    Interpretation of TCM holism by qi-monism in concept of perpetual motion*

    The concept of perpetual motion refers to the viewpoint of movement, development and change that should be held in understanding the change law of things, including the medical problems of birth, old age, illness and death. Motion is the most basic and most important feature of cosmic inventory and human life, and the motive power for the development and changes of things. Qi-monism is an important philosophical thought in the Pre-Qin Dynasty. It holds that qi is the origin of the universe, the intermediary of the relationship among things, and qi has the movement forms of upward, downward, inward and outward movement. It is precisely because of the continuous movement of qi that all things change vigorously, that is, qi-monism has the concept of perpetual motion. Based on the philosophical basis of qi-monism, the holism of TCM elaborates the unity between human and nature, the homology and isomorphism of human viscera and the unity and harmony between human and society, precisely because of its concept of perpetual motion. The monism of qi, based on the concept of constant motion, maintains the overall concept of traditional Chinese medicine and makes people, nature and society an organic whole.

    Expanded new concept five phase map: on model of metasystem

    Prof. Meng Kaitao, works on philosophical mathematics in Northwestern University and is an appointed researcher in the Institute of Chinese Basic Clinical Medicine of China Academy of Chinese Medical Sciences, originally creates philosophical mathematics for interpreting the cosmology from thinking perspective. He has discovered twelve laws of the world based on the dialectical logic thinking mode and refined the concept of metasystem. On the basis of TCM time-space medicine, Prof. Meng combines the thinking of Hetu and Yidao and creates a new concept five phase (wood, fire, earth, metal and water) map to comprehensively interpret the origin and evolution of TCM thinking mode. The connotation of metasystem is interpreted from thinking angle, and the content of new concept five phase map, a Prof. Meng’s metasystem thinking mode, is mined in the paper, including five phase distribution, central axis, middle qi and dimensions. The paper proposes that five-phase distribution is closely related to the contents of central axis, middle qi, and dimensions. The earth in the center is surrounded by other four phases, and spleen does not correspond only one season. Spleen governing four seasons is in line with the thinking mode of the universe. The engendering and restraining of five phases are not linear sequence. The movements of five phases show a perfectly round motion with spleen in the center. The middle qi is generated due to extreme changes, which is the congenital original qi and element power of upbearing, downbearing, floating and sinking. The shape of central axis is not a physical linearity, but a spirit of thinking construction with inner yang qi. The soft upbearing of yang qi sinking and its downbearing stems from floating. The central axis is in the position between two places at the lower left and the upper right with time-space connotation. The five phase map, drawing upbearing, downbearing, floating and sinking, aims at interpreting the connotation of four-dimensional phase. The floating and sinking are the noumenon of yin and yang, and upbearing and downbearing are the transformation of yin and yang. The name is different so connotation is different, and four motions are not the extending of same dimension, while belong different dimensions. In other words, Prof. Meng proposes the law of alternating and reciprocal transformation, indicates the difference in complexity of upbearing, downbearing, floating and sinking, and points out the dimension content implicating in movements of upbearing, downbearing, floating and sinking of new concept five phase map. The changes of universe show four-dimensionality and four dimensions in TCM also contains space-time content. The four dimensions are changing. The four dimensions of metasystem contain spirit, qi and physique and will be multiplied as long with yin-yang differentiation, which is not simple three-dimension or four-dimension construction and shows multi-dimensional nature.

    Theory construction of “the Earthly Branches transforming into qi” in theory of five circuits and six qi: a new interpretation*

    The theory of five circuits and six qi is an important part of TCM theory, in which “the Earthly Branches transforming into qi” is the most basic theory. The interpretation on it is contradictory at present. The relevant content in ancient literature is analyzed by applying the embryology methods of literature analysis and theoretical study based on 7 major discourses in Neijing (Huangdi’s Internal Classic). The theory construction of “the Earthly Branches transforming into qi” is affected by multiple factors including location of three-yin three-yang, seasonal distribution and previous theories. The construction of this theory is complex in the forming process, but closely relates to changes of four seasons and reflects plain materialism and dialectics. The theory construction of “the Earthly Branches transforming into qi” is a summary of the objective law of nature by ancestors and it is not mysterious.

