    Ontological view of nature in Huangdi’s Internal Classics:on genuine qi*

    Philosophy is the soul of the medical system in Huangdi’s Internal Classics while theory of qi is the core of its philosophical system. The traditional “qi monism” believed to be held by this classics, however, seems to be ambiguous when examined thoroughly. Ontological perspective of “genuine qi” and duality of yin and yang qi are presented instead. “Genuine qi” is the master of life; it is without the character of yin and yang, rise and fall, pure and turbid. The concepts of “kidney qi”, “meridian qi” and “visceral essence” in the theory of visceral manifestation are the embodiment of “genuine qi”.

    Object and task of exegetical studies on medical words in ancient books*

    The words in ancient medical books and medical terms in non-medical books are together named as medical words. Medical words are mainly stored in ancient medical books and ancient non-medical books. The medical words in classic medical books of the Han Dynasty have important foundation and demonstration effects on the later generations in terms of word-formation materials, word formation ways and diachronic uses. Dictionaries, as non-medical books, in all ages have preserved the maximum medical words. Medical words are basic objects of exegesis of Chinese medicine. The exegesis materials of medical words in the past are mainly kept in the main body of medical books as explanations, annotations of ancient books and dictionary interpretations. Different exegesis ways have their own advantages and disadvantages. The medical words in bamboo slips, silk books and sounds and meanings of Buddhist sutras are also worthy of attention. There are still many gaps and inaccuracies in medical lexical exegesis, including annotation of medical books, compilation of dictionaries and textual research of words and phrases. The tasks of medical lexical exegesis include study on pragmatic and stored meanings of medical words, study on modifiers and emphases of medical characters, study on medical nomenclatures, study on meaning changes of Chinese medical words in the process of translation, study on compilation and interpretation of medical and non-medical dictionaries, and study on history of medical book exegesis. All these studies are very important to exegesis, lexicology and terminology of Chinese medicine.E-mail:lionw@vip.sina.com

    Influence of Shenzhilin Oral Solution on expressions of hippocampal α7-nAChRs and mGluR5 in APP/PS1 double transgenic mice*

    Objective To discuss the influence of Shenzhilin (Ginseng, Cassia Twig and Indian Bread) Oral Solution (SZL) on expressions of hippocampal α7-nAChRs and mGluR5 (neuronal synapse signaling related proteins) in APP/PS1 double transgenic mice. Methods Male APP/PS1 double transgenic mice (3 months old) were randomly divided into 5 groups: model group, donepezil group [0.92 mg/(kg·d)], high-dose SZL group [50 g/(kg·d)], mid-dose SZL group [25 g/(kg·d)] and low-dose SZL group [12.5 g/(kg·d)]. Wild type C57BL/6J mice with the same age and background were chosen into normal group. After gavage for 3 months continuously, behavioral platform test was carried out in all groups, and then hippocampal tissue samples were collected for detecting the expressions of α7-nAChRs and mGluR5 in hippocampal CA1 zone by using Western blotting assay and immunohistochemistry technique. Results The results of Western blotting assay showed that size of protein bands of α7-nAChRs were significantly narrower and color was lighter (P<0.01), and size of protein bands of mGluR5 were wider and color was darker (P<0.01) in model group than those in normal group. Compared with model group, size of protein bands of α7-nAChRs were wider and color was darker (P<0.05 or P<0.01), and size of protein bands of mGluR5 were narrower and color was lighter (P<0.05 or P<0.01) in all medicated groups. The positive cell expressions of α7-nAChRs and mGluR5 detected by using immunohistochemistry technique were consistent with those detected by using Western blotting assay. Conclusion SZL may play an anti-dementia role through regulating the key synaptic proteins in hippocampal CA1 zone, improving synapse function for slowing down brain damage due to Alzheimer disease, and maintaining memory acquisition ability.

