    Differentiation and treatment of rheumatoid arthritis according to theory of latent pathogens*

    Rheumatoid arthritis (RA) takes on the characteristics of diseases due to latent pathogens. Essential qi deficiency is an internal factor, and ordinary factors of cold-dampness or damp-heat are preconditions of diseases due to latent pathogens. The six excesses are external pathogenic factors and pathogens of wind-cold and damp-heat are main pathogenic causes. The clinical characteristics of diseases due to latent pathogens are reflected in hiding and incubation, and aggravating factors will cause the diseases attack. The theory of latent pathogens can guide the diagnosis and treatment of RA, including avoiding six excesses and early treatment for getting rid of latent pathogens, and tonifying healthy qi and regulating constitution for truncating and reversing disease through starting treatment at asymptomatic stage, which reflects the thought of preventive treatment of disease. It is of great significance to improve clinical efficacy, decrease the occurrence of bone destruction and reduce disability rate

    Clinical characteristics of specific symptoms of water-a factor related to disease nature*

    Objective To propose the confirmatory method for water-a factor related to disease nature. Methods Based on a symptom-pathogenesis database of Chinese medicine, the clinical characteristics of some symptoms that their pathogenesis can and only can be directly explained by water were screened. Results Water maybe excessive and dilute pathological products existing in skin, pleural cavity and abdominal cavity or exhausted from urinary passage and anus because of metabolic disorders. It is related to the decrease of assimilation or re-assimilation of humor. Water, as a factor related to disease nature, has some specific symptoms as (skin) dropsy sunken when pressed, (pleural cavity, abdominal cavity and etc.) hydrops, clear abundant (urine) and watery (stool). Conclusion The evidences for conforming pattern factors are the clinical characteristics of specific symptoms, and pathogenesis explanation is the basis that can establish the close relationship between a pattern factor and clinical characteristics of symptoms. The disease nature can be directly diagnosed as water when any one of above clinical characteristics of specific symptom appears. Water is a pattern factor.

    Identification of receptacle and qi in Huangdi Neijing (Huangdi’s Internal Classic)*

    The theoretical system of traditional Chinese medicine (TCM), with Huangdi Neijing (Huangdi’s Internal Classic) as the core, is influenced by the traditional academic thought and swings between receptacle and qi. Based on analysis on the culture background of creative era of Huangdi Neijing and application of word-receptacle in its text, the symbolism (actual receptacle with qi as essence), functionality (in organs with organic qi) and practicability (creating receptacle and regulating qi by receptacle) of receptacle are extracted and compared with qi theory. The influence of mainstream academic thought rheology on TCM development is illustrated in the subsequent history, and differentiation between receptacle and qi in Huangdi Neijing and its practical significance are summed up.

    Regulation effects of different disassembled formulas of Huiyang Shengji Fang on macrophage phenotype transition*