    Pathogenesis of metabolic syndrome-turbidity toxin based on intestinal microecology*

    Metabolic syndrome is a clinical syndrome based on hyperinsulinemia and insulin resistance, which integrates multiple metabolic disorders. At present, there is no unified understanding on its etiology, pathogenesis and treatment in Traditional Chinese Medicine. Therefore, scholars mostly advocate that metabolic syndrome is treated from liver and spleen. In view of the widespread, entanglement, variability and abnormality of the pathogenesis of metabolic syndrome, some scholars have proposed the theory of turbidity toxin, which has been widely accepted, but the understanding on its connotation is still abstract. Fortunately, advances in modern intestinal microbiome have deepened the relationship between turbidity toxin and metabolic syndrome. It has been found that the pathogenesis of turbidity toxin coincides with that of metabolic disorders induced by the imbalance of intestinal flora and impaired intestinal mucosal barrier. Taking intestinal flora as a starting point, it is expected to provide a new interpretation for the treatment of metabolic diseases from the theory of turbidity toxin. Therefore, starting from the pathogenesis of turbidity toxin in TCM and taking “turbidity stagnating blood aspect-turbidity transforming toxin-toxin damaging collaterals” as mainline, the evolution of metabolic syndrome induced by intestinal flora imbalance is re-combed in order to provide theoretical basis for the prevention and treatment of metabolic diseases.

    Influence of Fufang Xueshuantong Capsule on neuroplasticity in mice with cerebral ischemia-reperfusion*

    To observe the influence of Fufang Xueshuantong(Compound for Resolving Thrombosis) Capsule (FFXST Capsule) on neuroplasticity in mice with cerebral ischemia-reperfusion (CIR) and discuss the relevant mechanism. Methods Male CIR mice were randomly divided into sham-operation group, CIR group, low-dose (0.75 g/kg), mid-dose (1.5 g/kg) and high-dose (2.25 g/kg) FFXST Capsule groups, and Buchang Naoxintong Capsule (BCNXT Capsule) group (0.55 g/kg). Each group was divided randomly into 1-d group 7-d group and 14-d group (each n=10). The model was established by using middle cerebral artery occlusion (MCAO). The sham-operation group was only given anesthesia and blood vessel separation without ligation and embolization. Other groups were orally given medicinals once a day. The status of neurological dysfunction was reviewed through neurological scoring, adhesive tape removal and forelimb strength determination experiment. The healthy condition of mice was observed through monitoring mortality. The morphologic changes and expressions of GAP-43, Syn, in cortical neurons of ischemic zone were detected by using Nissl staining and immunohistochemical staining (IHC). The content of nerve growth factor (NGF), brain-derived neurotrophic factor (BDNF) and neurotrophic factor-3 (NT-3) was detected by using ELISA, and levels of neurotrophic factors were observed after CIR injury. Results Compared with sham-operation group, the significant neurological impairment and motor disorders were observed in CIR group(P<0.05) with higher mortality, shallow Nissl’ staining vacuoles and sparse arrangement in the tissue around ischemic zone. The expressions of GAP-43 and Syn decrease and content of NGF, BDNF and NT-3 increased in CIR group. Compared with CIR group, the neurological dysfunction had been relieved and mortality was reduced in FFXST Capsule groups in different doses, with deepened Nissl staining, less vacuoles and more tight arrangement. The expressions of GAP-43 and Syn increased and content of NGF, BDNF and NT-3 increased in FFXST Capsule groups. The above effect were more significantly in low-dose FFXST Capsule group(P<0.05). Conclusion FFXST Capsule can induce axonal regenera tion, improve neuroplasticity and play the role of neuroprotection may through promoting GAP 43 and Syn expressions, and increasing neurotrophic factor content.