    Influence of different species of Haizao and modified administration of Gancao in Haizao Yuhu Tang on oxidative stress and liver Nrf2/HO-1 pathway in rats with goiter*

    Objective To investigate the influence of different species, Haihaozi (Alga Sargassi Pallidi, Sargassum pallidum) and Yangqicai (Alga Sargassi Fusiformis, Sargassum fusiforme), of Haizao (Algae, Sargassum) and modified administration of Gancao (Liquorice Root, Radix Glycyrrhizae) in Haizao Yuhu Tang (Algae Okho Decoction, HZYH Decoction) containing antagonism combination on oxidative stress and liver Nrf2/HO-1 pathway in rats with goiter. Methods All rats (n=144) were randomly divided into blank group, model group, positive drug euthyrox group (euthyrox group), HZYH Decoction with Yangqicai group, HZYH Decoction with Haihaozi group, HZYH Decoction with Yangqicai without Gancao group, HZYH Decoction with Haihaozi without Gancao group, HZYH Decoction without Haizao group, and HZYH Decoction without Haizao and Gancao group. Except of blank group, all other groups were orally given PTU for copying goiter model. Euthyrox was taken as positive control drug. The content of serum ALT, AST, MDA, H2O2, XOD, SOD and GSH-Px were detected in all groups. The expressions of Nrf2 mRNA HO-1 mRNA in liver tissue were detected. Results The content of MDA increased and expression of Nrf2 mRNA decreased in model group compared with blank group (P<0.01). The content of ALT and expression of HO-1 mRNA increased in HZYH Decoction with Haihaozi without Gancao group (P<0.05, P<0.01) and expression of HO-1 mRNA increased in HZYH Decoction with Haihaozi group (P<0.01). The content of ALT, AST, MDA and H2O2 increased, and GSH-Px content and expression of Nrf2 mRNA decreased in HZYH Decoction with Yangqicai without Gancao group and HZYH Decoction with Yangqicai group (P<0.05, P<0.01). The content of AST increased and expression of HO-1 mRNA had no significant changes in HZYH Decoction with Yangqicai group compared with HZYH Decoction with Yangqicai without Gancao group (P<0.05, P<0.01). The content of H2O2 increased and expression of HO-1 mRNA had no significant changes in HZYH Decoction with Yangqicai without Gancao group (P<0.05, P<0.01). The content of ALT, AST and H2O2 increased and expression of HO-1 mRNA had no significant changes in HZYH Decoction with Yangqicai without Gancao group compared with HZYH Decoction with Haihaozi group (P<0.05, P<0.01). The content of AST increased and expression of HO-1 mRNA had no significant changes in HZYH Decoction with Yangqicai group (P<0.01). Conclusion The modified administration of different species of Haizao (Haihaozi/Yangqicai) in HZYJ Decoction, and HZYH Decoction with Haihaozi group and HZYH Decoction with Haihaozi without Gancao group can protect liver tissue to some extent from peroxide injury. The mechanism may be related to activation of Nrf2/HO-1 pathway and improvement of antioxidant ability of liver cells. The liver function is affected in HZYH Decoction with Yangqicai group and HZYH Decoction with Yangqicai without Gancao group, which may be related to disturbance of oxidant-antioxidant system and failure of activating Nrf2/HO-1 pathway.

