    Modular pharmacology developed with the technology of block chain reveals the correlations between pattern and formula in Chinese medicine*

    As the Internet has been experiencing rapid development, the era of “block chain” is coming, with the properties of various technologies such as distribution, decentralization and self-organization. Shortly after the development into infrastructure of Bitcoin, the technology of block chain, considered to be with broader values, has been applied in the fields of healthcare, pharmaceuticals and biotechnology. In this paper, we discussed the advances in “block chain”, “modular pharmacology”, studies on the correlation between pattern and formula in Chinese medicine, as well as the required qualities of a researcher. Then we put forward the hypothesis that modular pharmacology developed with the technology of block chain could reveal the correlations between pattern and formula in traditional Chinese medicine.

    Alignment between essence-replenishing viscera-nourishing method and modern stem cell transplantation in repairing tissue damage based on “primordial essence-qi theory”

    With the property of essence, stem cell is the existing form of essence at the micro-level of the body. From the viewpoint of embryogenesis, stem cells conform to all the functional characteristics of essence. Essence generates primordial qi and can reflect the material structure, while primordial qi can embody the function of metabolism and energy transfer. Besides, the procedure of proliferation, differentiation and energy metabolism of stem cells after maturation is similar to the process of how essence generates primordial qi, as the generating and restraining relations described in the theory of five elements (i.e. five forms of energy). Modern medicine attempts to repair tissue damage by taking advantage of the functional characteristics of stem cells, which is theoretically identical to the theory of “primordial essence-qi” in traditional Chinese medicine and the method of “tonifying essence to nourish viscera”. As the kidney stores the essence, stem cells directly supplement congenital kidney essence to promote the repair of damaged tissues, playing an effect beyond the reach of any drug. It is a classic paradigm of “treating the root of chronic disease” in traditional Chinese medicine and provides new ideas and methods for the innovation and expansion of stem cell technology under the guidance of integrated theory of traditional Chinese and Western medicine.

    Distribution of circuits and qi in birth dates of on-campus students with gentleness constitution*

    Objective To analyze the characteristics of yunqi (circuits and qi) in birth dates of on-campus students with gentleness constitution, and to discuss its influence on the formation of gentleness constitution. Methods 900 cases of gentleness constitution were identified from on-campus students in Beijing University of Chinese Medicine by using constitution-differentiation questionnaires, and the correlation between the formation of gentleness constitution and their yunqi feature in birth dates was investigated by applying logistic regression to select the essential yunqi factors that had significant impacts on the formation of gentleness constitution. Results Heavenly stems (tian gan), guest qi and sitian-zaiquan (controlling heaven-terrestrial effect) in birth dates had impacts on the formation of gentleness constitution (P<0.05). Individuals who were born in gui year with guest qi of taiyang cold water, sitian of yangming dry metal and zaiquan of Shaoyin monarch fire were less likely to develop gentleness constitution. Conclusion The formation of gentleness constitution may be related to the characteristics of five circuits and six qi in birth dates.

    Analysis of the use of emetics in Rumen Shiqin*

    Zhang Zihe, one of the four masters of the Jin and Yuan Dynasties, was a representative physician of the “Pathogen-Attacking School”. He excelled at using the “emesis method” in treating diseases. He listed 36 kinds of medicinals as emetics in Rumen Shiqin (Confucian’s Duties to Their Parents), and classified them according to their nature, tastes and toxicities. It is clear that some of them are well known as emetics with good effects, while others are not. This article analyzed these medicinals in aspects of composition, efficacy, prescription and pathogenesis, classified them as monarch, minister, adjuvant and guiding medica, and revealed the fact that some of them are not actually emetics. This paper could helpimprove the understanding of the literary meaning of Rumen Shiqin and promote the clinical practice of emesis method.