    Mechanism of Qufeng Xuanfei Fang in intervention of neurogenic inflammation of human bronchial epithelial cells mediated by nerve growth factor*

    Objective To investigate the interventional effect and mechanism of Qufeng Xuanfei Fang (Wind-expelling Lung-dispersing Decoction, QFXF Decoction) to neurogenic inflammation of human bronchial epithelial cells (HBEC) mediated by nerve growth factor (NGF). Methods HBEC were randomly divided into normal group, model group, Asmeton group, and high-dose, mid-dose and low-dose QFXF Decoction groups. The model of in vitro neurogenic inflammation was established by using NGF. The normal group was given normal serum, model group, 100 ng/mL of NGF and normal serum, Asmeton group, 100 ng/mL of NGF and Asmeton medicated serum, and high-dose, mid-dose and low-dose QFXF Decoction groups, QFXF Decoction medicated serum in different doses(100 ng/mL of NGF and high-dose, mid-dose or low-dose QFXF Decoction). The proliferation rate of HBEC was detected by using MTT, apoptosis rate was detected by using flow cytometry, and mRNA and protein expressions of Pan-Ras, c-fos and NK-1R were detected by byusing RT-PCR and Western blotting assay. Results The proliferation rate and apoptosis rate of HBEC, and mRNA and protein expressions of Pan-Ras, c-fos and NK-1R increased all in model group compared with normal group(P<0.05). The proliferation rate and apoptosis rate of HBEC, and mRNA and protein expressions of Pan-Ras and c-fos decreased in Asmeton group, and high-dose, mid-dose and low-dose QFXF Decoction groups compared with model group(P<0.05). The mRNA and protein expressions of NK-1R decreased significantly in high-dose and mid-dose QFXF Decoction groups(P<0.05), while they had no changes in Asmeton group and low-dose QFXF Decoction group (P>0.05). Conclusion QFXF Decoction can relieve HBEC neurogenic inflammation mediated by NGF and reduce cough sensitivity through down-regulating the expressions of genes and protein related to Ras-Raf-MAPK signaling pathway.

    Influence of Zuogui Jiangtang Jieyu Fang on expressions of hippocampal NVU connexins in rats with diabetes mellitus complicated by depression*

    Objective To discuss the influence of Zuogui Jiangtang Jieyu Fang (ZHJYF, Left-Restoring Sugar-lowering Depression-relieving Formula), based on neurovascular unit (NVU), on the expressions of tight junction proteins and connexins in rats with diabetes mellitus (DM) complicated by depression. Methods Adult SD rats (n=70) were orally given high-fat emulsion, injection of streptozocin (STZ) in caudal vein and chronic unpredicted stress to establish the model of DM complicated by depression. The rats were randomly divided into 5 groups: model group, metformin+fluoxetine group, high-dose, mid-dose and low-dose ZHJYF groups. Other 12 unmodeling rats were chosen into blank control group. All group were orally given corresponding medicinals at the same time of stress for 28 d. The level of autonomic activity and learning-memory ability were reviewed by using open-field test and Morris water maze (MWM), and morphologic structure of hippocampal NVU was observed by using transmission electron microscope (TEM). The expressions of tight junction proteins (Claudin-1 and Occluding), connexins [aonnexin43 (CX43) and aquaporin4 (AQP4)] were detected by using immunohistochemistry technique and RT-qPCR. Results Compared with blank control group, the times of autonomic activity and space exploration time decreased significantly and escape latency was prolonged significantly in model group (P<0.01). The results of TEM and pathology showed that swollen hippocampal endothelial cells, confused perivascular tight and gap junctions and loosely arranged cells, and the protein and mRNA expressions of Claudin-1, Occludin, CX43 and AQP4 decreased significantly in model group (P<0.01). Compared with mode group, the times of autonomic activity and space exploration time increased significantly and escape latency was shortened significantly (P<0.05 or P<0.01), swelling of hippocampal endothelial cells was relieved, perivascular tight and gap junctions was recovered, and cell arrangement was tidiness, and the protein and mRNA expressions of Claudin-1, Occludin, CX43 and AQP4 increased significantly in high-dose ZHJYF group (P<0.05 or P<0.01). Conclusion ZHJYF can significantly relieve depressive behavior in rats, and the mechanism may be related to enhance of expressions of tight junction proteins and connexins and maintain the structure and function of hippocampal NVU.

