      摘要:The equity in health care system is one of guaranteed conditions for every body’s right.With China’s rapid economic growth,the equity of the health care system continues to be challenged.China’s government began a socalled "new health care reform",health care system reform,facing macro-situation and its own demand of health care system in 2009.From the aspect of fairness of health care system,the paper analysed the consideration on equity of health care financing,payment and medical service providers in the reform action program.The author believed that tax financing should be improved,public financing should be used in the social medical insurance(demandside) in order to promote the fairness level in health care financing and payment.The public hospitals play minority roles and show the commonweal of medical service providers,which reflects the equity in the reform action program and high attention on fairness by new health care reform.Some problems were raised lastly in the paper including consideration to both government dominance and market regulation,rural medical security,proper percentage of public funds in health care financing,and location and development of Chinese medicine in this reform.

      摘要:Dayuan Decoction,Shengjiang Powder and Qingwenbaidu Decoction are representative formulas designed respectively by WU You-ke,YANG Li-shan and YU Shi-yu,who were famous doctors of the school of warm pestilence.The paper compared and studied these three formulas from their disease background,medicinal compositions and effects,disease stages aimed at,and modern clinical applications through using comparative method.The outcomes indicated that Dayuan Decoction was aimed at pathogens,Shengjiang Powder was aimed at antagonizing pestilence through improving body’s resistance.Qingwenbaidu Decoction was aimed at clearing away external epidemic toxin or internal toxin.Although the three formulas had different thinking,medicinal compositions and herbal remedies,they all got the best curative effects on pestilence,which enlightened the prevention and treatment of pestilence at modern times.

      摘要:The paper analysed the symptom characteristics of different meridian sinews diseases in neck through studying Lingshu·Jingjin and later comments and biomechanics of whiplash injury.The author thought that the mechanism of lower cervical syndrome of whiplash injury was closely related to abnormal sprain of meridian sinews.According to the interpretations of MA Shi and ZHANG Jie-bin,who were scholars of the Ming Dynasty,and modern anatomy,the paper summarized 4 meridian syndromes of whiplash injury,including syndrome of Taiyang meridian-widespread spasm and tenderness in the neck with difficulty in extension and flexion;syndrome of Yangming meridian-problems of head rotation,pain and spasm in the neck and debility of shoulder;syndrome of Shaoyang-discomfort,spasm of head and neck with difficulty in lateral extension;syndrome of Shaoyin-pain in deep cervical area with difficulty in backward extension and dizziness.The whiplash injury is differentiated,classified and treated by applying the meridian sinews theory in Lingshu,and a better curative effect will be obtained.

      摘要:Objective To sum up the academic thought of Professor LV Ren-he’s in differentiation and treatment of diabetes at stages through the statistical analysis on the factors of TCM syndrome differentiation of diabetes in cases treated by Professor LV Ren-he’s.Methods The diabetic cases treated by Professor LV Ren-he(n=128) were classified into Pidan stage,Xiaoke stage and Xiaodan stage based on the statistical function of TCM Medical Record Database after the pretreatment with resolution and indexing.The causes,pathogeneses and locations were analysed respectively.Results At Pidan stage excessive contemplation and stagnant fire were main causes,and spleen,stomach,liver and kidney were main locations.At Xiaoke stage overeating greased food and raged impairment were main causes,and liver,kidney,stomach and spleen were main locations.The prominent pathogeneses included,at Xiaoke stage,damp-heat accumulation,liver-depression transforming heat,heat accumulating in intestines and stomach,yin deficiency with internal heat,and disquieted heart spirit besides dual deficiency of liver and kidney.At Xiaodan stage senility was an important cause,blood-stasis obstructing collaterals was the most important pathogenesis,and other pathogeneses included deficiency of qi and blood,dual deficiency of yin and yang,phlegm-stasis obstructing collaterals and meridian obstruction.The location statistic showed that five zang-viscera,six bowels and meridians could all be involved at Xiaodan stage.The analysis results of diabetes of three stages were accorded with Professor LV Ren-he’s theory of differentiation and treatment of diabetes at stages.Conclusion Professor LV Ren-he’s theory of differentiation and treatment of diabetes at stages is correspondent with clinical practice.

