    Characteristics and clinical significance of pulse condition of regularly intermittent pulse*

    The characteristics and clinical significance of pulse condition of regularly intermittent pulse are discussed based on carding and summarizing the knowledge on regularly intermittent pulse in ancient books and combining cardiovascular theory in Western medicine. As an independent pulse condition, regularly intermittent pulse is found to be characterized by a regular long pause between heartbeats, with pulse rate being normal, fast or slow. Clinically, regularly intermittent pulse can be observed in patients with visceral dysfunction, panic of seven emotions, traumatic injury, wind pattern, pain patterns and heart diseases. It can also be seen in pregnancy women and health people with preternatural capacity. It will be more accurate in clinic to describe bound-intermittent pulse with the text as follows: bound pulse and regularly intermittent pulse appear alternately, pulsing at regular or irregular intervals.

    Relationship between birth years of earthly branches and life span: studied based on theory of five circuits and six qi*

    Objective To investigate the relationship among different birth years of earthly branches, controlling heaven qi and terrestrial effect qi and life span based on TCM theory of five circuits and six qi. Methods The data of died cases (n=6 020) was collected from the Yantai Yuhuangding Hospital affiliated to Medical College of Qingdao University from Feb. 2002 to Jan. 2015. The difference in life span was given Kruskal-Wallis H in statistical test for discussing the discrepancy in life span in the cases born in different years with different earthly branches, controlling heaven qi and terrestrial effect qi. Results The difference in life span had statistical significance in the cases born in different years of earthly branches and controlling heaven qi (P<0.05). The order of length of life span in cases born in different years of earthly branches was as follows: zi year>chou year>wu year>wei year>si year>chen year>hai year>mao year>shen year>you year>yin year>xu year. The order of length of life span in cases born in different years of controlling heaven qi was as follows: Shaoyin junhuo year>Taiyin shitu year>Yangming zaojin year>Shaoyang xianghuo year>Jueyin fengmu year>Taiyang hanshui year. The difference in length of life span in cases born in different years of terrestrial effect qi had no statistical significance (P>0.05). Conclusion Different birth years of earthly branches and controlling heaven have some influence on length of life span. The average age of cases born in zi year, chou year, wu year and wei year of earthly branches qi is above 68-year-old and has an extend trend, which is coincide with that average age of cases born in Shaoyin junhuo year and Taiyin shitu year of controlling heaven qi is longer than that of cases born in other years of controlling heaven qi. It is inferred that the life span of a person born in Shaoyin junhuo year or Taiyin shitu year of controlling heaven qi can be improved, and controlling heaven qi in birth year has more influence on life span than terrestrial effect qi

    There were different records about the author of Tuiqiu Shiyi (Ascertaining My Teacher’s Ideas) in the bibliographic books in past dynasties. Some of them referred to WANG Ji, and others to DAI Yuanli. Today’s publications and bibliographic books all refer to DAI Yuanli. But the preface of Tuiqiu Shiyi indicated that DAI Yuanli referred to the author was a guess at that time. General Catalogue of Complete Library of Four Branches of Literature · Ascertaining My Teacher’s thought that this book was one of Zhengzhi Yaojue (Important Tips to Analysis and Treatment of Patterns), Zhengzhi Leifang (Analogous Formulas for Analysis and Treatment of Patterns) and Leizheng Yongyao (Usage of Medicinal for Treating Analysis Patterns) written by DAI Yuanli. But the comparison among them shows that there are a few similarities between Tuiqiu Shiyi and Michuan Zhengzhi Yaojue Ji Leifang (Esoteric Important Tips and Analogous Formulas for Analysis and Treatment of Patterns). Also it is not unlikely that Tuiqiu Shiyi is a book in WANG Ji’s name. The author of Tuiqiu Shiyi requires further investigation.