    Treatment of yin-pattern wound with Huiyang Shengji Ointment in diabetic rats*

    Objective To investigate the mechanism of Huiyang Shengji (Restoring Yang and Promoting Tissue Regeneration) Ointment on healing of yin-pattern wound in diabetic rats. Methods Male Wistar rats (n=66) were chosen and 12 rats (non-conformity) were excluded, and then divided randomly into test group, model group and control group (each n=18). The control group was directly given skin resection-plastic ring embedding, and other groups were given intraperitoneal injection of STZ-intramuscular injection of hormone-skin resection-plastic ring embedding for preparing model of yin-pattern wound. The test group was treated with HSO gauzes and model group and control group were treated with Vaseline gauzes once a day. After 3 d, 7 d and 10 d, 6 rats were executed in each group, and wound healing and morphologic changes were observed. The wound tissue were collected and prepared pathological sections for observing results of HE staining and immunohistochemical staining of CD34 (microangiogenesis marker). Results The healing rate was lower in model group than that in control group (P<0.01) after 3 d, 7 d and 10 d. The difference in healing rate was not significant between test group and model group after 3 d, but was significant after 7 d and 10 d. The healing rate was higher in test group than that in model group (P<0.05). The results of HE staining showed that granulation tissue grew vigorously, fibroblasts and capillaries were abundant, collagen fibers were thickened and inflammatory cells were less in test group and control group compared with model group after 7 d and 10 d. Inflammatory cells reached the highest value after 7 d and decreased significantly after 10 d, and capillaries began to increase and collagen fibers increased continuously in model group. The IOD value of CD34 increased significantly in test group and control group after 3 d and 7 d (P<0.01), and decreased after 10 d. There was statistical difference between test group and control group but was no significant. The content of CD34 increased slowly in model group during experimental stage, and there were significant statistical difference between model group and test group (P<0.01). Conclusion Huiyang Shengji Ointment can dynamically regulate yin-pattern wound to improve wound healing in diabetic rats based on theory of yang transforming qi and yin transforming body constituent.

    Specificity of artistic conception operating training in introverts and extroverts based on α-band electroencephalogram power*

    Objective To analyze the influence of artistic conception operating training on introverts and extroverts through detecting the changes of α-band electroencephalogram (EEG) power in introverts and extroverts accepted or no accepted artistic conception operating training. Methods Undergraduates were chosen as subjects and introverted personality and extroverted personality were stratified (introvert group and extrovert group), and then introvert group and extrovert group were divided randomly into introvert test group, introvert control group, extrovert test group and extrovert control group in a ratio of 1 to 1. The test groups received training of artistic conception operation and all groups were given experimental test of artistic conception operation. After artistic conception operating training or intervention, α-band EEG absolute power was collected, compared and analyzed in 2 groups. Meanwhile α-band EEG absolute power was compared and analyzed in all groups before and after the intervention. Results The absolute power had statistical difference between introvert group and extrovert group (P<0.05). The absolute power was higher in introvert test group and introvert control group after intervention, and self-comparisons had statistical difference within introvert test group and introvert control group before and after intervention (P<0.05, P<0.01). There was significant statistical difference between introvert test group and introvert control group (P<0.01), and absolute power was lower in introvert test group than that in introvert control group. There was no significant statistical difference in extrovert test group (P>0.05), there was significant statistical difference in extrovert control group (P<0.01) before and after intervention, and there was no statistical difference between extrovert test group and extrovert control group (P>0.05). Conclusion The intervention of artistic conception operation has tranquilizing mind effect on introverts, and training of artistic conception operation has excitatory effect on extroverts.

    Preparation technology and quality evaluation of Fuke Tongjing (Gynecologic Dysmenorrheal) Dispersible Tablet*

    Objective To determine the formula and preparation technology of Fuke Tongjing (Gynecologic Dysmenorrheal) Dispersible Tablet, and establish quality standard. Methods After a series of single-factor screening, the types and dosage ranges of fillers, disintegrators and other excipients were preliminarily determined and on this basis, they were screened and determined through D-optimal mixture design. Then the formula of Fuke Tongjing Dispersible Tablet was ensured through single-factor screening for determining dosages of glidants and corrigent. The disintegration time, hardness, friability, tablet weight difference, content of toxic components (3 kinds of diester aconitine alkaloids) and content of 6-gingerol (index component) were detected. Results The optimal formula of Fuke Tongjing Dispersible Tablet was as follows: 35.00% of powder, 17.90% of microcrystalline cellulose (MCC), 6.75% of sorbitol, 19.70% of crospovidone (PVPP), 17.40% of low-substituted hydroxypropyl fiber (L- HPC), 0.75% of aspartame, 1.00%of citric acid, 1.00% of aerosil and 0.50% of magnesium stearate. The average disintegration time was (50.83±1.47) s, average hardness was (6.44±0.27) kg/cm, average friability was 1.95%, and difference in tablet weight was 1.09%. There were no toxic diester aconitine alkaloids including new aconitine, aconitine and hypaconitine detected. The mass fraction of 6-gingerol was (0.617±0.007 9) mg/g. Conclusion The appearance of prepared Fuke Tongjing Dispersible Tablet is smooth, and disintegration time, hardness, friability, tablet weight difference and content of 3 kinds of diester aconitine alkaloids are all accord with requirement of Pharmacopoeia of the People’s Republic of China (2015). The content of 6-gingerol is uniform.

