      摘要:The current status of research on the prevention and treatment of myelodysplastic syndrome (MDS) withTraditional Chinese Medicine (TCM) was reviewed and analyzed, and through long-term clinical practice, aprecise TCM-based prevention and treatment strategy based on the theory of combination of deficiency and toxin was proposed. The main contents included: (i) precise layered prevention and treatment according to different treatment reactions and conversion risks, through the overall, dynamic, continuous, and accurate prevention and treatment of MDS, affecting the whole process of its occurrence, development, treatment, and outcome; and (ii) to monitor the prodromal diseases pre-MDS, includingIdiopathic cytopenia of undetermined significance (ICUS)andClonal cytopenia of undetermined significance (CCUS), which cause MDS, with the stimulation of hematopoiesis as the treatment target and the treatment is mainly based on tonifying deficiency, accompanied by detoxification, in order to inhibit cloning and prevent or delay the progression to MDS; and (iii) treating lower-risk MDS, emphasis is placed on inhibition of cloning and stimulation of hematopoiesis, while treating tonifying deficiency and detoxicating, thereby improving routine blood test values and the clinical remission rate; and (iv) controlling higher-risk MDS, inhibition of cloning is applied as the treatment strategy, in combination with detoxification as the main therapy and tonifying deficiency at the same time to prolong the time of transformation to leukemia and relapse drug resistance. Based on the focus on the main disease and the main goal of "strengthening the body resistance to eliminate pathogenic factors, attacking, supplementing, and maintaining balance, " the whole course management of "consolidating the root and eliminating pathogenic factors" was emphasized, relevant complications were actively prevented and treated, clinical efficacy and quality of life were improved, body function was restored, and treatment costs were reduced, so that more patients could benefit. At the same time, it was pointed out that the existing clinical literature concerning precision treatment with TCM that could reflect the synergistic advantages of TCM and Western medicine was limited in scope, and there is an urgent need for clinical research that is multi-centered, large in sample size, strictly randomized controlled, and of high quality to develop more high-quality evidence-based medicine.

      摘要:Based on the whole cycle of tumor occurrence, tumor development, and disease outcome, it is necessary to the construction of a theoretical system based on traditional Chinese medicine (TCM) in forthe prevention and treatment of malignant tumors is necessary to maximize the effectiveness of TCM. Our team adheres to the academic thought of "strengthening body resistance, consolidating body resistance, and clearing the source, " breaks through the previous diagnosis and treatment model of single treatment throughout the whole disease process based on the theories of "Yang transforming into Qi, with Yin constituting form" and "Qi rising and falling, " and innovatively proposes the TCM theoretical system of "deficiency-cold-poison-closure-failure in five-phase evolution, depression throughout, tumor poison is the core." Qi deficiency lies atis the root of tumorspathogenesis, and Yang deficiency and cold condensation coagulationare the cause of disease progression.Tumor toxin stagnation is athekey factor, while evil accumulation of poison is thea sign of gradual deterioration, and vital Qi decline is associated with poor prognosis. Following the core, based on strengthening qi and cultivating vital energy, warming yang and detoxication is the core, regulating qi throughout and truncating the evolution of the disease. This system interprets the core pathogenesis and evolution of each tumorstage, improves the diagnosis and treatment system of TCM, and provides a theoretical basis for realizing full-cycle management and accurate differentiation and treatment.

      摘要:Mingmen theory was formed in the Ming Dynasty. From the perspective of scientific model theory, Mingmen theory constructs the ontology model of Mingmen. The model group includes a congenital model and an acquired model, which is an ideological model combining the image model and the ideal model. The noumenon model of Mingmen is based on distributed reasoning, which integrates scientific investigation and selection with trial and error. The model adopts ideal model reasoning and the remote analogy method. It has been verified in practice, and it has led to the differentiation of noumenon . This differentiation embodies the essential attribute of deficiency syndrome, and it is the ultimate differentiation of zangfu organs. The differentiation of noumenon was used to treat chronic disease, serious disease, and critical disease, which played an important role in the Ming and Qing Dynasties. The noumenon model of Mingmen provides a brandnew theoretical basis for syndrome differentiation and the treatment of internal injuries and diseases. Our task is to organize and standardize the model and differentiation theory in a systematic and scientific way.