    Comparison of neuroendocrine changes in male and female rat models of liver stagnation pattern*

    Objective To explore the gender differences of the modern connotation of liver stagnation pattern by comparing changes in neuro-endocrine of female and male rat models of liver stagnation pattern. Methods One week after adaptive feeding in rats, male and female rats with stable estrous cycle were randomly divided into normal group and model group (n=10 in each group). Chronic restraint stress method for 28 consecutive days was used to replicate liver stagnation pattern model At 29th-30th days of the experiment, all rats were sacrificed and neuro-endocrine related parameters of each group were measured: Monoamine transmitter index: plasma 5-hydroxytryptamine (5-HT), adrenaline (E) and norepinephrine (NE); Adrenal axis index: hypothalamic corticotropin-releasing hormone (CRH), serum adrenocorticotropic hormone (ACTH) and corticosterone (CORT); Gonadal axis index: hypothalamic gonadotropin releasing hormone (GnRH) , Serum Follicle stimulating hormone (FSH), luteinizing hormone (LH), prolactin (PRL), testosterone (T), estradiol (E2); Thyroid axis index: hypothalamic thyrotropin-releasing hormone (TRH), Serum thyroid-stimulating hormone (TSH), triiodothyronine (T3), and thyroxine (T4). Results Compared with male rats in the normal group, plasma 5-HT, NE and E of male rats in the model group were significantly increased (P<0.01); hypothalamus CRH and serum CORT were significantly increased (P<0.05), while serum ACTH didn’t change significantly (P>0.05); serum FSH, LH and T were significantly decreased (P<0.05, P<0.01), whereas serum PRL and E2 were significantly increased (P<0.05, P<0.01); serum TSH was significantly increased (P<0.05); serum T3 and T4 were significantly decreased (P<0.05), and hypothalamic TRH didn’t change significantly (P>0.05). Compared with female rats in the normal group, plasma E of female rats in the model group was significantly increased (P<0.05); 5-HT and NE were significantly decreased (P<0.05, P<0.01); hypothalamus CRH and serum ACTH, CORT were significantly increased (P<0.01); hypothalamus GnRH, serum FSH, LH, PRL, T and E2 were significantly increased (P<0.01); TRH of the hypothalamus, serum T3 and T4 were significantly increased (P<0.01), and serum TSH was significantly decreased (P<0.01). The results showed that both the female and male rats of the liver stagnation model had hyperactivity of the HPA axis. Female rats also had metabolic disorders of peripheral monoamine neurotransmitters and hyperfunction of HPT and HPG axis. Male rats had sympathetic hyperfunction, inhibition of the function of HPT and HPG axis. Conclusion Female and male rats with liver stagnation models have different neuro-endocrine function abnormalities. It is speculated that the modern physiological connotation of liver stagnation pattern varies according to gender.

    Effects of three-circle pole-standing qigong on heart rate variability in college students with anxiety*

    Objective To explore the effects of pole-standing qigong on anxiety via investigating the heart rate variability (HRV) time and frequency domain parameters. Methods College students (Self-Rating Anxiety Scale, SAS≥50) as volunteer subjects were recruited and randomly divided into experimental group and control group at 1∶1 ratio. The experimental group received standing qigong intervention for 10 weeks (≥5 times a week) while the control group did not receive any intervention. Before and after standing qigong intervention, The HRV results of both groups were collected for comparative analysis. Results The SDNN, RMSSD and pNN50 of the experimental group were higher than resting state before standing status, and this difference before and after intervention was statistically significant (P<0.05). LFn was reduced while HFn increased in the experimental group. The difference between LFn, HFn, LFn/HFn before and after the study was statistically significant (P<0.05) in the experimental group but not significant in the control group. Conclusion The pole-standing qigong intervention has a significant effect on the HRV in the practitioners, which is characterized by improved parasympathetic nerve activity, reduced sympathetic nerve activity, and benefiting from the balance of autonomic function. Its relieving effect on anxiety status warrants further study.

