    How do we learn to use Xiaochaihu Tang*

    The cognitive process of learning to use Xiaochaihu Tang (Minor Decoction of Bupleurum Root) is described for revealing what happened during the process from imitating other physicians to becoming proficient in clinical application. By using the basic logic and psychological methods, the reasons why the application of Xiaochaihu Tang is expanded are discussed based on some basic concepts mainly involved the deductions between shaoyang disease and pattern of Xiaochaihu Tang, shaoyang and bile, and harmonizing and releasing shaoyang and soothing and draining liver and bile. Besides, it points out that the effectiveness of inference will be questioned and the clinical cure rate will decline, with the continuous transformation of the extension and connotation of Xiaochaihu Tang.

    A new interpretation on five-excess and five-deficiency in Huangdi Neijing*

    The statements of five-excess and five-deficiency were put forward in Yuji Zhenzang Lun (Discussion on Genuine-zang Pulses) in Huangdi Neijing: Suwen (Yellow Emperor’s Internal Classic: Plain Conversation). The five-excess contains five symptoms including vibrant pulse, skin fever, abdominal distension, obstruction of urine and stool and delirium, and five-deficiency contains five symptoms including fine pulse, cold skin, less qi, incontinence of urine and stool, and incompatibility of diet. Ancient annotators such as Wang Bing, Zhang Jingyue and Zhang Zhicong all interpreted the statements from the excess and deficiency of five zang-organs, which may lose the original meaning. The paper puts forward a new view that the five-excess is qi movement congesting, i.e. so-called block pattern today, and five-deficiency is pattern of deficient original qi, i.e. so-called collapse pattern today. In the original text, the block pattern is treated with regulating qi movement through perspiration and defecation, and the collapse pattern is treated with therapies of nourishing stomach qi and nourishing the sources of qi. These may be the source of the therapeutic ideas such as opening orifices and block, and replenishing qi and relieving collapse. The “five” in five-excess and five-deficiency means count and emphasizes that all five symptoms are necessary conditions to make a reliable diagnosis. This expression method is conducive to the inheritance of clinical experiences and improvement of diagnostic specificity. The interpretation of five-excess and five-deficiency from block pattern and collapse pattern is logically reasonable and clinically realistic. This classic text may be the earliest record about block pattern and collapse pattern in Chinese medicine.

    Connotation of “extreme yin being related to earth qi” in Suwen*

    Suwen: Liujie Zangxiang Lun (Plain Conversation: Discussion on Six-Plus-Six System and the Manifestations of Organs) is one of important chapters on the theory of visceral manifestations in Huangdi Neijing (Yellow Emperor’s Internal Classic). On the understandings of the passage of “spleen, stomach, large intestine, small intestine, triple energizers, bladder… are extreme yin and they are related to earth qi”, the physicians in successive dynasties had great difference. The paper proposes, after analyzing the problems existing in several common viewpoints on the passage and combining the application of quartered yin-yang model and writing mode in the chapter, that the extreme yin can be regarded as a compromise saying by the author who tried to explain the relationship between four seasons and five zang-organs, and “extreme yin being related to earth qi” can be understand as extreme yin being related to qi of water and food. The paper also indicates that spleen and other zang-organs interact to each other in physical or pathological conditions. So it is a summarization on their functions of water-food conversion to name spleen and other zang-organs as extreme yin, which has clinical significance.

    Rrecognition on kidney storing essence in Huangdi Neijing and clinical understand on male infertility*

    The related thesis on theory of kidney storing essence in Huangdi Neijing (Yellow Emperor’s Internal Classic) and male infertility were discussed combined with clinical practice. Some feelings and experiences are put forward in the paper, including “essence being stored in kidney rather than born in kidney”, “although kidney storing essence and its treatment being in liver”, “focus on liver should being in line with the five elements”, “kidney and essence deficiency being treated considering both spleen and stomach”, “spermatogenesis being easy to move and its treatment should considering warm therapies”, “peace and spermatogenesis intending to fine-tune”, and “expensive being in pattern differentiation and subtle being in modification”. The administration rules of formulas and medicinals are summarized in clinical treatment of male infertility. The important role of liver in the theory of kidney storing essence and male infertility is stressed with hope of providing a broader idea for the diagnosis and treatment of infertility.