      摘要:Objective To investigate the mechanism of Qilianqianhua Decoction(QL) in the repairing liver lesion and postponing hepatic fibrosis.Methods The rat model of liver lesion was established by the hypodermic injection of 50% CCI4-oliveoil mixture(2 mg/kg) twice a week for 6 weeks.At the same time the QL group and DDB group were given intragastrically QL(12 g/kg) and dimethyl diphenyl bicarboxylate(DDB,2.25 g/kg) respectively once a day.The content of ALT,AST,ALB,CHE,SOD and MDA were determined.The expression of hepatic PCNA was detected after PLP perfusion and staining of HE and Masson at the end of the 3rd and 6th week.Results Compared with the model group,SOD content increased in QL group and DDB group at the end of the 4th to 6th week,and the difference was significant.At the end of 6th week MDA content decreased in QL group and the difference was significant compared with the model group.The result of immunohistochemistry assay showed that the positive PCNA of liver increased in QL group at the end of 6th week.Conclusion QL can reduce the degree of liver lesion through inhibiting lipid peroxidation and improving liver regeneration.Its effect is better than DDB’s.

      摘要:Objective To observe the influences of different methods of promoting penetration on percutaneous absorption rate of mesaconitine in order to find out the best condition and method for promoting percutaneous absorption of medicinals.Methods All rats were randomly divided into 4 groups,including medicinal group,penetration enhancer group(PE group),group of low frequency electromagnetic complex-pulse(LFEC,instrument group) and group of PE combined with LFEC(combined group).The percutaneous fluid was prepared with the extract of Caowu(Radix Aconiti Kusnezoffii).Azone(4%) and propylene glycol(4%) were added to the percutaneous fluid used in the PE group and combined group.The electrodes of LFEC meter were fixed on skin surface of rats in instrument group and combined group.The quantity of mesaconitine in medicinal fluid absorbed by different groups were detected by using HPLC at different time points.The cumulative infiltration volume and percutaneous diffusion rate were calculated.Results The percutaneous rate was 3.693 μg/(cm2·h) in the medicinal group,32.143 μg/(cm2·h) in the PE group,25.754 μg/(cm2·h) in the instrument group,and 58.517 μg/(cm2·h) in the combined group.Conclusion PE and LFEC have the effect of promoting penetration,while the combination of both have more significant effect with a cooperativity,which can promote percutaneous absorption rate of mesaconitine.

      摘要:Objective To observe the change trend of content of brain interleukin-1β(IL-1β),TNF-α and IL-10 in rats with second cerebral ischemic lesion at four time points(24 h,72 h,120 h and 168 h) after cerebral ischemic preconditioning(CIP),and to discuss the actions of IL-1β,TNF-α and IL-10 in cerebral ischemic tolerance.Methods All rats were randomly divided into 6 groups,including the sham-operation group,ischemia-reperfusion group,group of second lesion after CIP for 24 hours(24-h group),72 hours(72-h group),120 hours(120-h group) and 168 hours(168-h group).All CIP groups were given CIP by the transient occlusion of bilateral carotid artery for 10 minutes,and after 24,72,120 and 168 hours the second ischemic lesion was induced by using suture method to obstruct the middle cerebral artery.In the ischemia-reperfusion group the middle cerebral artery was obstructed for only 2 hours.The content of brain IL-1β,TNF-α and IL-10 were detected by using ELISA in rats with the second ischemic lesion at four time points.Results Compared with the sham-operation group,the content of IL-1β,TNF-α and IL-10 increased significantly in the ischemia-reperfusion group(P<0.05).The content of IL-1β increased significantly in the 24-h group and 72-h group(P<0.05),and decreased gradually in the 120-h group and 168-h group compared with the ischemia-reperfusion group.The content of TNF-α increased significantly in the 24-h group and 120-h group(P<0.05),and trended to reduce in the 72-h group and 168-h group.The content of IL-10 increased significantly in the 24-h group and 72-h group(P<0.01,P<0.05),and trended to reduce in the 120-h group but increased again in the 168-h group(P<0.05).Conclusion After CIP for 24 hours,the content of brain cytokines,IL-1β,TNF-α and IL-10,increased significantly.As trigger substances influenced by CIP,cytokines activated earlier to stimulate the body having endogenous protective effects,resisting the lesion induced by the second ischemia,and improving brain ischemic tolerance.

      摘要:Objective To compare the antiviral effects of major constituents of Huangqin(Radix Scutellariae) on influenza A virus(FM1) in vitro,and determine the main effective constituents of antivirus in Huangqin.Methods The maximum non-toxic concentration(TC0) and median toxic concentration(TC50) of baicalin,baicalein and wogonin on MDCK cells were detected by using MTT.The influences of baicalin,baicalein and wogonin on the cytopathic effect(CPE) of FM1 were observed by using CPE method.The antiviral effective rates(ER) of baicalin,baicalein and wogonin with different concentration were detected by using MTT.The median inhibitory concentration(IC50) and treatment indexes(TI) of baicalin,baicalein and wogonin on viral infection were calculated with Probit regression.The influence of baicalin on the activity of FM1 neuraminidase was detected by using fluorescence method.Results Baicalin had a significant inhibitory effect on the pathogenic effect of influenza virus.When the concentration of baicalin was from 0.015 6 to 0.125 0 g/L,the survival rate of MDCK was higher significantly.The survival rate was the highest when the concentration of baicalin was 0.125 g/L,and ER was 94.6% with IC50 being 0.016 2 g/L and TI being 21.34.Baicalein had some inhibitory effect on the pathogenic effect of influenza virus.When the concentration of baicalein was 0.086 0 g/L,ER was 48.72% with IC50 being 0.077 8 g/L and TI being 8.89.Wogonin had no inhibitory effect on the pathogenic effect of influenza virus with IC50 being 0.023 6 g/L and TI being 0.051.When the concentration of baicalin was or was higher than 0.031 3 g/L,it decreased significantly the activity of viral neuraminidase.Conclusion The antiviral effects of the major constituents of Huangqin,baicalin,baicalein and wogonin,are in a descending order,which indicates that the major constituent with antiviral effect in Huangqin is baicalin.