    Effects of Açaí and its alcohol extract on the immunity, endocrine hormone and neurotransmitter of mice with deficiency-heat and deficiency-cold pattern*

    Objective To investigate the immunity, endocrine hormone and neurotransmitter of Açaí alcohol extract on deficiency-heat and deficiency-cold models and to clarify the material basis of the cool properties of Açaí. Methods KM male mice were randomly divided into normal group, deficiency-heat model group, deficiency-heat + Açaí group, deficiency-heat + Açaí alcohol extract group, deficiency-cold model group, deficiency-cold + cinnamon group, deficiency-cold +Açaí group, deficiency-cold +Açaí alcohol extract group. The mice were administered orally at the dose of 160 mg/kg thyroxine and 25 mg/kg hydrocortisone in the afternoon per day for 14 days to induce deficiency-heat and deficiency-cold models respectively; each administration group was given the corresponding medication. At the end of the experiment, changes of related immune factors, endocrine hormones and neurotransmitter were measured. Results Consistent with Açaí, Açaí alcohol extract can improve the symptoms of asthenia fever, such as increased autonomous activity, easily irritated and active reaction in mice. It can reduce the level of immune factors such as IgG, C3, C4 (P<0.01, P<0.001, P<0.01), increase CORT (P<0.01), decrease E2 (P<0.01), elevate 5-HT (P<0.01), and reduce the NE content (P<0.001). Açaí alcohol extract had an effect on deficiency-heat model, in contrast to cinnamon, while there was no significant effect on the deficiency-cold model. Conclusion The alcohol extract of Açaí is the material basis of the cool properties of Açaí.

    Anti-atherosclerotic mechanism of phlegm-transforming and stasis-resolving Chinese medicinals*

    Objective To observe the regulation of miRNA-126 and its downstream signal expression in mouse aorta and vascular endothelial cells by observation of phlegm and blood stasis method to study the mechanism of Chinese medicine in inhibiting atherosclerosis. Methods In the cell experiment, the vascular endothelial cells were randomly divided into 4 groups: control group, model group, high-dose Chinese medicine-containing serum group, and low-dose Chinese medicine-containing serum group. Different groups of vascular endothelial cells were treated with different drug-containing serum to observe cell proliferation and apoptosis. RT-PCR and Western-blot assay were used to detect miRNA-126, RGS16, CXCL12, CXCR4, VCAM-1, mRNA and protein expression in vascular endothelial cells. The aortic cross-sectional pathological damage was evaluated with HE staining in vivo. The aortic miRNA-126, RGS16, CXCL12, CXCR4, VCAM-1 mRNA and protein expression were measured with RT-PCR and Western-blot methods. Results The results of in vitro experiments showed that the apoptosis rates of VEC in the model group, high-dose group and low-dose group were significantly higher than that in the control group (P<0.05), and the proliferation rate was significantly decreased (P<0.05). Compared with the model group, the apoptotic rate of VEC was significantly decreased in the high-dose group and the low-dose Chinese medicine-containing serum group (P<0.05), and the proliferation rate was significantly increased. Compared with the high-dose group, there was no significant difference in apoptotic rate and proliferation rate (P>0.05). After intervention, the expressions of miR-126 mRNA in the VEC of the model group, the high-dose group and the low-dose group were significantly lower than that of the control group (P<0.05). Compared with the model group, the expression of the miR-126 mRNA in the VEC of the high-dose group and the low-dose group was significantly increased (P<0.05). Compared with the high-dose group, there was no significant difference in the expression of miR-126 mRNA in the low-dose group (P>0.05). After intervention, the expressions of RGS16, CXCL12, CXCR4, VCAM-1 mRNA and protein in the VEC of the model group, high-dose group and low-dose group were significantly higher than those of the control group (P<0.05). Compared with the model group, the expressions of RGS16, CXCL12, CXCR4, VCAM-1 mRNA and protein were significantly decreased in high-dose group and low-dose group (P<0.05). Compared with high-dose group, there was no significant difference in RGS16, CXCL12, CXCR4, VCAM-1 mRNA and protein expression in low dose group VEC (P>0.05). The results of in vivo experiments showed that through HE staining, in the control group, the aortic vessels were even, the endometrium smooth and flat, and there was no atherosclerotic lesion; In the model group, the aortic vessels were uneven, plaque formed, and the thickness of the blood vessels and the cross-sectional area of the AS plaque were significantly larger than those of the other groups; In the high-dose group and the low-dose group, the aortic layers were normal, the inflammatory cells infiltrated lightly with mild lesions; the AS plaques were small and the lesion was significantly lighter than the model group. After intervention, the expressions of miR-126 mRNA in the arteries of the model group, high-dose group and low-dose group were significantly lower than that of the control group (P<0.05). Compared with the model group, the expression of miR-126 mRNA in the arteries was significantly increased (P<0.05). Compared with the high-dose group, there was no significant difference in the expression of miR-126 mRNA in the arteries of the low-dose group (P>0.05). After intervention, the expressions of RGS16, CXCL12, CXCR4, VCAM-1 mRNA and protein in the arteries of the model group, high-dose group and low-dose group were significantly increased compared with the control group (P<0.05). Compared with the model group, the expressions of RGS16, CXCL12, CXCR4 and VCAM-1 mRNA and protein in the arteries of the high-dose group and the low-dose group were significantly decreased (P<0.05). Compared with the high-dose group, there was no significant difference in the RGS16 and CXCL12 CXCR4 and VCAM-1 mRNA and protein expression in the arteries of the low-dose group. (P>0.05). Conclusion The mechanism of action in inhibiting the formation of atherosclerotic plaque may be related to the regulation of miRNA-126 affecting the expression of downstream signals GRS16, CXCL12, CXCR4 and VCAM-1.