    Influence of Jiawei Shaoyao Gancao Tang and its medicated serum on estrogen, estrogen receptor and aromatase P450 in treatment of adenomyosis*

    Objective To investigate the deep mechanism of Jiawei Shaoyao Gancao Tang (Modified Peony and Licorice Root Decoction) and its medicated serum in treatment of adenomyosis. Methods The cells in ovarian focus tissue were given in vitro primary culture in 6 patients with adenomyosis undergone hysterectomy, and the focus cells were identified by using immunocytochemistic method. There were 6 groups in this study including high-dose Jiawei Shaoyao Gancao Tang group (high-dose JSGT group, 20 mg/mL), low-dose Jiawei Shaoyao Gancao Tang group (low-dose JSGT group, 10 mg/mL), Jiawei Shaoyao Gancao Tang medicated serum group (medicated serum group), blank serum group, mifepristone group and blank control group. The focus cells were intervened for 24 h and 48 h respectively in all groups, and then the expression of estrogen (E2) was detected by using enzyme-linked immunosorbent assay (ELISA). The expressions of estrogen receptor (ER) and aromatase P450 (P450arom) were detected by using immunocytochemistic method in all groups. Results The identification results of immunocytochemistic method showed that collected from 6 patients and successfully cultured cells were all focus cells of adenomyosis. The level of E2 decreased significantly in medicated serum group compared with blank control group and blank serum group (P<0.05). The level of ER decreased significantly in high-dose JSGT group, low-dose JSGT group, medicated serum group and mifepristone group after 24 h (P<0.05). The total efficacy of decreasing ER was inferior in mifepristone group to that in high-dose JSGT group, low-dose JSGT group and medicated serum group (P<0.05), and efficacy of decreasing ER was superior after 24 h to that after 48 h in high-dose JSGT group (P<0.05). The level of P450arom decreased significantly in high-dose JSGT group, low-dose JSGT group, medicated serum group and mifepristone group after 24 h compared with blank serum group and blank control group (P<0.05), and the efficacy decreasing P450arom was better in high-dose JSGT group, low-dose JSGT group and medicated serum group than that in mifepristone group after 24 h (P<0.05). The efficacy decreasing P450arom was superior after 24 h to that after 48 h in low-dose JSGT group (P<0.05). Jiawei Shaoyao Gancao Tang’s efficacies of decreasing ER and P450arom were better than mifepristone and these efficacies showed timeliness: the efficacies were better after 24 h than that after 48 h. There was no dose dependence. Conclusion Jiawei Shaoyao Gancao Tang can reduce the levels of ER and P450arom, cut off the positive feedback loop of E2 and P450arom and weaken E2 effect, which may be its mechanism in treatment of adenomyosis.

    Influence of Pixu Fang 1 (Spleen-deficiency Formula 1) on protein and gene expressions of myosin light chain in rats with functional dyspepsia-spleen deficiency pattern*

    Objective To investigate the intervention of Pixu Fang 1 (Spleen-deficiency Formula 1) to the protein and mRNA expressions of myosin light chain (MLC) in gastric tissue in rats with functional dyspepsia (FD)-spleen deficiency pattern (SDP). Methods SD rats (n=60, aged 7 d) were randomly divided into normal group (n=10) and FD-SDP model group (n=50). The normal group was orally given 2% sugar solution and FD-SDP model group was given 0.1% iodoacetamide sugar solution in a dose of 0.2 mL each rat a day for 6 d. The FD-SDP model group was fed with normal forage until rats aged 6 weeks and additionally given modified multiple platform method (MMPM) continuously for 14 d. After modeling, FD-SDP model group was divided randomly into model group (distilled water, 10 mL/kg·d), domperidone group (3.125 mg/kg·d), low-dose Pixu Fang 1 group (low-dose group, 1.275 g/kg·d), mid-dose Pixu Fang 1 group (mid-dose group, 2.550 g/kg·d) and high-dose Pixu Fang 1 group (high-dose group, 5.100 g/kg·d, each n=10), and all groups were given orally corresponding medicinal for 14 d. The protein and mRNA expressions of MLC in gastric tissue were detected by using immunohistochemistry technique, Western blotting and reverse transcription-polymerase chain reaction (RT-PCR). Results Compared with normal group, MLC protein expression decreased in model group detected by immunohistochemistry technique and Western blotting, and MLC mRNA expression decreased in model group detected by RT-PCR (P<0.01). Compared with model group, MLC protein expression increased in low-dose group, mid-dose group and high-dose group detected by immunohistochemistry technique (P<0.05, P<0.01). MLC protein expression increased in mid-dose group and high-dose group detected by Western blotting (P<0.01). MLC mRNA expression increased in high-dose group detected RT-PCR (P<0.01). Conclusion There are decreases of protein and mRNA expressions of MLC in FD-SDP rats, and Pixu Fang 1 can up-regulate these expressions.