    Influence of BuzhongYiqi Granule on expressions of Treg/Th17 cytokines in rats with experimental autoimmune thyroiditis*

    Objective To investigate the influence of BuzhongYiqi(Middle-tonifying Qi-replenishing) Granule (BZYQ Granule) on expressions of Treg/Th17 cytokines in rats with experimental autoimmune thyroiditis (EAT), and discuss the mechanism of BZYQ Granule in treatment of Hashimoto’s thyroiditis from the aspect of unbalance of Treg/Th17 cytokines. Methods EAT rat model was established by using mixed immune injection of porcine thyroglobulin and complete and incomplete Freund’s adjuvant and high iodine feeding, and then the rats were randomly divided into blank group, model group, BZYQ Granule group, Euthyrox group and BZYQ Granule+Euthyrox group. After medicinal intervention for 2 months, the samples of rat spleen tissue were collected for detecting the protein and mRNA expressions of transforming growth factor-β (TGF-β), Foxp3 transcription factor (Foxp3), retinoic acid-related orphan receptor γT (RORγT) and interleukin-17 (IL-17) by using RT-qPCR and Western blotting assay. Results Compared with blank group, the protein and mRNA expressions of RORγT and IL-17 were up-regulated (P<0.05), and mRNA expressions of Foxp3 and TGF-β were down-regulated (P<0.05) but protein expressions of them had no statistical significance (P>0.05) in model group. Compared with model group, the mRNA expressions of RORγT and IL-17 were down-regulated (P<0.05) and protein expressions of them decreased, and the expression of RORγT had statistical significance(P<0.05) but expression of IL-17 had no statistical significance in all medicinal groups. The mRNA expressions of Foxp3 and TGF-β were up-regulated (P<0.05) and protein expressions of them had statistical significance (P>0.05) in all medicinal groups compared with model group. The comparison among medicinal groups showed that the mRNA expressions of RORγT, IL-17, Foxp3 and TGF-β had no significant difference among these groups, while protein expression of Foxp3 was higher in Euthyrox group than that in BZYQ Granule group (P<0.05). The protein expressions of RORγT, IL-17 and TGF-β had no significant difference among BZYQ Granule group, Euthyrox group and BZYQ Granule+Euthyrox group. Conclusion BZYQ Granule can inhibit thyroid autoimmune inflammatory, and the mechanism may be related to the regulation of balance status of Treg/Th17 cytokines.

    Effect and mechanism of Huxie Huaji Ointment combined with cisplatin on apoptosis of H22 cells of liver cancer*

    Objective To investigate the effect and mechanism of Huxie Huaji Ointment combined with cisplatin on apoptosis of H22 cells of liver cancer. Methods SD rats were randomly divided into blank control group and Huxie Huaji Ointment, and the blood samples were collected from abdominal aorta for preparing medicated serum of Huxie Huaji Ointment after treatment continuously for 3 d. The logarithmic H22 cells were randomly divided into 5 groups: normal group, blank control group, cisplatin group, 20% of medicated serum group and 20% of medicated serum+cisplatin group. The influence of the medicated serum on H22 apoptosis was detected by using CCK-8 method, and apoptotic morphologic changes of H22 cells was observed after Hoechst 33258 staining in all groups. The expressions of B-cell lymphoma-2 (Bcl-2), cytochrome C (Cyt C), BCL2-associated X (Bax) and cysteinyl aspartate specific proteinase-9 (caspase-9) in H22 cells were detected by using high-content cell imaging analysis technology. Results Compared with blank control group, the proliferation of H22 cells was inhibited in a dose-time-dependent manner in medicated serum group,and the efficacr was the best in 20% of medicated serum group. Compared with blank control group, cisplatin group and medicated serum group, H22 cells showed nuclear chromatin aggregation and karyopyknosis in varying degrees, and more typical apoptotic characteristics in medicated serum+cisplatin group. The mean fluorescence intensity values of Cyt C, Bax and caspase-9 increased significantly in H22 cells (P<0.01 of P<0.05), and their protein expressions increased significantly. The mean fluorescence intensity value of Bcl-2 decreased significantly in H22 cells (P<0.01 of P<0.05), and its protein expression decreased significantly. Conclusion The medicated serum of Huxie Huaji Ointment combined with cisplatin can significantly inhibit the proliferation and induce the apoptosis of H22 cells, and the mechanism may be related to the imbalance of Bax and Bcl-2 protein expressions, and increases of Cyt C secretion and caspase-9 protein expression.