    Mechanism of Xiaokui enema formula in the treatment of rat ulcerative colitis*

    Objective To investigate the effects of Xiaokuifang (Ulcer-curing Formula, XKF) on TH17/Treg signal pathways and the expression of FOXP3、RORγt and cytokine IL-17, IL-6, IL-10, and TGF-β in rats with ulcerative colitis. Methods Rat model of ulcerative colitis was established by using enema administration with TNBS and ethanol. The mesalazine(0.7 g/kg), high-dose (18.3 g/kg) and low-dose (9.15 g/kg) XKF were administered in enema to model rats divided into different groups. After enema, rat peripheral blood Thl7 and Treg cells of CD4+ Tcell percentage were measured with flow cytometry; IL-17、IL-6、IL-10 and TGF-β level in the rat serum were tested by using enzyme-linked immunosorbent (ELISA), colon tissue of Treg transcription factor FOXP3 and Th17 transcription factor RORγt gamma expression level were detected with Western blot assay. Results Both the mesalazine and high and low-dose of XKF groups could reduce the concentration of proinflammatory factor IL-17 and IL-6, and improve the concentration of IL-10 and TGF-β after treatment, high and low-dose XKF group and mesalazine group could improve the expression of FOXP3, reduce RORγt expression, and contribute to the effect of anti-inflammatory promote healing of ulcer surface. And the high-dose group was better than both the low-dose group and mesalazine group at the same time. Conclusion Xiaokuifang could promote Treg cells in rat colon tissue differentiation, mediate immune tolerance levels, inhibit the differentiation of Th17 cells, regulate the balance of Th17/Treg, maintain an immune balance, reduce the inflammatory response and play a role in the treatment of ulcerative colitis.

    Effects of Tibetan medicine Tinospora sinensis on the expression of inflammatory cytokines and NMDA receptors in the spinal cord of rats with pain-sensitive hyperinjury*