    Anti-depressant effect and mechanism of Peiyuan Jieyu Fang (Tonifying-original Qi Releasing-depression Formula) in several mouse models of depression*

    Objective To study the anti-depressant effect and mechanism of Peiyuan Jieyu Fang (Tonifying-original Qi Releasing-depression Formula). Methods The reserpine depression model was established by intraperitoneal injection of reserpine (2.5 mg/kg) in mice, acute stress depression model, by forced swimming test (FST) in mice, and chronic stress depression model, by long-term bandage (14 d) and noise in mice. The influences of Peiyuan Jieyu Fang on body temperature, ptosis and rate of overstepping bound in reserpine depression model, and on immobility time of FST in acute stress depression model and chronic stress depression model were observed. The content of brain 5-hydroxytryptamine (5-HT), noradrenalin (NA) and indoleamine-2, 3-dioxygenase (IDO) in acute stress depression model, and content of 5-HT and IDO in chronic stress depression model were detected by using enzyme-linked immunosorbent assay (ELISA). The expression of IDO mRNA was detected by using fluorescence quantitative polymerase chain reaction (PCR) in chronic stress depression model. Results The decrease of body temperature was antagonized in all Peiyuan Jieyu Fang groups (abbreviated as high-dose group, mid-dose group and low-dose group) in reserpine depression model (P<0.01). The ptosis and akinesia induced by reserpine were relieved in high-dose group (P<0.05), and akinesia was relieved in low-dose group (P<0.05). The immobility time of FST was shortened (P<0.05, P<0.01), and content of 5-HT and NA were significantly increased in acute stress depression model in mid-dose group and low-dose group (P<0.05, P<0.01). The immobility time of FST was shortened in varying degrees, content of 5-HT was increased, content of IDO was decreased and IDO mRNA expression was up-regulated in chronic stress depression model in high-dose group, mid-dose group and low-dose group (P<0.05). Conclusion Peiyuan Jieyu Fang has some relief effects on depression symptoms induced by reserpine, FST and chronic stress in mice. The mechanism may be related to that Peiyuan Jieyu Fang can increase the content of 5-HT and NA, inhibit IDO activity, and down-regulate IDO mRNA expression.

    Heart protective effect of Qihong Fang (Wolfberry and Hawthorn Decoction) in aging rats with cold-congealing and blood-stasis pattern*

    Objective To discuss the influence of Qihong Fang (Wolfberry and Hawthorn Decoction) on heart in aged rats with cold-congealing and blood-stasis pattern. Methods SD rats (n=48) were divided randomly into normal control group, model group, vitamin E group (13.5 mg/kg), low-dose Qihong Fang group (45 mg/kg), mid-dose Qihong Fang group (90 mg/kg) and high-dose Qihong Fang group (180 mg/kg). The aged rat model of cold-congealing and blood-stasis pattern was established through 28-d ozone leading to aging and 14-d ice water leading to cold-congealing and blood-stasis pattern in other 5 groups except of control group. On the 29th d, the blood samples were collected from abdominal aorta for isolating serum and plasma, and heart was separated too. The vitalities of serum total superoxide dismutase (T-SOD), Cu-Zn SOD, Mn-SOD, catalase (CAT), glutathione peroxidease (GSH-PX), content of malondialdehyde (MDA), and prothrombin time (PT), activated partial thromboplastin time (APTT), thrombin time (TT) and fibrinogen (FIB) were detected. The morphologic changes of heart cells were observed after HE staining, fiber deposition quantity was observed after Masson staining, expression of transforming growth factor β1 (TGF-β1) was detected by using immunohistochemistry technique for reviewing aging model of cold-congealing and blood-stasis pattern, and protective effect of Qihong Fang on heart was observed in rats. Results Compared with normal control group, the vitalities of T-SOD, Cu-Zn-SOD, Mn-SOD, CAT and GSH-PX decreased significantly (P<0.05), MDA content increased significantly (P<0.05), PT and APTT decreased significantly (P<0.05), and FIB content increased significantly (P<0.05) in model group. After Masson staining, morphometry observation by light microscope showed deposition of heart collagen fibers increased significantly (P<0.05), and expression of TGF-β1 increased significantly (P<0.05). Compared with control group, the vitalities of T-SOD, Cu-Zn-SOD, CAT and GSH-PX increased significantly (P<0.05), MDA content decreased significantly (P<0.05), PT and ATPP increased significantly (P<0.05), FIB content decreased significantly (P<0.05), deposition of heart collagen fibers decreased significantly (P<0.05), and expression of TGF-β1 decreased significantly (P<0.05) in mid-dose and high-dose Qihong Fang groups. Conclusion Qihong Fang has a protective effect on heart in aged rats with cold-congealing and blood-stasis pattern.