    Objective To observe the influence of Huiyang Shengji Fang(Restoring Yang and Promoting Tissue Regeneration Decoction, HYSJF) and its decomposed formulas-Yiqi Wenyang Fang(Qi-supplementing and Yang-warming Decoction, YQWYF) and Huoxue Tongluo Fang (Blood-activating and collateral-freeing Decoction, HXTLF) on macrophage phenotype inversion, and discuss the mechanism of YQWYF and HXTLF in regulating macrophage phenotype inversion during the period of chronic skin ulcer. Methods THP-1 cells were stimulated and transformed into macrophages with PMA. After stimulated with LPS and INF-γ for 48 h, the macrophages were transformed into M1 macrophages. Lovastatin (10 μmol /L) was taken as control drug. The influence of YQWYF and HXTLF on activities of macrophages was observed by using CCK-8 method, and their influence on the phagocytosis of macrophages was detected by using neutral red uptake assay (NRU). The expressions of iNOS mRNA and Arg-1 mRNA were detected by using polymerase chain reaction (PCR) after M1 macrophages stimulated with lovastatin, YQWYF and HXTLF for 24 h. The levels of supernate TNF-α, IL-6, IL-12, IL-10 and VEGF were detected by using CBA method. The level of supernate TGF-β was detected by using ELISA. Results YQWYF, in dose of 4 mg/L (low-dose YQWYF group), 0.8 mg/L (mid-dose YQWYF group) and 0.16 mg/L (high-dose YQWYF group), and HXTLF, in dose of 32 mg/L (low-dose HXTLF group), 6.4 mg/L (mid-dose HXTLF group) and 1.28 mg/L (high-dose HXTLF group) had no influence on proliferation of macrophages but improved phagocytosis of NRU of macrophages. The expression of iNOS mRNA of M1 macrophages increased significantly after stimulated by LPS and INF-γ, and decreased significantly in lovastatin group, YQWYF group and HXTLF group compared with model group (P<0.01). The expression of Arg-1 mRNA of M2 macrophages decreased significantly after stimulated by LPS and INF-γ compared with normal group, was improved significantly in lovastatin group, YQWYF group and HXTLF group compared with model group (P<0.01), and decreased significantly in YQWYF group and HXTLF group compared with lovastatin group. After M1 macrophages stimulated by above drugs for 24 h, the levels of TNF-α, IL-6 and IL-1 decreased significantly, and levels of VEGF and TGF-β increased significantly. The level of IL-10 increased significantly in high-dose YQWYF group (P<0.05), The levels of IL-10, VEGF and TGF-β had statistical difference in HXTLF group and high-dose YQWYF group compared with lovastatin group. Conclusion YQWYF and HXTLF can inhibit expression of iNOS mRNA of M1 macrophages and induce expression of Arg-1 mRNA of M2 macrophages, and can inhibit the secretion of M1 macrophage cytokine and improve secretion of M2 macrophage cytokine.

    Influence of Chaihu Longgu Muli Tang on mobilization of bone marrow c-kit+ stem cells in rats with myocardial infarction*

    Objective To investigate the influence of Chaihu Longgu Muli Tang (Chinese Thorowax Root, Bone Fossil of Big Mammals and Oyster Shell Decoction, CLMT) on the mobilization of bone marrow c-kit+ stem cells in rats after myocardial infarction (MI). Methods Male SD rats were randomly divided into sham-operation group, model group and CLMT group, and each group was divided again into 3-d sub group, 7-d subgroup and 14-d subgroup. The CLMT group was intragastrically given CLMT, and other groups were orally given distilled water. The heart function was reviewed by using echocardiogram, changes of myocardial pathology were reviewed after HE staining, and severity of myocardial fibrosis was observed by after Masson staining. The number of c-kit+ positive cells in bone marrow and peripheral blood was detected by using flow cytometry (FCM), and number of c-kit+ positive cells in MI border zone was detected by using immunohistochemistry technique. Results The heart function declined gradually along with MI time extension in model group, and difference in heart function had statistical significance compared with sham-operation group at all time points (P<0.05). The heart function trended to stable in CLMT group and was better than that in model group, and difference in LVEF and LVFS had statistical significance compared with model group at time points of 7 d and 14 d (P<0.05). Pathological staining showed that myocardial cells were in alignment without collagen fibers in sham-operation group, necrosis and fibrosis of myocardial cells in model group that aggravated gradually along with MI time extension. The severity of myocardial necrosis and fibrosis was milder in CLMT group than that in model group. The number of c-kit+ positive cells in bone marrow and peripheral blood reached peak in model group and CLMT group 7 d after MI, and difference number of c-kit+ positive cells had statistical significance compared with sham-operation group, and was higher in CLMT group than that in model group. The difference in number of c-kit+ positive cells had statistical significance between 7-d subgroup and 14-d subgroup (P<0.05). The number of c-kit+ positive cells in MI border zone was higher in model group than that in sham-operation group, and difference in number of c-kit+ positive cells had statistical significance between 7-d subgroup and 14-d subgroup (P<0.05). The number of c-kit+ positive cells in MI border zone was higher in CLMT group than that in model group, and difference in number of c-kit+ positive cells MI border zone had statistical significance between 7-d subgroup and 14-d subgroup (P<0.05). Conclusion CLMT can improve the mobilization of bone marrow c-kit+ stem cells after MI and heart function.