    Protective effect of Mongolian medicinal Xieri GA-4 in rat model of kidney injury*

    Objective To study the protective effect of Mongolian medicinal Xieri GA-4 in rat model of acute kidney injury induced by gentamicin. Methods Male healthy Wistar rats were randomly divided into blank group, model group, low-dose, mid-dose, high-dose Xieri GA-4 groups, group of Shenyan Xiaozhong Pian (Nephritis Swelling-relieving Tablet, SYXZ Tablet group) and prednison group. Except of blank group, other groups were given hypodermic injection of gentamicin (80 mg/kg·d) for reducing acute kidney injury for 6 d, and were treated with corresponding medicianls after 2 h for 14 d. The levels of serum creatinine (SCr), blood urea nitrogen (BUN), urine protein (UP), uracil-N-glycosylase (NAG) and kidney injury molecular (KIM-1) were detected by using ELISA after treatment. The kidney index (KI) and prostatic index (PI) were calculated in all groups. The histopathological changes of kidney tissue were observed with light microscope after HE staining. Results Compared with blank group, the urine volume(P<0.01), and levels of Scr, BUN, UP, NAG and KIM-1 (P<0.01) increased, and expressions of KI and PI (P<0.01) also increased in model group. Compared with model group, the levels of Scr, BUN and UP and PI decreased quickly(P<0.01), and expressions of NAG and KIM-1 were reduced after medicinal reaching to certain dose after continuous administration in low-dose Xieri GA-4 group(P<0.01). The urine volume, and levels of Scr, BUN, UP, NAG and KIM-1(P<0.05, P<0.01), and expressions of KI and PI decreased in mid-dose and high-dose Xieri GA-4 groups(P<0.05, P<0.01). The levels of some factors decreased in prednison group and SYXZ Tablet group in a period, but the results were better in all Xieri GA-4 groups during the whole experimental course. The proliferation of mesangial cells was relieved and tubular desquamation was significantly inhibited in low-dose, mid-dose, high-dose Xieri GA-4 groups. Conclusion Xieri GA-4 has a protective effect on rat model of kidney injury induced by gentamicin, and has therapeutic effect on PI increase induced by kidney injury.

    Controlling mechanisms of Qishan Fang based on metabonomics in mouse model of lung cancer*

    Objective To study the changes of endogenous metabolites in mouse model of lung cancer after intervention by Qishan Fang (Milkvetch Root and Taxus chinensis Formula), and investigate the metabolites and metabolic pathway related closely to lung cancer, and therapeutic effect of Qishan Fang. Methods The mouse model of Lewis lung cancer was established and all mice were randomly divided into Qishan Fang group, model group and blank group. The blood samples of all groups were given metabonomics analysis by using UPLC-HDMS technology platform. The mass spectrometry metabolic profiling was analyzed by using Ezionfo software. The potential metabolic biomarkers related to lung cancer were analyzed through OPLS-DA combined with retrieving of HMDB and KEGG and Progenesis QI software for compound identification. The influence of Qishan Fang on blood metabolic profiling and biomarkers were further studied, and mechanism of Qishan Fang in intervening blood metabonomics in Lewis mouse model of lung cancer. Results There were 22 metabolic biomarkers may related to lung cancer. It was found that Qishan Fang had significantly callback effect on 18 metabolic biomarkers related to lung cancer. The identified biomarkers with callback effects were further given MetPA analysis and 7 metabolic pathways were obtained, including pyrimidine metabolism, biosynthesis of unsaturated fatty acids, selenoamino acid metabolism, pentose phosphate pathway, glutathione metabolism, cysteine and methionine metabolism, and glycine serine and threonine metabolism. Conclusion It was found that has some therapeutic effects on lung cancer through regulating metabolic biomarkers related to lung cancer based on metabonomics research method.