    Influences of sonophoresis of Fufang Sanqi Xiaotong (Compound Notoginseng Pain-relieving) Ointment on synovial morphology and expressions of chondrocyte caspase-9 and XIAP in rabbits with knee osteoarthritis*

    Objective To observe the influences of sonophoresis of Fufang Sanqi Xiaotong (Compound Notoginseng Pain-relieving) Ointment on changes of synovial morphology and expressions of chondrocyte caspase-9 and X-linked inhibitor of apoptosis protein (XIAP) in rabbits with knee osteoarthritis (KOA). Methods New Zealand white rabbits (n=32, 6 months old) were randomly divided into group of sonophoresis of Fufang Sanqi Xiaotong (sonophoresis group), group of coupling agent ultrasound treatment (coupling agent group), model group and normal group (each n=8). Except of normal group, other groups received anterior cruciate ligament transaction (ACLT). After 6 weeks, sonophoresis group and coupling agent group were given corresponding therapies for 2 treatment courses and model group and normal group were not given any interventions. The changes of synovial histomorphology were observed with light microscope after HE staining, and expressions of caspase-9 and XIAP were detected by using immunohistochemistry technique in all groups after 2 treatment courses. Results The degeneration degree of synovial membrane under light microscope showed an ascending order in all groups as follows: normal group, sonophoresis group, coupling agent group and model group. The value of integral optical density (IOD) of caspase-9 was higher, and IOD value of XIAP was lower in sonophoresis group, coupling agent group and model group than those in normal group. The IOD value of caspase-9 was lower, and IOD value of XIAP was higher in sonophoresis group and coupling agent group than those in model group. The IOD value of caspase-9 was lower, and IOD value of XIAP was higher in sonophoresis group than those in coupling agent group. Conclusion The sonophoresis of Fufang Sanqi Xiaotong is superior to ultrasound treatment, and has the protective effect on cartilage and relief effect on synovial inflammation, and can delay KOA development. The mechanism may be related to decrease of caspase-9 expression and increase of XIAP expression.

    Chemical compositions of Mongolian medicine compound formula-Shudage-4 in rat serum*