    Influence of Tiaozhi Tongmai Granule on blood fat, RLP-C, sICAM-1 and sVCAM-1 in ApoE-/- mice*

    Objective To investigate the influence of Tiaozhi Tongmai (Fat-regulating and Vessel-freeing) Granule (TZTMG) on blood fat, triglyceride-rich remnant-like particle (RLP) and cell adhesion molecule in ApoE-/- mice. Methods After established model of atherosclerosis (AS), ApoE-/- mice (n=30) were randomly divided into model group, fenofibrate group and TZTMG group (each n=10). C57BL/6J mice (n=10) were chosen into normal group. All medicinal groups were fed with high-fat diet, meanwhile were intervened with TZTMG and fenofibrate. The indexes of triglyceride-rich RLP (RLP-C) and 4 items of blood fat were detected detector after 12 weeks. The levels of serum intercellular adhesion molecule-1 (ICAM-1) and vascular adhesion molecule-1 (VCAM-1) were detected by using ELISA. The aortic root was collected to prepare paraffin sections for observing morphological changes after HE staining. Results Since the 4th week, mouse weight decreased in TZTMG group and fenofibrate group compared with model group. The levels of serum RLP-C, TC, TG, HDL-C and LDL-C increased in model group, fenofibrate group and TZTMG group compared with normal group (P<0.05). The levels of serum RLP-C and TG decreased in fenofibrate group and TZTMG group compared with model group (P<0.05). The levels of ICAM-1 and VCAM-1 increased significantly in model group, fenofibrate group and TZTMG group compared with normal group (P<0.05). The levels of serum ICAM-1 and VCAM-1 decreased in fenofibrate group and TZTMG group compared with model group (P<0.05). Comparison in VCAM-1 among normal group, fenofibrate group and TZTMC group had no statistical significance. HE staining showed there were thickened aortic intima and vulnerable plaques in model group, and thickened aortic intima and stable plaques in TZTMG group. Conclusion TZTMG can decrease the expression of adhesion molecule and delay AS development through reducing RLP-C level.

    Preparation technology for liposome of new aconitine*

    Objective To study and optimize the preparation technology and prescription of liposome of new aconitine aiming at reducing toxicity, improving efficiency, and being suitable for industrial production with higher stability. Methods The liposome of new aconitine was prepared by using freeze-drying method, and its prescription and preparation technology were optimized by using single-factor experiment. The encapsulation efficiency (EE) and particle size were taken as review indexes, the EE of liposome of new aconitine was determined by using centrifugation and centrifugation conditions were investigated. Results The best prescription included that ratio of lecithin to cholesterol was 3: 1, ratio of drug to lipid was 1: 20, ratio of sugar to lipid was 5: 1, and ratio of organic solvent to water was 1: 3. The centrifugal conditions for determining EE were 2 000 r/min and centrifugation time was for 5 min. Under these conditions, the free drug could be separated from the drug-containing liposome. Conclusion The liposome of new aconitine obtained by this technology has reasonable prescription, feasible process and higher EE and stability.