      摘要:TraditionalChinese medicine(TCM)lies atthe basis of carrying forwardChinese culture, and the characteristics of Chinese culture can be elucidated on the basis of Confucianism. Although the original meanings of medicine and Confucianism are quite different, the two have been blended in the long history of Chinese civilization. This article first analyzes the unidirectional relationship between Confucianism andmedical application , including the combination of technique and knowledge, the lack of essentials, failure to connect to each other, and difficulty in interpretation. Second, it horizontally explains the reverse creation of classic medical books with a Confucian interpretation, including improving the medical state, interpreting scripturesquoting scriptures to interpret scriptures , textual research of the medical compendium, and introducing new knowledge. Third, it expounds their mutual involvement and application in the multivariate harmony of medical and Confucian interests.Whether it is the benevolent talk of Confucianism by doctors or the sincere words on medicine by Confucianists, they areit is one of the powers that shape the style of Chinese medicine.Only byintensively studying classicsdetailing, deeplyexamining and reasoning, and seeking common grounds whilepreserving differencesreserving differences, can we accurately explore the influence of academic thoughts on TCM and comprehensively describe the cultural trajectory of the integration of medicine and Confucianism.

      摘要:The theory of spleen and stomach occupies an important position in traditional Chinese medicinetheory because of its great theoretical value and clinical application value. In this paper, the similarities and differences between Li Dongyuan’s spleen and stomach theory and Huang Yuanyu’s spleen and stomach theory were analyzed from the perspectives of physiology, pathology, treatment models, and medication characteristics. Physiologically, both doctors affirmed the close relationship between the innate qi and the spleen and stomach, which are the main organs affectingqi’s rise and fall . Li Dongyuan believed that the spleen and stomach played a pivotal role together, while Huang Yuanyu believed that the spleen and soil dominated the rise and the stomach and soil dominated the fall. In the relationship between spleen and stomach, Li Dongyuan paid more attention to stomach qi, while Huang Yuanyu paid more attention to spleen soil. In pathology, Li Dongyuan believed that internal and external injuries were mostly caused by the maladjustment of air and fire and the abnormal rise and fall. Huang Yuanyu often argued that the soil was wet because of yang deficiency and the middle qi was not carried. In treatment, Li Dongyuan and Huang Yuanyu used the theoretical model of zangfu shengke’s five elements and the five elements of earth in yin-yang, respectively. In terms of dispatching prescriptions, both prioritized the spleen and stomach. Li Dongyuan used a wide range of medicines and a low dosage of drugs, and the emperor and minister ensured their compatibility . Huang Yuanyu uses small does, but his effects are concentrated.

      摘要:The study of traditional Chinese medicine (TCM) theoretical literature is an important part of TCM scientific research. At present, there is a lack of unified reporting norms in TCM theoretical literature research. By discussing the significance of TCM theoretical literature research and the main points of required reports, this paper discusses the necessity and feasibility of standardized reports of TCM literature research. The standardized report scheme described in this paper consists of 7 parts and 19 items, namely: (1) summary; (2) background; (3) methods (introduction of research ideas; sampling methods; description of samples; introduction of auxiliary tools; description of the analysis process; methods of quality control; preservation of raw data); (4) results (descriptive report of results; collection of important notes, topics, and information ; visual display of the results); (5) discussion (answered scientific questions; innovation; limitations); (6) conclusions; and (7) acknowledgments, funding, and references. It can improve the reliability of TCM theoretical literature research and reduce the heterogeneity of the research. For this reason, report standardization is of significance to TCM theoretical literature research and the inheritance, development, and dissemination of TCM theory.