    Objective To establish in vivo and in vitro the characteristic chromatograms of Mongolian medicine compound formula-Shudage-4, and affiliate and identify in vitro chemical compositions of Shudage-4 and its compositions in vivo absorbed into blood. Methods Wistar rats were intragastrically given Shudage-4 and its different single medicinal, and then serum samples were prepared and determined by using high performance liquid chromatography (HPLC). The HPLC characteristic chromatograms of in vitro samples of Shudage-4 and its different single medicinal and Shudage-4 medicated serum were established and analyzed. The chemical compositions in vitro of Shudage-4 and its compositions in vivo absorbed into blood after taken by rats were compared, affiliated and identified. Results There were totally 64 peaks found after analyzing in vitro HPLC characteristic chromatograms of Shudage-4. There were 23 chemical compositions absorbed into blood, and 17 in vitro ones absorbed directly into blood and 6 speculated as new metabolites. These 23 chemical compositions were all derived from different were derived from Gaoliangjiang (Rhizoma Alpiniae Officinarum, Galangal Rhizome), peak 14, from Muxiang (Radix Aucklandiae, Common Aucklandia Root) and peak 13, from Shichangpu (Rhizoma Acori Tatarinowii, Grassleaf Sweetflag Rhizome). The peaks 15, 20, 21 and 22 were derived from Gaoliangjiang, Muxiang and Shichangpu, peaks 6, 10, 17, 18 and 19, from Gaoliangjiang and Shichangpu, peaks 7 and 16, from Gaoliangjiang and Muxiang, peak 11, from Muxiang and Shichangpu, and peaks 4, 5, 8 and 12, from the whole formula of Shudage-4, from which 7 chemical compositions were identified. Conclusion The chemical compositions and metabolites in blood may be the active principles of Shudage-4 with direct effects in vitro, which are derived respectively from different single medicinal in Shudage-4.

    Volatile constituents of Linglingxiang (Lysimachia foenum-graecum Hance, Holy Basil) analyzed by applying solid phase microextraction-gas chromatography-mass spectrometry*

    Objective To analyze the volatile constituents of Linglingxiang (Lysimachia foenum-graecum, Hance, Holy Basil) by applying solid phase microextraction-gas chromatography-mass spectrometry (SPME-GC/MS). Methods The heads of solid phase microextraction (SPME) with 4 kinds of different coating, 75 μm CAR/PDMS, 65 μm DVB/PDMS, 85 μm PA and 100 μm PDMS, were used to extract the under the best extraction condition, and then these volatile constituents were detected by using gas chromatography-mass spectrometry (GC-MS). Results The key parameters of SPME for extracting volatile constituents from Linglingxiang were as follows: temperature was 85℃ and extracting time was for 50 min. A total of 10 types including 103 kinds of volatile constituents were identified by 4 different SPME extraction heads, and phenols, esters and hydrocarbons were main chemical types of volatile compounds in Linglingxiang. There were totally 6 volatile constituents detected by 4 different SPME extraction heads, and they were Sugarlactone, Dihydroactinidiolide, (+)-cedrol, phytone, phenanthrene, and 3-Amino-4, 5-dimethyl-2(5h)-furanone. Among all extraction heads, the head of 75 μm CAR/PDMS detected the largest number of volatile constituents reached to 43. Conclusion SPME-GC/MS was used for the first time form determining the volatile constituents in Linglingxiang. The method is simple, rapid and accurate, which can offer reference to further studies on the material base of Linglingxiang.

    Technique optimization for extracting the total alkaloids from Baiqucai (Herba Chelidonii, Greater Celandine) by response surface analysis*

    Objective To optimize the best technique for extracting the total alkaloids from Baiqucai (Herba Chelidonii, Greater Celandine). Methods The yield of the total alkaloids was taken as review index by using acid dye colorimetry, and Plackett-Burman design (PBD) test was applied at first for screening 3 main factors influencing the technique of extracting the total alkaloids from Baiqucai and they were ethanol volume fraction, solid-liquid ratio and extracting times. Then the best technologic conditions for extracting the total alkaloids from Baiqucai were determined by applying response surface analysis (RSA) of Box-Behnken design (BBD) test. Results The best extracting technique was as follows: ethanol volume fraction was 85%, extracting duration was 60 min, extracting temperature was 70℃, degree of grinding was 40 meshes, solid-liquid ratio was 1: 15, and extracting times were 3 times. In these conditions, the yield of the total alkaloids from Baiqucai was up to 1.92%, which was basically the same as theoretical prediction (1.95%). Conclusion The extracting technique optimized in this test is reasonable and feasible, and can get higher content of the total alkaloids

    Influence of heat-clearing dampness-resolving stasis-dispelling therapy on JAK/STAT signaling pathway mediated by Leptin in patients with chronic renal failure-damp-heat pattern*