    Prediction on transport properties in vivo of oligopeptides peptide in Zhima based on computer-assisted proteolysis*

    Objective To predict transport properties in vivo of oligopeptides in Zhima (Sesame, Sesamum indicum) based on computer-assisted proteolysis. Methods Main protein sequences in Zhima were collected, protein simulated digestive hydrolysis was conducted with computer-assisted proteolysis, and obtained oligopeptides were taken to establish virtual structure database. A support vector machine (SVM) model of Pep T2 substrate was established. Through data treating and parameter optimization to select the bast modef for predicting Pep T2 substrate components in Zima.Results There were 346 oligopeptides in the database of Zhima oligopeptides and 45 descriptors obtained after screening. SVM model of PepT2 substrate was established and its accuracy, sensitivity and specificity were all higher than 85%. The analysis of receiver operating characteristic curve (ROC) showed that area under curve (AUC) was greater than 0.9. The prediction of the database of Zhima oligopeptides by using SVM model of PepT2 substrate showed that 36% Zhima oligopeptides had latent activities of transporting by PepT2. Conclusion A database of Zhima oligopeptides is established by using computer-assisted proteolysis. The SVM model of PepT2 substrate is used to predict transport properties of Zhima oligopeptides, and it is accuracy, stable and sensitive with high specificity. The study provides a new method for the researches on absorption properties of oligopeptides components in Chinese materia medica.

    Influence of electroacupuncture on blood fat and volume of cerebral infarction and mechanism exploration in rats with hyperlipidemia complicated by cerebral ischemia*

    Objective To discuss the influence of electroacupuncture (EA) on blood fat and volume of cerebral infarction in rats with hyperlipidemia complicated by cerebral ischemia, and explore the mechanism. Methods SD rats (n=78) were randomly divided into normal group (n=6), and model group, sham-operation group, EA group 1 and EA group 2 (each n=18). Except of normal group, other groups were given high fat diet for 6 weeks for establishing hyperlipidemia model. After 6 weeks, EA group 1 was given EA in bilateral Fenglong (ST40) once a day for 7 d. On the 50th d, the model of middle cerebral artery occlusion (MCAO) was established in alll groups except of rormal group, and then EA group 1 and EA group 2 were given EA in Baihui (GV20) and bilateral Fenglong (ST40) once a day. The neurological function and volume of cerebral infarction were detected 1 d after treatment. The levels of total cholesterol (TC), triglyceride (TG), high-density lipoprotein (HDL) and low-density lipoprotein (LDL) were detected 1 d, 7 d and 14 d after treatment. The protein expressions of β-NGF and TrkA were detected by using Western blotting assay in diseased brain. Results The levels of TC, G and LDL increased and level of HDL decreased after treatment (P<0.01, P<0.05). Compared with model group, level of TG decreased in EA groups after treatment for 7 d (P<0.01), and level of LDL decreased in EA group 1 after treatment for 14 d (P<0.05), which had no significant in EA group 2. Compared with model group, neurological deficit score (NDS) decreased in EA group 1 after treatment for 1 d (P<0.05), and left volume of cerebral infarction decreased in EA groups (P<0.01), which was smaller in EA group 1 than that in EA group 2(P<0.05). Compared with normal group, expressions of β-NGF and TrkA had an ascending trend in model group after treatment for 1 d, and recover to normal after treatment for 7 d. Compared with model group, expressions of β-NGF and TrkA had an ascending trend in EA group 1 and continuously increased after treatment for 14 d, which was more significantly in EA group 1. Conclusion EA can improve neurological function, reduce volume of cerebral infarction and promote expressions of β-NGF and TrkA through regulating blood fat level in hyperlipidemia stage, and its regulation effect is better than that of acupuncture after cerebral ischemia. The possible mechanism is related to that GV20 and ST40 can cmprove expressions of β-NGF and TrkA.

    Kinematic parameter characteristics of treading manipulation based on three-dimensional dynamic capture technique*