      摘要:Most of the ancient Chinese medicine books are written in classical Chinese. The characters "left" and "right" appear many times in these books, initially referring to "left hand" and "right hand." Later generations evolved and extended its concept and connotation, giving it multiple meanings. This article studies the meaning of the words "left" and "right" in ancient books.Our main findings are as follows: (1) the majorityof the time, "left" and "right" still refer to the left and right hands, and sometimes they are extended to the left and right sides of the human body; (2) left is yang and right is yin, but the kidney’s yin is left and the kidney’s yang is right; (3) the right is the place of qi and the left is the place of blood; however, Zhang Jiebin emphasized that qi and blood are fundamental to each other and cannot be divided into left and rightforcibly; (4) when diagnosing diseases, there is a rule of "typhoid fever takes the left vein as the surface and the right vein as the inside, miscellaneous diseases take the right vein as the surface and the left vein as the inside ; " and (5) the fetal male and female can be distinguished from the aerodynamics of the left and right yin and yang of the pregnant woman, the left and right sides of the uterus, and the left and right pulses, and the left and right sides may be directly used to refer to the gender of men and women. In short, in the study of ancient Chinese medicine books, the meaning of a word or wordsdepends on the historical background, the context of the text, and the writing habits of the author.

      摘要:The Maziren pill was first described in the "Treatise on Febrile Diseases." It is mainly used to treat spleen syndrome and constipation. At present, it is generally believed that The main pathogenic mechanism of this formula is heat in the stomach and dryness in the intestines, and the ruling herb is hemp seed kernel, which is classified as a laxative, and the primary ingredient is Huomaren, which is classified as a moisturizer. The above misunderstandings about Main treatment mechanism, ruling medicine, categorization are the primary reasons for the abuse of this prescription for the long-term treatment of habitual constipation. Based on textual research and analysis of prescriptions, we believe that the root cause of the above problems is that the "Treatise on Febrile Diseases" does not indicate a unified unit of measurement to indicate the dosage of drugs. In addition, Cheng Wuji’s study was deficient-it did not confirm that stomach heat was the key subject of the formula, and mistakenly considered fire hemp seed as the key drug of the formula.We believe that a burning stomach lies at the root of the pathogenesis of the prescription. The burning stomach is not healed, the dry intestines are difficult to heal, the stomach heat is drained, and the dry intestines are self-moisturizing. The prescription has the largest dosage and strength, which is Key Medications; classifying this prescription as a cold medicine is not only in line with Zhongjing’s original intention, but it also has practical significance in limiting the abuse of this prescription.

      摘要:The vague depiction of the pulse in traditional pulse diagnosis and the lack of an objective physical measure not only hinder pulse diagnosis, but they also make it impossible to interface with modern scientific systems.In the following, we report our scientific and modern reconstruction of the pulse diagnosis system based on the principles of Huangdi Neijing, "the diagnosis of abnormal pulse" , and Jingui Yaolue, "the pulse should be taken as too much or too little" . We conducted Systematic Sphygmology for Pattern Differentiation breaking through the limitations of physical images, and we extended the concept of "pulse condition elements" beyond the physical level.Our analysis clearly answers the question of "too much or too little" of cognitive pulse diagnosis put forward by the ancients. "Physical image" refers to the use of descriptive language that is difficult to quantify to describe an object; in contrast to the physical image, "physics" refers to the use of measurable physical quantities to objectively describe things, so as to more clearly present pulse information.The reconstruction of the pulse diagnosis system is very important for clinical and scientific research.