    Characteristics of depression pattern based on depression, anxiety and somatoform disorders*

    Objective To discuss the symptoms, pattern manifestations and disease locations related to zang-fu organs of depression pattern of traditional Chinese medicine (TCM) through the clinical survey on patients with depression, anxiety and somatoform disorders. Methods The patients in accordance with internalized standard were divided into group of major depressive disorder (MDD group), group of anxiety disorder (AD group) and group of somatoform disorders (SFD group). All groups were given scoring by using Self-rating Depression Scale (SDS), Self-rating Anxiety Scale (SAS) and Composite Autonomic Symptom Scale (COMPASS-31). The clinical information was collected from 3 groups for reviewing TCM pattern manifestations and TCM diseases. The TCM characteristics in patients with depression pattern were analyzed by using methods of frequency statistics, chi-square test, Wilcoxon rank sum test and Spearman correlation analysis. Results ①There were totally 291 cases observed, and 124 in MDD group, 92 in AD group and 75 in SFD group. There were more women and mental workers in 3 groups (P>0.05). ②The average numbers of symptoms were (5.98±3.03) in 3 groups, and the more severe the condition, the more the symptoms in MDD group and AD group (P<0.05). ③The scores of COMPASS-31 were positively correlated to symptom numbers in different groups and in combined 3 groups (P<0.05). ④The patients with only psychosomatic symptoms or somatic symptoms accounted for 1.37% and 38.49%, and those with two of them accounted for 60.14%. ⑤The most common clinical manifestations were in spleen-stomach disease system, and secondly in qi-blood-fluid-humor disease system and heart disease system. Compound pattern manifestations were more than simple pattern manifestations. The disease locations related to zang-fu organs were mostly in heart system (44.67%), and secondly in systems of liver, spleen, kidney, stomach, gallbladder and lung. The scores of COMPASS-31 were the highest in kidney disease system. The disease names were the most in heart disease system and secondly in qi-blood-fluid-humor disease system, liver disease system, spleen-stomach disease system and kidney disease system. Conclusion The clinical manifestations of TCM depression pattern appear mostly in the form of somatic symptoms with higher dispersion in pattern properties. The disease locations related to zang-fu organs are mainly heart, liver and spleen involving many disease names.

    Medication characteristics of Wang Zhongqi and Wang Renzhi (physicians in Xin’an Wang’s internal medicine) in treatment of stomach duct pain*