    Objective To study the kinematic characteristics of Luo’s treading manipulation, and provide evidence-based data reference for unique manipulations of prominent senior experts. Methods Three complete treading motions manipulated by Professor Luo Caigui and beginners were recorded by using 3D motion capture technique, and their motions of hip, knee and ankle joints were recorded by using high speed camera. The dynamic changes of angle, angular speed and barycentric velocity were observed in the total cycle and at important time points, and overall and local kinematic characteristics of Luo’s treading manipulation were analyzed. Results Compared with beginners, the overall cycle characteristics of Professor Luo’s treading motions showed that the difference in angles of hip and ankle joints had statistical significance (P<0.01). The local characteristics showed that angular speed of knee joint changed slightly at the moment of barycenter descending to the lowest point in cycle 1 (P<0.05), and hip angle was smaller (P<0.05) and angular speed of ankle joint changed slightly (P<0.01) in cycle 3. At the moment of barycenter ascending and knee joint straightening, angular speeds of knee joint and ankle joint changed slightly (P<0.01) and barycentric velocity was smaller (P<0.01) in cycle 1. The angular speeds of hip joint and knee joint changed slightly (P<0.05), and angle of ankle joint was smaller (P<0.05) in cycle 2, and angular speeds of hip joint and knee joint changed slightly (P<0.05) in cycle 3. At the moment of barycenter descending and maximum flexion of knee joint, Professor Luo’s angle of knee joint was larger, angular speed changed slightly (P<0.01), and barycentric velocity changed slightly (P<0.05) in cycle 1. The barycentric velocity was smaller (P<0.05) in cycle 2, and angles of hip joint and knee joint were smaller (P<0.01) in cycle 3. Conclusion Professor Luo’s motions have obvious local characteristics, i.e. both angles and angular speeds of joints change slightly during treading period, and at the same time of keeping body stable, his motions are constant, steady, dexterous and strengthened associated activity and inertia. Luo’s treading manipulations lay emphasis not only on overall stable, but also on dexterous and trigger force, and reflect the characteristics of associating activity and inertia and combining softness and hardness.

    Correlation between waist hip ratio and carotid atherosclerosis in women aged over 40 with high risk of stroke and study on TCM pattern manifestations*

    Objective To investigate the correlation between waist hip ratio (WHR) and carotid atherosclerosis and TCM pattern manifestations in women aged over 40 with high risk of stroke. Methods The perimenopausal population with high risk of stroke (n=629) was chosen and divided, according to quartile of WHR, into 4 groups, and general clinical data and incidence of carotid atherosclerosis were compared among 4 groups. Taken group I as reference, risk of occurring carotid atherosclerosis was analyzed in other 3 group. The correlation between WHR and incidence of carotid atherosclerosis was studied by using binary Logistic regression analysis. Meanwhile, the population was divided into menopausal group and non-menopausal group, and WHR, incidence of carotid atherosclerosis, TCM symptoms and features of tongue and pulse were compared between 2 groups. Results The indexes of age, family history of stroke, menopausal cases, body mass index (BMI), overweight cases, triglyceride (TG) and high-density lipoprotein (HDL) had significant difference among 4 groups (P<0.05). The incidence of carotid atherosclerosis increased successively in 4 groups and difference had statistical significance (P<0.01). Compared with group I, risk of occurring carotid atherosclerosis was by 1.348 times, 1.927 times and 2.305 times in other 3 groups (P<0.01). WHR was correlated to incidence of carotid atherosclerosis after controlled other variables (P<0.01). WHR was higher (P<0.05) and incidence of carotid atherosclerosis increased significantly (P<0.01) in menopausal group compared with non-menopausal group. The TCM pattern manifestations of qi deficiency, yin deficiency, yang deficiency and blood stasis were significantly more in menopausal group than those in non-menopausal group (P<0.05 or P<0.01). The pattern identification of viscera and bowels involved liver, spleen and kidney, and disease nature showed deficiency complicated by excessiveness and deficiency was the basis. Conclusion The risk of carotid atherosclerosis will be higher when WHR increases in women aged over 40 with high risk of stroke. WHR can be taken as an indicator of carotid atherosclerosis. The treatment with TCM is based on pattern identification of liver, spleen and kidney, and disease nature shows deficiency complicated by excessiveness and deficiency is the basis.