    Objective To observe the influences of Qingshen (Kidney-Clearing) Granules, with effects of clearing heat, resolving dampness and dispelling stasis, on expressions of Janus-activated kinase 2 (JAK2) and signal transducers and activator of transcriptions 3 (STAT3) in Janus-activated kinase-signaltransducer and activator of transcription (JAK/STAT) signaling pathway mediated by Leptin in patients with chronic renal failure (CRF)-damp-heat pattern, and investigate the mechanism of Qingshen Granules in treatment of CRF. Methods The patients with CRF-damp-heat pattern (n=68) were randomly divided into treatment group and control group (each n=34). The study was completed actually in 60 patients, and 29 in treatment group and 31 in control group. All 2 groups were given basic treatment of Western medicine, and treatment group was additionally given Qingshen Granules (thrice daily and 1 bas every time) for 12 weeks. The clinical efficacy and changes of serum creatinine (SCr), estimated glomerular filtration rate (eGFR), serum Leptin, JAK2 and STAT3 were observed in 2 groups. Results The clinical efficacy was 86.21% (25/29) and total effective rate of TCM patterns was 82.76% (24/29) in treatment group, and 58.06% (18/31) and 54.84% (17/31) in control group (P<0.05). After treatment, the level of SCr decreased significantly (P<0.01) and level of GFR increased significantly (P<0.01) in treatment group compared with those before treatment, which were superior to those in control group at the same term (P<0.05). After treatment, the level of Leptin, and expressions of JAK2 and STAT3 decreased significantly (P<0.01) in treatment group compared with those before treatment, which were superior to those in control group at the same term (P<0.05 or P<0.01). Conclusion Qingshen Granules can reduce the level of Leptin and expressions of JAK2 and STAT3, inhibit the activation of JAK/STAT signaling pathway mediated by Leptin, relieve clinical symptoms and improve kidney function in patients with CRF-damp-heat pattern.

    Cluster analysis on TCM patterns in patients with thyroid cancer after surgery*

    Objective To investigate the distribution features of TCM patterns and differentiation method of thyroid cancer through cluster analysis on data of TCM four examinations after surgery. Methods The data of TCM four examinations were collected from 304 patients by applying cross-section epidemiological survey, and then given quantitative classification and a database was established. The quantitative scores of each patient were given cluster analysis by using sample clustering analysis method (Q clustering) for getting the proportion of each category of four examination information. The preliminary criterion for patterns of thyroid cancer was made according to the proportion of four examinations information and expert survey of Delphi method at earlier stage, which was combined with clinical practice to determine the patterns. Results All 304 patients were clustered into 9 categories indicated by from A1 to A9. The frequency of A1 was 77 cases; A2, 7; A3, 1; A4, 7; A5, 105; A6, 7; A7, 87; A8, 11 and A9, 2. Based on clinical practice and experts’ experiences, A1 and A7, A5 and A6, and A3 and A4 were merged and evaluated respectively as pattern of dual deficiency of qi and yin (164 cases, 53.9%), pattern of liver depression and qi stagnation (112 cases, 36.8%) and spleen-kidney yang deficiency pattern (8 cases, 2.6%). A2 was evaluated as pattern of yin damaged by stasis heat (7 cases, 2.3%). The proportions of four examination information of A8 and A9 were disperse and evaluated as other patterns (13 cases, 4.4%). Conclusion The distribution of TCM patterns in the patients with thyroid cancer after surgery has its regularity. It is feasible to evaluate TCM patterns with cluster analysis combining with experts’ experiences.

    Pathogenesis differentiation and treatments of gouty nephropathy*

    The obstruction of kidney collaterals by turbid and blood stasis is a characteristic pathogenesis of gouty nephropathy, which runs throughout the whole process of the disease. The pathogenesis of disease of gouty nephropathy is different from that of other chronic kidney diseases, determines the occurrence and development direction of patterns and symptoms, and is the common pathogenesis behind different patterns. The pathogenesis of pattern is the main body of pattern differentiation and treatment of gouty nephropathy, damp-heat obstruction and spleen-kidney deficiency and is the pathogenesis base of same treatment for different diseases. The pathogenesis of symptoms is a direct pathogenesis induced by symptoms. In treatment of gouty nephropathy, symptomatic treatment can improve the therapeutic effects based on disease and pattern differentiations.