      摘要:Objective To observe the influences of puerarin on the content of intercellular adhesion molecule-1(ICAM-1),vascular cell adhension molecule-1(VCAM-1),monocyte chemoattractant protein-1(MCP-1),interleukin-6(IL-6) and nitric oxide(NO).Methods Human umbilical vein endothelial cells(HUVE) were collected and divided into the normal group,model group,puerarin group and Rapamycin group.The model group was stimulated by thrombin in a final concentration(5 u/L).The growth of HUVE was detected by using MTT and flow cytometry.The expressions of IL-6,MCP-1,ICAM-1 and VCAM-1 were examined by applying ELISA and NO expression was determined by using the method of nitrate reductase.Results The outcomes of MTT detection showed that when the concentration of puerarin was in 1.0×10-2 mol/L or 1.0×10-4,it inhibited significantly the growth of HUVE stimulated by thrombin(P<0.01).The outcomes of flow cytometry showed that,compared with the model group,the phases of S and G2 induced by thrombin were inhibited(P<0.05),the expressions of IL-6,MCP-1,ICAM-1 and VCAM-1 decreased significantly(P<0.05),NO content increased significantly(P<0.01),and expression of ICAM-1 was not changed(P>0.05) in the puerarin group.Conclusion Puerarin can inhibit the division and proliferation of vascular endothelial cells induced by thrombin,which is manifested as inhibition of phases of S and G2.The mechanism may be related to the increase of NO content,and inhibition of the release of VCAM-1,MCP-1 and IL-6 by vascular endothelial cells.

      摘要:Objective To study the analgesic constituents and mechanism of Daguomujiangzi(Fructus Cinnamomi).Methods Kunming mice were randomly divided into the normal group,placebo group,aspirin group,tramadol group and high-dose,mid-dose and low-dose groups of petroleum ether extract(petroleum ether group 1,2,3),dichlormethane extract(dichlomethane group 1,2,3) and n-butanol extract(n-butanol group 1,2 3) of Daguomujiangzi.Except the normal group and placebo group,all groups were given relevant medicinals intragastrically,and the placebo group was given normal saline water.①Twisting test:mice were injected with 0.6% acetic acid(0.2 mL/g) intraperitoneal 45 minutes after medicinal administration and the times of twisting of mice were observed within 15 minutes.②Hot-plate test:female mice were given relevant medicinals intragastrically,and the time of pain reaction was determined by using hot-plate test after 2 hours.③Formalin test:2 hours after medicinal administration,mice were injected with 2.5 % formalin(20 μL) and pain reactions were detected at 510 min.(Ⅰ phase) and 2030 min.(Ⅱ phase).Results ①All extract fractions of Daguomujiangzi had significant analgesic effects on acetic acid-induced twisting reaction in mice.Compared with the normal group,the times of twisting decreased significantly in petroleum ether groups(P<0.01).②In petroleum ether groups and dichlomethane groups,the significant analgesic effects on hot-plate-related pain were observed and pain threshold was increased significantly compared with the normal group(P<0.01).③In petroleum ether groups,the significant analgesic effects on formalin-related pain were observed,and the pain reaction time was decreased significantly at I phase and II phase.There was a significant difference in analgesic effect between the petroleum ether groups and the placebo group(P<0.05 or P<0.01).Conclusion The liposoluble constituents of Daguomujiangzi have the analgesic effects,which may be related to the central mechanism.