    TCM constitutions and depression-anxiety status in crew of high-speed rail trains of Beijing Railways: a survey and a correlation study*

    Objective To survey TCM constitutions in crew of high-speed rail trains of Beijing Railways, and analyze the correlation between different constitutions and depression-anxiety status. Methods TCM constitutions and depression-anxiety status were surveyed through cluster sampling and questionnaire by using Zung self-rating depressive scale (Zung’s SDS) and Zung self-rating anxiety scale (Zung’s SAS) in crew of high-speed rail trains of Beijing Railways. The characteristics of TCM constitutions and correlation between TCM constitutions and depression-anxiety status were analyzed. Results The crew with yang deficiency constitution (including those with accompanied yang deficiency constitution) accounted for 47.0%, those with qi deficiency constitution (including those with accompanied qi deficiency constitution), 42.3%, with qi depression constitution (including those with accompanied qi depression constitution), 40.9%, with yin deficiency constitution (including those with accompanied yin deficiency constitution), 31.7%, with phlegm-dampness constitution (including those with accompanied phlegm-dampness constitution), 30.6%, with blood stasis constitution (including those with accompanied blood stasis constitution), 28.1%, with damp-heat constitution (including those with accompanied damp-heat constitution), 14.6%, with specific constitution (including those with accompanied specific constitution), 12.1%, and with normal constitution, 3.9%. All constitutional types were significantly correlated to depression-anxiety status (P<0.05). The incidence of anxiety was lower in crew with normal constitution, and increased significantly in those with 8 deflective constitutions including qi deficiency, yang deficiency, yin deficiency, phlegm-dampness, damp-heat, blood stasis, qi depression and specific constitutions (P<0.005 or P<0.001). The risk of depression in those with qi deficiency constitution was 2 times as much as in those without qi deficiency constitution (OR=2.03, 95%CI: 1.05-3.94). Qi deficiency constitution was significantly correlated to anxiety (OR=1.95, 95%CI: 1.07-3.56). Conclusion The yang deficiency constitution (including those with accompanied yang deficiency constitution) and qi deficiency constitution (including those with accompanied qi deficiency constitution) are most common constitutions in crew of high-speed rail trains of Beijing Railways. The adjustment of these two constitutions may be a thinking train and method for controlling depression-anxiety status in crew of high-speed rail trains of Beijing Railways, which can be an important content in occupational health guardianship.