    Objective To observe the effect of water extract of Tinospora sinensis on hypersensitivity to temperature and mechanical stimulation in rats with inflammatory pain and its anti-inflammatory and analgesic mechanism. The expression levels of IL-1α in serum, IL-6, IL-10, TNF-α and N-methyl-D-aspartic acid (NMDA) receptors in the spinal dorsal horn of the affected side were measured to explore the anti-inflammatory and analgesic mechanisms of Tinospora sinensis in the treatment of inflammatory pain in rats. Methods Rats were randomly divided into five groups by using randomized sequence table: normal group, model group, prednisone acetate group, high-dose, and mid-dose Tinospora sinensis water extract (TSWE) groups. After 24 hours of Freund’s adjuvant injection to establish the inflammatory model, the rats were intragastrically administered for 12 days. The reaction value of 50% mechanical foot contraction pain, reaction time of thermal foot contraction and swelling rate of affected foot and sole were then evaluated. After 1 day of the last administration, the rats were sacrificed with feet perfused and fixed. ELISA method was used to determine the expression of IL-1α, IL-6, IL-10 and TNF-α. The spinal cord was taken out for immunohistochemistry to observe the expression of NMDA receptor in the dorsal horn of the affected side. Results Compared with the control group, rats in the model group developed significant swelling in the ankle and feet (P<0.01). The 2.4 g/kg Tinospora sinensis water extract significantly reduced the degree of swelling at day 12 of administration (P<0.01, compared with the model group, P<0.05). The reaction time of heat-shrinking foot in model rats was significantly reduced and there was significant difference between the two groups (P<0.01, compared with the control group). Different doses of Tinospora sinensis could obviously increase the reaction time of rat’s heat-shrinking foot and prolong the pain-sensitive reaction of affected limbs at different time points, which had statistical significance (P<0.01, compared with the model group). The 50 % mechanical pain response value of the model rats was significantly lower than that of control group (P<0.01). The analgesic effect of Tinospora sinensis group lasted for 12 days, which was significantly different from that of the normal group (P<0.01). Compared with the normal group, serum inflammatory cytokines IL-1α, TNF-α and IL-6 in the model rats were significantly increased (P<0.01), and IL-10 was significantly decreased (P<0.01). After 12 days of administration, Tinospora sinensis water extract could significantly reduce the IL-1α, TNF-α and IL-6 and increase the IL-10 in the model rats (P<0.01, compared with the normal group). Compared with the normal group, the expression of NMDA receptor in the spinal dorsal horn of the injured side of the inflammatory pain model rats increased (P<0.01). Compared with the model group, 1.2, 2.4 g/kg Tinospora sinensis water extract could significantly reduce the expression of NMDA receptor in the spinal cord of rats with inflammatory pain model (P<0.01). Conclusion Tinospora sinensis water extract can effectively reduce the high sensitivity of temperature and mechanical stimulation in rats with inflammatory pain, and its anti-inflammatory and analgesic mechanism is related to reducing the production of peripheral inflammatory factors and inhibiting the expression of NMDA receptors.

    Intestinal absorption of iridoid glycosides in Simiao Yong’an Tang with different compatibility*

    Objective To investigate the intestinal absorption of iridoid glycosides from Simiao Yong’an Tang aqueous extract in different combination. Methods The rat single pass intestinal perfusion model was established. The contents of harpagide, sweroside, loganin and harpagoside from various intestinal segment perfusates were determined by using UPLC-MSn. As an evaluation index for intestinal absorption, the absorption rate constant (Ka) and the apparent permeability coefficient (Papp) of iridoid glycosides from Simiao Yong’an Tang in different intestinal section were calculated. Results In the ileum and duodenum, the absorption of iridoid glycosides from Simiao Yong’an Tang with different combination was better than that in jejunum and colon (P<0.05). The intestinal absorption parameters of four iridoid glycosides in ileum were the highest in the compatibility of jinyinhua (Lonicerae Japonicae Flos), xuanshen (Scrophulariae Radix) and danggui (Angelicae Sinensis Radix). In duodenum, sweroside, loganin showed higher intestinal absorption parameters when gancao (Glycyrrhizae Radix et Rhizoma) was compatible with danggui, while the intestinal absorption parameters of harpagide and harpagoside were the highest in the prescription and the lowest in the single herb. Conclusions The intestinal absorption parameters of iridoid glycosides were higher in prescription than those in single herb. To some extent, these results indicated that the compatibility could promote the absorption of iridoid glycosides and improve the problem of low oral bioavailability.

    Mitochondrial energy metabolism disorder in depression*

    Depression is a major disease which can cause serious damage to human health. Its pathogenesis is still unclear; and the existing monoamine hypothesis failed to adequately explain its multi-system complex symptoms. Moreover, drug therapy also encountered bottlenecks. Many evidences have indicated that mitochondrial energy metabolism disorder is closely related to the occurrence and development of depression, which may become a new target for depression drug develop ment. In this paper, we put forward the hypothesis of mitochondrial energy metabolism disorder in depression. Our hypothesis is that mitochondrial energy metabolism disorder is an important pathological mechanism for the occurrence and development of depression, and a key pathological phenomenon manifesting as the aggravation of mental symptoms and the generalization of physical symptoms. Therefore, improving the disorder can effectively alleviate the mental and physical symptoms of depression. This hypothesis can provide new ideas for the study of depression.