    Treatment of acute exacerbation of chronic obstructive pulmonary disease-phlegm-heat congesting lung pattern with Qingfei Huatan Tang through immune regulation*

    Objective To investigate the immune regulation mechanism of Qingfei Huatan Tang(Lung-clearing Phlegm-resolving Decoction, QFHT Decoction) in treatment of acute exacerbation of chronic obstructive pulmonary disease (AECOPD)-phlegm-heat congesting lung pattern. Methods The patients with AECOPD-phlegm-heat congesting lung pattern (n=64) were randomly divided into treatment group and control group. The control group was given therapies of anti-infection, relieving asthma and mechanical ventilation in necessity. The treatment group was additionally given QFHT Decoction based on the same therapies in control group for 7 d. The changes of symptoms were observed, including cough, expectoration, panting, shortness of breath, spontaneous sweating, wheezing, lassitude of spirit and laziness to speak, loss of appetite and abdominal distension after meal, before and after treatment. The cells of peripheral blood C-reactive protein (CRP) and CD4+CD25+FoxP3+Treg, percentage of Th17, expression of TLR4 mRNA, levels of serum interleukin-8 (IL-8), noradrenalin (NE) and tumor necrosis factor-α (TNF-α), and absorbance of nuclear factor-κB (NF-κB) were compared in 2 groups before and after treatment. Results The clinical symptoms were all significantly relieved in 2 groups after treatment. The symptoms including expectoration, panting, shortness of breath, wheezing and abdominal distension after meal were significantly relieved in treatment group compared with control group (P<0.05), and control rate was significantly higher in treatment group than that in control group (P<0.05). The levels of serum NE and TNF-α, expressions of TLR4 mRNA, and percentage of peripheral blood Th17 decreased significantly in treatment group compared with control group (P<0.05). Conclusion QFHT Decoction combined with Western drugs can relieve clinical symptoms more significantly in AECOPD patients. QFHT Decoction may intervene the inflammatory reactions of AECOPD through regulating TLR4-MyD88/TRIF-NF-κB signaling pathway and acquired immunity mediated by Th17 on the base of animal studies.

    Application of nominal group technique in criterion establishment of “Fatigue-Prominent Suboptimal Health Status Determination and TCM Pattern Differentiation*

    Objective To establish the criterion for the determination of fatigue-prominent suboptimal health status and common TCM pattern differentiation based on nominal group technique (NGT). Methods The literatures related to fatigue-prominent suboptimal health status were retrieved and sorted systematically, and research progresses in relative definitions, determination standards and TCM pattern differentiation were summarized. The framework and content of the draft standard of “Fatigue-Prominent suboptimal Health Status Determination and TCM Pattern Differentiation” were preliminarily formed, and consensus on the framework and content was reached by using NGT. Results The consensus was basically reached by using NGT and there were 3 parts in the content including 1.Definitions of fatigue-prominent suboptimal health status: subhealth status with fatigue as main manifestation. 2.Determination criteria of fatigue-prominent suboptimal health status: ①meeting the criteria of subhealth; ②fatigue being the main complaint and continuing for 3 months or longer, and physical fatigue, mental fatigue, fatigue consequences or general fatigue being mild or above. 3.Common TCM patterns of fatigue-Prominent suboptimal Health Status: liver qi depression pattern, spleen qi deficiency pattern, spleen yang deficiency pattern, lung qi deficiency pattern, pattern of liver depression and spleen deficiency, pattern of spleen deficiency with dampness obstruction, heart blood deficiency pattern and pattern of internal obstruction of phlegm-dampness. Conclusion NGT is applied in this study to establish the definition, determination criterion and common TCM patterns of fatigue-prominent subopimal heaeth statas The authoritative forces are avoided and every participator can equally express their own ideas. The criterion is feasible and objective .