    Medication rules of anti-tumor formulas containing marine Chinese medicinals*

    Objective To investigate the rules of medication and combinations of anti-tumor formulas containing marine Chinese medicinals. Methods The anti-tumor formulas containing marine Chinese medicinal were collected from Marine Chinese Medicinal and Effective formulas, Dictionary of Chinese Marine Medicinals, Marine Chinese Materia Medica, and databases including CNKI. A database was established for data mining by using V2.5 software of TCM inheritance support system (TCMISS). Results There were totally 251 anti-tumor formulas containing marine Chinese medicinals involved 417 Chinese medicinal including 36 marine Chinese medicinals. The nature of medicinals in these formulas were mainly cold, neutral and warm, flavors of them were mainly bitterness, pungency, sweetness and saltiness, and they mainly entered into meridians of liver, lung, stomach, spleen and heart. The analysis of association rules showed that core combination was Muli (Oyster Shell, Concha Ostreae)-Haizao (Seaweed, Sargassum)-Kunbu (Kelp, Thallus Laminariae), which was commonly combined with medicinals with effects of clearing heart, tonifying, resolving phlegm, dissipating binds, activating blood and resolving stasis in clinical practice. Entropy clustering analysis showed that there were 12 combinations with core medicinals, and they were Haizao-Haigeqiao (Clam Shell, Goncha Meretricis seu Cyclinae)-Kunbu, Zaojiaoci (Chinese Honeylocust Spine, Spina Gleditsiae)-Chuanshanjia (Pangolin Scales, Squama Manitis)-Jiangcan (Batryticated Silkworm, Bombyx Batryticatus), Muxiang (Common Aucklandia Root, Radix Aucklandiae)-Walengzi (Ark Shell, Concha Arcae)-Puhuang (Cattail Pollen, Pollen Typhae), Chansu (Toad Venom, Venenum Bufonis)-Xionghuang (Realgar, Realgar)-Zhenzhu (Pearl, Pernulo), Huangbai (Amur Cork-Tree, Cortex Phellodendri)-Yunaoshi (Fish Otolith, Asteriscus Pseudosciaenae)-Bingpian (Bomeol, Bomeolum Syntheticum), Haima (Sea Horse, Hippocampus)-Qingpi (Green Tangerine Peel, Pericarpium Citri Reticulatae Viride)-Qianniuzi (Pharbitis Seed, Semen Pharbitidis), Haizao-Kunbu-Gancao (Liquorice Root, Radix Glycyrrhizae), Wangbuliuxing (Vaccaria Seed, Semen Vaccariae)-Chuanshanjia-Muli, Fengfang (Hornet Nest, Nidus Vespae)-Walengzi-Qianxie (Scorpion, Scorpio), Shexiang (Musk, Moschus)-Xuejie (Dragon’s Blood, Risina Draconis)-Zhenzhu, Huangbai (Amur Cork-Tree, Cortex Phellodendri)-Ruxiang (Frankincense, Olibanum)-Bingpian, and Qingpi-Wulingzhi (Trogopterus Dung, Faeces Trogopterorum)-Dafupi (Shell of Arecanut, Pericarpium Arecae). There were 6 new combinations, and they were Haizao-Haigeqiao-Kunbu-Gancao, Zaojiaoci-Chuanshanjia-Jiangcan-Wangbuliuxing-Muli, Muxiang-Walengzi-Puhuang-Fengfang-Qianxie, Chansu-Xionghuang-Zhenzhu-Shexiang-Xuejie, Huangbai-Yunaoshi-Bingpian-Ruxiang, and Haima-Qingpi-Qianniuzi-Wulingzhi-Dafupi. The high-frequency tumor diseases, totally 47, treated with anti-tumor formulas containing marine Chinese medicinals, were successively thyroid tumor, lung cancer, liver cancer, uterine fibroids, nasopharyngeal carcinoma, esophageal cancer, gastric cancer and mammary cancer. Conclusion There are rules to follow in treatment of tumors with marine Chinese medicinal. The different therapeutic effects can be achieved by different medicinal combinations, which has a very important value in guiding clinical medication and new medicinal development.

    Treatment of stenosal tendosynovitis with percutaneous micro-invasive release therapy by using push-cut knife: a clinical trial*

    Objective To evaluate the effectiveness and safety of percutaneous micro-invasive release therapy by using push-cut knife on stenosal tendosynovitis. Methods The patients with stenosal tendosynovitis were chosen according to inclusion criteria, and then divided, based on random digital table and 1∶1 of random concealmert, into push-cut knife group and needle-knife group. The push-cut knife group was given push-cut knife therapy one time, and was given traditional needle-knife one time. The main outcome index was cure rate on the 27th week after treatment, and secondary indexes included pain scores, joint range of motion (ROM) and trigger indications. The safety indexes included complications and adverse events. Results There were totally 120 patients were chosen, but one quitted due to serious disease in needle-knife group, and 119 patients completed the study and were brought into the statistical analysis of outcome indexes. On the 27th week after treatment, there were 55 cases (91.67%) cured in push-cut knife group and 39 (66.10%) cured in needle-knife group (P<0.001). The indexes of trigger indications, pain and ROM were significantly relieved in 2 groups, and improved more significantly over time. The comparisons at the time points of the 1st, 3rd, 6th, 12th and 27th weeks showed that, the reliefs of trigger indications, pain and ROM were superior in push-cut knife group than those in needle-knife group (P<0.05). There were no adverse events or complications occurred in 2 groups. Conclusion The push-cut knife has higher one-off cure rate in treatment of stenosal tendosynovitis. The signs of pain, ROM and trigger indications are relieved immediately after treatment, and continuously relieved after half year. There are no adverse events occurred.