    Retrospection and introspection on development of harmonizing therapy and harmonizing-releasing formulas*

    There is a lot of controversy in understanding the connotation of harmonizing therapy and harmonizing-releasing formulas among ancient physicians and modern scholars. On the basis of tracing systematically the development of harmonizing therapy and harmonizing-releasing formulas, the paper discusses their connotations and the relationship between the two, and further puts forward views on the composition of harmonizing-releasing formulas. The study shows that harmonizing therapy is a type of therapeutic method with harmonizing and releasing effects. The harmonizing-releasing formulas, embody harmonizing therapy, have the effects of harmonizing and releasing the exterior and interior and harmonizing zang-fu organs, and their indications are patterns of disharmony between the exterior and interior and among zang-fu organs. The medicinal combinations are commonly groups of medicinals with opposite natures and effects. The indication patterns of harmonizing-releasing formulas involve multiple opposite pathogeneses according to the principle of formulas correlating patterns. Besides, disease locations involve commonly liver, gallbladder, spleen, stomach, heart and kidney, and disease tendency is slower. The research on harmonizing therapy and harmonizing-releasing formulas not only contributes to the perfection of the theory of harmonizing-releasing formulas, but also has a certain demonstrating meaning for the in-depth study on the relationship between therapeutic methods and formulas.

    Academic thought of treating infertility in Fu Qingzhu Nyuke*

    Fu Qingzhu Nyuke (Fu Qingzhu’s Obstetrics and Gynecology) is written by Fu Shan, and it is specialized for gynecology. Common gynecological diseases such as leucorrhea, metrorrhagia, regulating menstruation, pregnancy, gestation and postpartum are listed in the book. The treatment of infertility in the book reflects Fu’s academic thoughts, pattern differentiation thinking, therapies, formulas and medicinals, which has great influence on later generations. In Pregnancy Chapter, Fu’s therapies for infertility emphasize tonifying kidney, liver and spleen, coordinating heart and kidney, reinforcing healthy qi and eliminating pathogenic factors and combining with Menstruation Regulation Chapter, Fu Shan had realized that pregnancy followed regulating menstruation. Fu’s academic thoughts on treatment of infertility are analyzed from above aspects, and the purpose is to further understand the book’s essence, enrich physicians’ clinical experiences and offer services for more infertile patients.

    Influence of Buyin Qianzheng Fang on brain mitochondria in mice with Parkinson’s disease: a study based on Drp1-Ser637 phosphorylation*

    Objective To investigate the possible mechanism of Buyin Qianzheng Fang (BYQZF, Yin-tonifying Healthy qi-leading Formula) in protection and treatment of Parkinson’s disease (PD) through studying the changes and their influence on mitochondria of dynamic-related protein 1 (Drp1) and its phosphorylation site serine 637 (Drp1-Ser637) in PD mice. Methods C57BL/6 mice (n=72) were randomly divided into normal group, model group, BYQZF group and madopar group (each n=18). The PD model was established through intraperitoneal injection of 1-methyl-4-phenyl-1,2,3, 6-tetrahydropyridine (MPTP) in model group, BYQZF group and madopar group. The BYQZF group was orally given decoction of BYQZF twice a day for 28 d, and madopar group was given intraperitoneal injection of madopar once every other day for 28 d. The behaviors of mice were observed by using climbing and gripping methods. The number of positive neurons of tyrosine hydroxylase (TH) in midbrain substantia nigra was observed by using immunofluorescence histochemistry technique. The level of brain mitochondrial adenosine triphosphate (ATP) was detected by using luciferase assay. The brain mitochondrial membrane potential (MMP) was detected by using JC-1 method. The expressions of Drp1 and Drp1-Ser637 were detected by using Western blotting assay. Results Compared with normal group, the climbing time was longer (P<0.01), gripping strength was weaker (P<0.01), number of TH positive neurons decreased (P<0.01), levels of brain MMP and ATP decreased (P<0.01), Drp1 expression increased (P<0.01), and Drp1-Ser637 expression decreased (P<0.01) in model group. Compared with model group, the climbing time was shorter on the 14th d (P<0.05), gripping strength was higher (P<0.05), number of TH positive neurons increased (P<0.01), Drp1 expression decreased (P<0.01), Drp1-Ser637 expression increased (P<0.05), and levels of brain MMP and ATP increased significantly (P<0.01) in BYQZF group. In madopar group only the behaviors of mice were significantly improved on the 14th d (P<0.05), and other indexes had no statistical difference compared with model group. Compared with madopar group, the number of TH positive neurons and levels of ATP and brain MMP had difference (P<0.05, P<0.01), and Drp1-Ser637 expression increased significantly (P<0.01) in BYQZF group. Conclusion Buyin Qianzheng Fang can improve the brain mitochondrial function to protect dopamine neurons in midbrain substantia nigra and prevent and treat PD through down-regulating Drp1 expression and up-regulating Drp1-Ser637 expression in PD mice.

