlation based on Shanzha medical system, including systemic correlation and application validity, can evaluate the quality of hawthorn pieces effectively and accurately.

    Objective To enrich the clinical meanings of common pulse manifestations, and to explore mechanisms of those pulses by analyzing the correlation between pattern elements of disease nature and common pulse manifestations in patients with viral hepatitis or cirrhosis based on the combination of pattern and diseases. Methods A national multicenter cross-sectional epidemiological survey was conducted to collect TCM symptoms, tongue manifestations and pulse conditions(including depth, frequency, width, length, hardness, and frequency of pulse)of patients with viral hepatitis or cirrhosis. The survey questionnaire used the Information Collection form of Vital Hepatitis Cirrhosis drafted by the research group. Standard of Hepatitis Cirrhosis Pattern Elements Differentiation was made to identify patterns and pattern elements.Descriptive statistics such as frequency and rate, Chi-square test,and correlation analysis were used.Results Pulses of 801 patients were collected. Pulses with occurrence rate above 20% was defined as common pulse manifestations, in which wiry pulse accounted for 75.2% (n=602);thready pulse,29.0 % (n=279);deep pulse 29% (n=232); and slippery pulse, 23.1% (n=185). There was positive correlation between wiry pulse and blood stasis and qi stagnation (correlation coefficient was 0.141 and 0.166 respectively,P<0.05).There was positive correlation between slippery pulse and yin deficiency and damp heat (correlation coefficient were 0.093 and 0.131 respectively,P<0.05). There was positive correlation between deep pulse and qi deficiency and yang deficiency(correlation coefficient was 0.099 and 0.111 respectively, P<0.05). There was positive correlation between thready pulse pulse and yin deficiency,qi deficiency, yang deficiency and water retention(correlation coefficient was 0.089,0.109,0.105 and 0.075 respectively, P<0.05). Conclusion There is a certain correlation between pattern elements and common pulse manifestations in viral hepatitis cirrhosis patients.

    Effects of herbal fumigation therapy combined with meibomian gland massage on diabetic dry eyes*

    Objective To explore new therapy for diabetic dry eyes through herbal fumigation therapy combined with meibomian gland massage.Methods 84 cases (168 eyes) of diabetic dry eyes in 2016 from ophthalmology outpatient department of Eye Hospital, affiliated to China Academy of Chinese Medical Sciences,were randomly divided into two groups (n=42 in each group). The control group was given artificial tear eyedrops, while treatment group was given herbal fumigation therapy and meibomian gland massage. One month later, symptoms,tear film breakup time(BUT), results of Schirmer I test(SIT), and changes of meibomian gland function were determined.Results In the experiment group, new therapy was remarkably effective on 56 eyes, effective on 20 eyes, and ineffective on 8 eyes, and the effective rate was 90.5%; In the control group, artificial tear eyedrops was markedly effective on 20 eyes, effective on 36 eyes, and ineffective on 28 eyes, and the effective rate was 66.7%. There was a significant statistic difference of symptom score, BUT, SIT, and meibomian gland function between the two groups after treatment (P<0.05). The effects of treatment group was superior to that of control group.Conclusion Herbal fumigation therapy accompanied with meibomian gland massage appears to be effective for diabetic dry eye syndrome.