    Objective To analyze the medication characteristics of Wang Zhongqi and Wang Renzhi (physicians of Xin’an Wang’s internal medicine) in treatment of stomach duct pain. Methods The formulas for treating stomach duct pain were chosen from Wang Chongqi’s Medical Cases (74 cases) and Wang Renzhi’ s Medical Cases (160 cases) and medicinals were selected. The eligible formulas to internalized standard were input into Office 2010 and high-frequency medicinals were calculated by using frequency analysis. SPSS 22.0 software was used to given a correlation analysis on the high-frequency medicinals, and IBM SPSS Modeler 14.1 software was used in the analyses of Apriori algorithm and maximum frequent association pattern. Results There were totally 888 Chinese medicinals appeared in Wang Chongqi’s Medical Cases, and top 3 medicinals were Banxia (Pinellia Tuber, Rhizoma Pinelliae), Fuling (Indian Bread, Poria) and Xuanfuhua (Inula Flower, Flos Inulae). There were totally 2146 Chinese medicinals appeared in Wang Renzhi’ s Medical Cases, and top 3 medicinals were Gualou (Trichosanthes fruit, Fructus Trichosanthis), Banxia (Pinellia Tuber, Rhizoma Pinelliae) and Zhiqiao (Bitter Orange, Fructus Aurantii). The results of association analysis showed that couplet medicinals for stomach duct pain used by Wang Chongqi were mainly Gualou-Xiebai (Macrostem Onion, Bulbus Allii Macrostemi), Banxia-Xiebai and Peilan (Eupatorium, Herba Eupatorii)-Chenpi (Tangerine Peel, Pericarpium Citri Reticulatae), and couplet medicinals for stomach duct pain used by Wang Renzhi were mainly Gualou-Xiebai, Banxia-Zhiqiao, Zhiqiao-Banxia and Banxia-Xiebai. Conclusion These 2 physicians of Xin’an Wang’s internal medicine take unblocking as major therapy in treatment of stomach duct pain to remove the causes of gastric qi obstruction, including dredging qi movement, or improve transportation of spleen and stomach, or activating blood and resolving stasis, or promote digestion and remove food stagnation or warming the middle and move qi. Stomach duct pain is commonly accompanied by qi stagnation, which is treated often with therapies of pungent for opening and bitter for descending by using medicinals with effects of regulating qi with pungent and aroma medicinals for harmonizing stomach and relieving pain. The downward features of stomach should be complied with to free body’s original qi and improve recovery of disease. Wang Zhongqi and Wang Renzhi have the same thinking on treatment of stomach duct pain, but they have their own medication characteristics. Wang Renzhi puts more emphasis on long-term stomach duct pain with blood stasis and phlegm retention, and often uses therapies of unblocking yang, discharging turbidity, resolving stasis and freeing collaterals.

    Combination of insect medicinals for improving curative effect applied in treatment of ischemic stroke*

    Objective To study the combination characteristics of insect medicinals in treatment of ischemic stroke, and discuss the combination methods of insect medicinal for improving clinical efficacy based on literature study. Methods All clinical research literatures resent 10 years (from 2007 to 2018) on insect medicinals were studied in, and a database was established by applying Excel 2010. The frequency of medicinal administration was statistically analyzed and given cluster analysis by using SPSS 24 software, and RapidMiner software was used in association analysis of insect medicinals. Results There were totally 156 eligible literatures, 150 formulas, 153 medicinals and 8 561 patients involved in the study. Among all insect medicinal, the appearance frequency of Dilong (Earthworm, Lurmbricus), Shuizhi (Leech, Hirudo) and Quanxie (Scorpion, Scorpio) were the highest, and they were commonly used as couplet medicinals. The medicinals with effects of tonifying deficiency, activating blood, resolving phlegm and clearing heat were often combined with sectic medicinals in administration. Conclusion The couplet medicinals of Dilong-Shuizhi-Quanxie are commonly used for improving clinical efficacy. The insect medicinal are combined with medicinals with effects of tonifying deficiency, activating blood, resolving phlegm and clearing heat can promote the effects of activating blood and resolving stasis, and resolving phlegm and free collateral.

    Chinese medical pattern factors of symptomatic cerebral artery stenosis*

    Objective To investigate the characteristics of pattern factors of traditional Chinese medicine (TCM) of symptomatic cerebral artery stenosis (SCAS). Methods The patients diagnosed with cerebral artery stenosis (CAS, n=140) were chosen from Out-patient and In-patient Department in Chinese Medicine Hospital of Hubei Province from Dec. 2015 to June 2018. The information of CAS severity, past medical history, clinical symptoms and TCM pattern factors were collected. The patients were divided, according to clinical symptoms, into SCAS group (n=118) and non-SCAS group (n=22). The characteristics of TCM pattern factors were observed and analyzed in 2 groups. SCAS group was divided, according to CAS severity, into mild CAS group, moderate CAS group and severe CAS group. The characteristics of TCM pattern factors were analyzed in all groups. Results Wind pattern was the most common type of TCM pattern factor of SCAS (P<0.05), and they were positively correlated to each other. The percentage of wind pattern was the highest in mild CAS group and moderate CAS group. The common symptoms of wind pattern included tongue trembling, wiry pulse, dizziness and rapid changes of illness. Conclusion CAS was closely correlated to wind pattern (one of TCM pattern factors), and also related to phlegm and blood stasis. To understand and grasp the laws of TCM pattern factors of CAS can intervene early the patients with stroke risk factors and prevent them from stroke attack.