    Influence and mechanism of Danshen Tongluo Jiedu Tang combined with bone marrow stem cell transplantation on myocardial fibrosis in rats with acute myocardial infarction*

    Objective To investigate the influence and mechanism of Danshen Tongluo Jiedu Tang (Salvia Root Collateral-freeing Removing Toxicity Decoction) combined with bone marrow stem cell transplantation (BMSCT) on myocardial fibrosis in rats with acute myocardial infarction (AMI). Methods The bone marrow mesenchymal stem cells (BMSCs) were isolated and cultured by using whole bone marrow adherence method combined with density gradient centrifugation. The model of AMI was established through ligating the left anterior descending branch (LAD) of coronary artery. The rats were randomly divided into sham-operation group, AMI model group, BMSCT group, Danshen Tongluo Jiedu Tang group (Chinese medicinal group) and Danshen Tongluo Jiedu Tang+BMSCT group (combination group). The model was established in all groups except of sham-operation group, and then Chinese medicinal group and combination group were respectively given orally DTJT and other groups were given orally equivalent saline water once a day for 14 days continuously. The content of collagen in myocardial tissue was observed after Masson staining, and concentration of procollagen type III N-terminal peptide (PⅢP), procollagen type IV N-terminal peptide (PIVP), nitric oxide (NO) and endothelin-1 (ET-1) were detected by using ELISA. The expressions of endothelial protein C receptor (EPCR) and thrombomodulin (TM) were detected by using Western blotting assay, and all data was analyzed by using SPSS 19.0 statistical software. Results Compared with sham-operation group, the protein expressions of EPCR and TM decreased, NO level decreased, levels of ET-1, PⅢP and PⅣP increased, and myocardial collagen fiber generation in varying degrees was observed in other groups (P<0.01). The protein expressions of EPCR and TM and NO level increased, and levels of ET-1, PⅢP and PⅣP and myocardial collagen fiber generation decreased in BMSCT group, Chinese medicinal group and combination group compared with AMI group (P<0.01). The protein expressions of EPCR and TM and NO level increased, and levels of ET-1, PⅢP and PⅣP and myocardial collagen fiber generation decreased in Chinese medicinal group compared with BMSCT group (P<0.01). The protein expressions of EPCR and TM and NO level increased, and levels of ET-1, PⅢP and PⅣP and myocardial collagen fiber generation decreased in combination group compared with Chinese medicinal group (P<0.01). Conclusion Danshen Tongluo Jiedu Tang combined with BMSCT can inhibit myocardial fibrosis, and the mechanism may be related to that the protein expressions of myocardial EPCR and TM are up-regulated and vascular endothelial function is protected in AMI rats.