      摘要:Objective To set up a method for quantitatively determining total alkaloids in Fumaotiebangchui(Aconitum flavum Hand.-Mazz).Methods The content of total alkaloids in Fumaotiebangchui was determined by applying spectrophotometry.Aconitine was taken as a reference substance,and bromocresol green as acidic dye.The absorbance was determined when pH value of buffer solution was 3.0,chloroform extractive time was 45 minutes and determining wavelength was 416 nm.Results Aconitine showed a good linear relationship at range of 7.7 mg/L to 26.18 mg/L,the regression equation showed A=0.031 8 C+0.008 3(r=0.999 4) and precision was good(RSD=0.99%).The sample solution was stable within 4 hours after coloration(RSD=1.27%).The method had a higher repeatability(RSD=1.75%).The average recovery was 100.89%(RSD=1.82%).Conclusion The method is easy and accurate,which can be used in the quantitative determination of total alkaloids in Fumaotiebangchui.

      摘要:Objective To observe the content of two butyrophenone constituents in five kinds of decoction pieces of Dahuang(Radix et Rhizoma Rhei),crude pieces,pieces roasted with millet wine and vinegar,and prepared and charring pieces.Methods The content of 4’-hydroxy-phenyl-2-butanone(butyrophenone 01) and 4’-hydroxy-phenyl-2-butanone-4’-O-β-D-(6″-galloyl)-glucopyranoside(butyro-phenone 02) were determined by using HPLC.Methanol-glacial acetic acid(20∶80) was used asmobile phase in gradient elution at the temperature of 30 ℃,and the flow rate was 1.0 mL/min.The detection wavelength was at 280 nm.Results Butyrophenone 01 and butyrophenone 02 were detected in five kinds of pieces.The content of butyrophenone 01 was approximate in the crude pieces,pieces roasted with millet wine and vinegar,and prepared pieces,which increased by 138% in the charring pieces compared with the crude pieces.The content of butyrophenone 02 changed moderately,which was the highest in the pieces roasted with vinegar,increased by 23% compared with the crude pieces,and decreased by 47% in the charring pieces compared with the crude pieces.Conclusion The butyrophenone constituents can be transformed into relevant aglycones after Dahuang was stir-baked to charcoal.

      摘要:Objective To extract the water-soluble and fat-soluble constituents simultaneously from Danshen(Radix Salviae Miltiorrhizae) through selecting one solvent,and to save the costs.Methods The extractive technique of alcohol reflux of Danshen was examined and determined by using orthogonal test method taking tanshinone ⅡA and salvianolic acid B(SalB) as indexes after selecting 4 factors,including alcohol concentration(A),solvent multiple(B),extraction time(C) and times(D).Results If tanshinone ⅡA was extracted,the No.6 technique could be applied:85% alcohol with 12 times water for 2 hours in 2 times.If tanshinone ⅡA and SalB were extracted synchronously,the No.2 or No.3 technique could be applied:75% alcohol with 10 times water for 1.5 hours in two times,or 75% alcohol with 12 times water for 2 hours in 3 times.Conclusion This technique is characterized time-saving,labor-saving and energy-saving,which can be used in the extraction of the water-soluble and fat-soluble constituents simultaneously from Danshen.

      摘要:Objective To observe the curative effect of the therapy of enriching kidney and calming heart in the treatment of the pattern of heart-kidney non-interaction of menopausal syndrome.Methods The cases(n=51) were divided into the treatment group treated with Ninggeng Pellets II(with the actions of enriching kidney and calming heart) and the control group treated with Kunbao Pills.The therapeutic course was 2 months.The levels of serum E2,FSH and LH were detected before and after the treatment.Results The marked efficiency rate was 60% in the treatment group and 42.86% in the control group.The difference between two groups was significant(P<0.05).The symptoms,such as hot flushes and sweating,vexing heat in the chest,palms and soles,palpitation,dryness in mouth and throat,and soreness of waist and knees,were relieved significantly with an effective rate being over 90%.The level of FSH tended to reducing in two groups after the treatment but without statistical significance(P>0.05).The level of E2 increased a little but without statistical significance(P>0.05) too.Conclusion The therapy of enriching kidney and calming heart can significantly relieve the symptoms in the patients with the pattern of heart-kidney non-interaction of menopausal syndrome and influence on the changes of E2,FSH and LH.

      摘要:Objective To observe the curative effect and application value of Tongluo Powder in the prevention of crura deep venousthrombosis after major orthopedic surgery.Method The patients(n=264) were divided into two groups:the treatment group(n=156) treated with Tongluo Powder orally,and the control group(n=108) treated with the injection of low molecular heparin calcium.The curative effect,clinical integrals,thrombus types and incision drainage amount after the surgery were observed.Result The treatment group was better than the control group in the curative effect,clinical integrals and incision drainage amount after the surgery,especially in clinical integrals(P<0.01) and incision drainage amount(P<0.01).There were no toxic and side effects observed in the treatment group,but gum and nasal bleeding and hematoma of injective site were observed in the control group.Conclusion Tongluo Powder is safe and effective in the prevention of crura deep venousthrombosis after major orthopedic surgery.