    Objective To explore the correlation between simulated defecography and TCM patterns. Methods Patients with chronic constipation (n=80) were diagnosed as the following patterns: liver-qi stagnation, spleen-lung qi deficiency, spleen-kidney yang deficiency and liver-kidney yin deficiency. PACS system was used to examine patients and collect imaging data. Their correlation was analyzed. Results Through the interpretation of corresponding TCM patterns and defecographical findings, different TCM patterns and related defecography manifestations were distinguished in different quadrants. Specificity analysis showed that there was a strong correlation between spleen-lung qi deficiency and spleen-kidney yang deficiency, while there were significant differences between liver-kidney yin deficiency and liver-qi stagnation. There was no significant difference between the symptoms and signs of perineal descent and mucosal intussusception. Puborectalis syndrome was more specific. Liver-qi stagnation was strongly correlated with puborectalis syndrome, while liver-kidney yin deficiency was strongly correlated with mucosal prolapse and rectocele. Spleen-lung qi deficiency and spleen-kidney yang deficiency were more likely to lead to perineal descent, mucosal intussusception, sacrum-rectal separae, small intestinal hernia and sigmoid hernia. Conclusion The study found that there was a certain correlation between defecographic findings and TCM patterns. It may provide the basis for clinical treatment based on pattern diagnosis and may promote the development of dialectical and Objective differentiation of traditional Chinese medicine.

    Calcifying tendinitis of the supraspinatus treated with thermo-acupuncture: a randomized controlled trial

    Objective To observe the clinical therapeutic effects of the thermo-acupuncture in the treatment of calcifying tendinitis of the supraspinatus and to explore its mechanism. Methods A total of 60 patients with calcifying tendinitis of the supraspinatus were randomly divided into thermo-acupuncture group (TA) (observation group) and electroacupuncture group (EA) (control group), with 30 cases in each group. In the TA group, “Ping Bu” (moderate reinforcing) and “Ping Xie” (moderate reducing) manipulations were applied to acupoints of Taichong (LV 3), Yanglingquan (GB 34), Xuehai (SP 10) and Fenglong (ST 40) in both sides while warm needles were applied to the acupoints of Jianliao (SJ 14) and Jugu (LI 16) in the affected side. The treatment was given once every 4-5 days. After 2-4 times when the symptoms were alleviated, the fire needles were applied to Jianliao points (SJ 14) in the affected side and Ashi points near the calcification once a week, which lasted for eight weeks. The acupoints of the EA group were the same as those of the TA group. Differently, the electroacupuncture was applied to Jianliao (SJ 14) and Jugu (LI 16) points while the other acupoints received moderate manipulations. The treatment was given once every 4-5 days with a total of 8 times. The Visual Analogue Scale (VAS) of shoulder joint pain and the Constant System Core of the shoulder joint function before and after treatment were compared between the two groups; and the changes of calcification were compared before and 3 weeks after the 8 treatments. Results After 8 treatments, the VAS scores were lower (P<0.05) and the Constant scores were higher (P<0.05) in both groups. However, the TA group was better than the EA group (P<0.05). As for the changes of calcification, 25 cases disappeared after treatments in the TA group, which were 83.3% of 30 cases, and 4 cases decreased, which accounted for 13.3%. In the EA group, no calcification disappeared and only 6 cases of calcification decreased after treatments, which were 20% of the total of 30 cases. It was suggested that the efficacy of thermo-acupuncture to eliminate calcification far exceeds that of electroacupuncture. The difference was statistically significant (P<0.05), for the curative rate of the TA group was 96.6% (29/30) and higher than that of the EA group (20/30). Conclusion TA, the combination of warm needle and fire needle, is an independent acupuncture therapy for calcifying tendinitis of the supraspinatus with the advantages of short-treatment term, no trauma, low cost, and high curative effect.