    Diosgenin’s inhibitory effects on proliferation and migration of MCF-7 breast cancer cells through demethylation of microRNA-145*

    Objective To discuss the effect and possible mechanism of diosgenin on MCF-7 breast cancer cells. Methods Human normal breast epithelial cell line MCF-10A and human breast cancer cell line MCF-7 were chosen, and diosgenin in different concentrations was added in the cells. The influence of diosgenin on proliferation of MCF-7 breast cancer cells was detected by using MTS assay. The influence of diosgenin on migration ability of MCF-7 breast cancer cells was detected by using wound-healing test. The effect of diosgenin on methylated microRNA-145 (miR-145) of MCF-7 breast cancer cells was detected by using methylation-specific polymerase chain reaction (MSP). The expression of miR-145 mRNA of MCF-7 breast cancer cells was detected by using quantitative real-time polymerase chain reaction (qPCR). Results Diosgenin inhibited significantly the proliferation of MCF-7 breast cancer cells in a concentration-and time-dependent manner, and proliferation inhibitory rate for MCF-7 breast cancer cells was significantly higher than that for normal mammary epithelial cells MCF-10A (P<0.05). Diosgenin reduced significantly the migration ability of MCF-7 breast cancer cells in a concentration-dependent manner (P<0.01). Compared with control group, miR-145 showed a demethylation state, and expression of miR-145 mRNA increased significantly (P<0.01), which was positively correlated to diosgenin concentration in diosgenin group. Conclusion Diosgenin can inhibit the proliferation and migration of breast cancer cells, and the mechanism may be related to the regulation of expression and methylation of miR-145.

    A systematic review on heat-clearing and toxicity-removing therapy for Alzheimer’s disease*

    Objective To review systematically the curative effect and safety of heat-clearing and toxicity-removing therapy [Huanglian Jiedu Tang (Coptis Root Removing Toxicity Decoction)] on Alzheimer’s disease (AD). Methods The databases of CNKI, WanFang Data and PubMed were retrieved for screening clinical randomized controlled trials (RCTs) about AD treatment with heat-clearing and toxicity-removing therapy from database establishment time to May 22, 2019. The quality of the RCTs was reviewed by applying Jadad scale, and curative effect of Chinese medicinals with effects of clearing heat and removing toxicity was given a Meta-analysis by using RevMan 5.3 software from Cochrane Library. The mean differences of relative risk (RR), mini-mental status examination (MMSE) and activities of daily living (ADL) were calculated. Results There were totally 6 RCTs enclosed and 344 AD cases in test group and 328 in control group. The results of Meta-analysis showed that the clinical improvement rate was higher in test group than that in control group (RR=1.27, 95%CI: 1.08-1.50,I2=0%, P=0.004), and MMSE had significant difference between 2 groups (MD=2.69, 95%CI: 2.13-3.26, I2=84%, P<0.000 01). In addition, heat-clearing and toxicity-removing therapy improved ADL in AD patients (MD=-3.40, 95%CI: -4.92~-1.87, I2=59%, P<0.000 01). Conclusion The heat-clearing and toxicity-removing therapy is safe and effective in AD treatment, and can improve patients’ cognitive function and ADL. It tends to be used in AD patients complicated by behavioral and psychological symptoms in moderate or advanced stages.

    Mechanism of Manpixiao in treatment of chronic atrophic gastritis based on biological network module*

    Objective To discover the potential effect targets and signaling pathway of Manpixiao (MPX), an experiential formula of Professor Lu Zhizheng’s for chronic atrophic gastritis (CAG) through biological network and modular analysis, and explain its potential effect mechanism at point of bioinformatics. Methods The active principles of MPX and of MPX-corresponded target and CAG-related target were obtained by using Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform (TCMSP), BATMAN-TCM, Online Mendelian Inheritance in Man (OMIM), Human Phenotype Ontology (HPO) and DisgeNet Database. The known protein-protein interaction (PPI) was obtained through Human Protein Reference Database (HPRD) and Biological General Repository for Interaction Datasets (BioGRID) for establishing MPX-CAG PPI network. The core functional modules were excavated by using ClusterONE software. Cluego software was used for functional explanation of gene ontology (GO) of the modules, and Kyoto Encyclopedia of Genes and Genomes (KEGG) was used for enrichment analysis of signaling pathway of the modules. Results There were 1885 MPX-corresponded genes and 209 CAG-related genes obtained after retrieving databases. There were 44 nodes and 292 edges for establishing MPX-CAG PPI network, among them there were 31 nodes for establishing core network modules. The results of GO functional explanation and KEGG signaling pathway enrichment analysis showed that the mechanism of MPX in CAG treatment might be related some signaling pathways related to inflammatory reactions and tumors including to IL-17 signaling pathway, inflammatory bowel disease, HIF-1 signaling pathway and VEGF signaling pathway. MPX might take part in GO-related differentiation, metabolism and apoptosis, especially it had important relative effects on targets of tumor protein 53 (TP53), interleukin family cytokines, PTGS2/COX-2, tumor necrosis factor-α (TNF-α), vascular endothelial growth factor A (VEGF-A) and epidermal growth factor receptor (EGFR). Conclusion MPX can alleviate gastric microenvironment possibly through regulating immune inflammatory reactions, gastric acid secretion and VEGF, and play a role in CAG treatment and control of CAG malignant transformation. The study provides a useful exploration on the pharmacological and pharmacodynamic mechanism of MPX. On the other hand, it also provides research ideas and methodology reference for explaining the effect mechanism of famous physicians’ experience formulas.