    Objective To observe the influence of eye acupuncture and body acupuncture on 24 h expression of cerebral aquaporin 4 (AQP4) in rats with cerebral ischemia-reperfusion injury (CIRI), and discuss the difference in mechanism of treating CIRI between eye acupuncture and body acupuncture. Methods SD rats (n=48) were divided into 6 groups by using random digital method: normal control group, 24 h sham-operation group, 24 h model group, 24 h eye acupuncture group, 24 h body acupuncture group and 24 h outside area group. The model of CIRI was established by using thread approach in 24 h model group, 24 h eye acupuncture group, 24 h body acupuncture group and 24 h outside area group. After making ischemia for 1 s, the thread was pulled out to make reperfusion. After 1 h and rats revived fully, the neurologic function was scored by applying ZeaLonga scoring method, and acupuncture intervention was given to rats. Then reperfusion was continued for 24 h and rats were executed. The changes of cerebral neurocyte morphology were observed by using microscope, and protein and mRNA expressions of AQP4 were detected by using Western blotting assay and RQ-PCR. Results The neurological functional deficit scores decreases significantly in eye acupuncture group compared with model group. In eye acupuncture group cerebral cortex neurons increased in volume with large cores and abundant cytoplasm, and swelling of mitochondria and rough endoplasmic reticulum were significantly relieved. The protein and mRNA expressions of AQP4 decreased significantly (P<0.05). The comparison showed that there was no difference between eye acupuncture group and the body acupuncture group. Conclusion Eye acupuncture and body acupuncture have relieving effects on 24 h CIRI, and the mechanism is related to decrease of brain AQP4 expression. The difference between eye acupuncture and body acupuncture is not significant.

    Influence of tip-root acupoint combination on myocardial cells in diabetic rats with myocardial ischemia-reperfusion injury and mechanism study*

    Objective To observe the influence of tip-root acupoint combination pretreatment on calcium homeostasis, myocardial oxidative stress and inflammatory cytokines in diabetic rats with myocardial ischemia-reperfusion (I/R) injury, analyze the mechanism from the aspect of p38MAPK signaling pathway and provide a new theoretical discussion and experimental basis for prevention and treatment of cardiovascular diseases in clinic. Methods All rats (n=50) were randomly divided into 5 groups: sham-operation group, model group, electro-acupuncture retreatment group, inhibitor pretreatment group and electro-acupuncture+inhibitor pretreatment group (each n=10). The sham-operation group was not given other treatment after opening chest to expose heart, model group was given in vivo myocardial I/R operation, electro-acupuncture retreatment group was given needling in Neiguan (PC6) and Zusanli (ST36) one week before modeling, inhibitor pretreatment group was given injection of SB203580 (inhibitor of p38MAPK signaling pathway) before modeling, and electro-acupuncture+inhibitor pretreatment group was given injection of SB203580 one week after electro-acupuncture retreatment and before modeling. All rats in 5 groups were treated and executed for collecting samples. The indexes related to calcium homeostasis, myocardial oxidative stress and inflammatory cytokines were detected in all groups. Results The difference in Na+-K+-ATPase, Mg2+-ATase, Ca2+-ATPase, superoxide dismutase (SOD), malondialdehyde (MDA) and tumor necrosis factor-α (TNF-α) were significant between sham-operation group and model group (P<0.05). There difference in the above mentioned indexes had statistical significance compared among inhibitor pretreatment group and electro-acupuncture+inhibitor pretreatment group. Conclusion The pretreatment of tip-root acupoint combination aims at maintaining cardiomyocyte calcium homeostasis, improving ability of scavenging oxygen free radicals, inhibiting myocardial inflammation, and alleviating significantly damage of heart function due to I/R in diabetic rats with I/R injury.