    Depression and synaptic microenvironment*

    Depression is a major disease that threatens human health, but its pathogenesis is still unclear at present. The structure and functional changes of neuronal synapses play an important role in the pathogenesis of depression. Synaptic microenvironment is the internal and external environment in which neuronal synapses locate. It is affected by various pathological factors such as inflammation, oxidative stress and disorders of mitochondrial energy metabolism. Synaptic microenvironment is related to the development of depression and can provide new ideas for research on depression mechanism. However, there are few studies on synaptic microenvironment currently. Future research needs to pay more attention to the relationship between depression and synaptic microenvironment, and then propose a new strategy for depression treatment targeting the synaptic microenvironment.

    Ulcerative colitis and its TCM pattern-differentiation classification and inflammatory activity indexes: a correlation analysis*

    Objective To discuss the correlation among ulcerative colitis (UC), its TCM pattern differentiation and inflammatory activity indexes through collecting and analyzing the clinical materials from UC patients. Methods UC patients (n=325), who visited Jiangsu Hospital of Chinese Medicine and were eligible to internalized standard, were chosen from Jan. 2017 to Feb. 2019. The patients were classified according to disease activity grades and lesion ranges, and their clinical information was collected for TCM pattern differentiation. At the same time the data of erythrocyte sedimentation rate (ESR), C-reactive protein (CRP) and fecal calprotectin (FC) were collected. The data was analyzed statistically by using chi-square test, independent sample t-test, single-factor analysis of variance and Spearman correlation analysis. The correlation among TCM pattern types, lesion ranges, stages, activity classification of UC and inflammatory activity indexes were discussed. Results The TCM pattern types in 325 patients, the most common one was large intestinal dampness-heat pattern (53%), and secondly ones were spleen deficiency with dampness accumulation pattern (34%) and liver depression and spleen deficiency pattern (13%, P<0.05). The levels of inflammatory activity indexes (ESR, CRP and FC) were higher in group of large intestinal dampness-heat pattern than those in group of liver depression and spleen deficiency pattern, and was a little higher in group of liver depression and spleen deficiency pattern than those in group of spleen deficiency with dampness accumulation pattern (P<0.05). The levels of inflammatory activity indexes (ESR, CRP and FC) increased significantly in patients at UC active stage compared with those at clinical remission stage (P<0.05). The levels of inflammatory activity indexes (ESR, CRP and FC) were significantly higher in severe active group than those in mile active group and moderate active group, and were higher in moderate active group than those in mile active group (P<0.05). The expressions of inflammatory activity indexes (ESR, CRP and FC) were the highest in patients with extensive UC, the secondly higher in patients with left hemicolon UC and the lowest in patients with rectal UC (P>0.05). The TCM pattern type was mainly spleen deficiency with dampness accumulation pattern, secondly was liver depression and spleen deficiency pattern and thirdly was large intestinal dampness-heat pattern in patients at clinical remission stage. The TCM pattern type was mainly spleen deficiency with dampness accumulation pattern, secondly was large intestinal dampness-heat pattern and thirdly was liver depression and spleen deficiency pattern in patients at mile active stage. The TCM pattern type was mainly large intestinal dampness-heat pattern in patients at moderate active stage and severe active stage (P<0.05). The levels of ESR, CRP and FC were positively correlated to Mayo scores in 325 patients, and the relevancy was the highest between ESR, CRP and FC and Mayo scores in patients with large intestinal dampness-heat pattern (P<0.05). Conclusion The levels of ESR, CRP can objectively reflect the clinical activity and severity of UC, and large intestinal dampness-heat pattern has the highest predictive value. It can assist clinical and curative effect reviews and improve UC diagnosis and treatment levels.

    Medication regularity of Xin’an Wang’s internal medicine in treatment of spleen-stomach diseases: a data mining study based on unblocking yang theory*

    Objective To study the formula design and medication regularity for spleen-stomach diseases in Xin’an Wang’s internal medicine based on data mining technology. Methods A database was established by using Microsoft Excel 2010. The association rule analysis was conducted by using frequent itemset algorithm in IBM SPSS Modeler 14.1, and factor analysis was conducted by using IBM SPSS Statistics 21. Results A total of 688 formulas were studied involving 365 medicinals. The administrative times of Fuling (Indian Bread, Poria), Zhiqiao (Bitter Orange, Fructus Aurantii) and Banxia (Pinellia Tuber, Rhizoma Pinelliae) were the highest in single medicinal, and those of Banxia-Zhiqiao, Fuling-Peilan (Eupatorium, Herba Eupatorii) and Gualou (Trichosanthes fruit, Fructus Trichosanthis)-Banxia were the highest in couplet medicinals. The medicinals with cold and warm natures, bitter and sweet flavors, and those enter the meridians of spleen (stomach) and lung (large intestine) were administrated the most frequently. The association rule analysis found 3 groups of the most common couplet medicinals, and there were 13 common factors were extracted by factor analysis. Conclusion Xin’an Wang’s internal medicine takes therapies of regulating qi, freeing yang and dissipating binds as major methods in treatment of spleen-stomach diseases, and takes Gualou Xiebai Banxia Tang (Trichosanthes fruit, Macrostem Onion and Pinellia Tuber Decoction) as a basic formula. The therapeutic methods take regulating qi, promoting spleen, dissipating binds and harmonizing stomach as cores, and pay attention to warm tonification and light formulas. And the therapeutic methods of clearing heat and draining dampness, diffusing qi movement, warming yang and activating blood, and tonifying qi and unblocking vessels were used for regulating yin and yang and remedying defects.

    Clinical medication regularity in treatment of bronchial asthma with Chinese medicinals in recent 30 years: a dada mining study*

    Objective To discuss the characteristics of clinical medication and regularity of formula construction in treatment of bronchial asthma with Chinese medicinals based on data mining. Methods The databases including CNKI, WanFang Data, VIP Database and CBM were retrieved for searching the documents about treatment of bronchial asthma with Chinese medicinals and establishing a database of TCM formulas and medicinals. The study on the documents was conducted through frequency statistics, association rules analysis, system cluster analysis and principal component factor analysis. Results There were totally 1132 formulas included involving 342 medicinals and 60 high-frequent medicinals such as Mahuang (Ephedra, Herba Ephedrae), Gancao (Liquorice Root, Radix Glycyrrhizae), Kuxingren (Bitter Apricot Kernel, Semen Armeniacae Amarum), Dilong (Earthworm, Lurmbricus) and Banxia (Pinellia Tuber, Rhizoma Pinelliae), etc. The medicinals commonly used were those with effects of resolving phlegm, suppressing cough and calming panting, tonifying deficiency and releasing the exterior. There were 32 binomial correlations and 18 trinomial correlations obtained from association rules analysis. There were 19 combinations obtained from clustering analysis, and 21 common factors obtained from factor analysis. Conclusion The treatment of bronchial asthma with Chinese medicinals pays attention to therapies of suppressing cough and calming panting, dispersing wind and diffusing lung, and extinguishing wind to arrest convulsions. When deficiency complications enter in collaterals in chronic diseases, the therapies of tonifying lung, spleen and kidney, and dispelling stasis and freeing collaterals should be stressed.