      摘要:ObjectiveWe aimed to investigate the mechanisms underlying the effects of Illicium henryi injection on rheumatoid arthritis (RA).MethodsMale DBA/1 mice were randomly divided into: (i) the normal group, (ii) the model group, (iii) the leflunomide group, and (iv) the Illicium henryi injection group (n = 8 mice per group). A collagen-induced arthritis (CIA) mouse model was established by multipoint injection of bovine type Ⅱ collagen emulsifier in the tail and back. Mice in the normal group and model group were given normal saline. Mice in the leflunomide group were given leflunomide (3 mg/kg) by gavage. Mice in the Illicium henryi injection group were intraperitoneally injected with Illicium henryi injection (0.3 mL/kg) once a day for 28 consecutive days. Joint inflammatory scores were recorded every 7 days after the intervention to measure the degree of joint swelling. After 28 days, HE staining was used to observe the histopathological morphology and record the histopathological score of the ankle joint. Safranin O staining was used to observe the degree of damage to articular cartilage and score the loss of staining. The protein expression levels of JAK2, p-JAK2, STAT6, and p-JAK6 in synovial tissues were determined by Western blot.ResultsCompared with the model group, the arthritis index score was lower in the Illicium henryi injection group (P<0.05), the foot joint was thinner (P<0.05, P<0.01), the pathological changes of the joint were less severe, and the expression and phosphorylation of JAK2 and STAT6 in synovium were lower (P<0.05, P< 0.01).ConclusionIllicium henryi injection has anti-inflammatory and immunomodulatory effects on CIA mice through inhibiting the phosphorylation and expression of JAK2 and STAT6 and regulating the JAK/STAT signaling pathway.

      摘要:ObjectiveTo explore the effects of different Pinelliae Rhizoma products on the intervention of pulmonary fibrosis.MethodsUsing transforming growth factor β 1(10 μ g / L) induced human embryonic lung fibroblast cell line MRC-5 to establish an in vitro model of pulmonary fibrosis then treated with waterextracts of Pinelliae Rhizoma(PR)or Pinelliae Rhizoma Praeparatum cum Alumine(PRPCA)or Pinelliae Rhizoma Praeparatum cum Zingibereet Alumine(PRPCZEA) or Pinelliae Rhizoma Praeparatum(PRP). The working concentration(PR, PRPCA, PRPCZEA, PRP) equivalent tothe crude herb was 2.1, 4.2, 8.4, 16.7 g/L respectively. After 48 hours, the cell activity was detected by CCK-8 method and the collagen content was determined by Sirius red staining method. After screening, the expression of α-SMA protein inmodeltreated with PRwas assessed using Western blot. In vivo, C57BL/6N mice were exposed to bleomycin then treated with water extract of PR (2.5, 5.0, 10.0 g/kg) on day 14. On the 28th day, the lung tissue was observed by he and Masson staining, and the content of hydroxyproline (Hyp) was measured.ResultsCompared with the control group, 16.7 g/L of PR or PRPCA inhibited the cell viability of MRC-5 (P<0.05), while 8.4 g/L of PR can selectively suppress the cell viability of MRC-5 after TGF-β1 stimulation (P<0.05). Compared with the model group, PR (8.4, 16.7 g/L) reduced the synthesis of collagen in MRC-5 cells induced by TGF-β1 (P<0.05), but PRPCA, PRPCZEA and PRP groups had no statistically significant difference in inhibitory effects on collagen. Western blotanalysis showed that PR (2.1、4.2、8.4 g/L) decreased the expression of α-SMA in MRC-5 cells induced by TGF-β1 compared with the model group(P<0.05).In vivo experiments, the bleomycin-treated mice receiving PR (10 g/kg) displayed less collagen deposition in the lung interstitium, less inflammatory infiltration, and more restored alveolar structure compared with mice only receiving bleomycin. PR of 10 g/kg reversed HYP level in lung tissue after bleomycin challenge (P<0.05).ConclusionPR reduces cell viability of pathological myofibroblasts induced by TGF-β1, suppresses differentiation of myofibroblasts, and reduces the synthesis of collagen. Administration of PR in vivo decreases ECM deposition and exerts antifibrotic effects. PR may be used to treatpulmonary fibrosis in the future.

      摘要:ObjectiveWe observed the clinical efficacy of modified Suanzaoren formula combined with Wuxing music in the treatment of primary insomnia.MethodsA total of 66 patients with primary insomnia treated in the Third Affiliated Hospital of Beijing University of Chinese Medicine between January 2021 and June 2021 were selected and randomly divided into the observation group and control group using a random number table, with 33 patients in each group. In the control group, 30 cases were observed (3 cases dropped out) and were given sleep health education with traditional Chinese medicine characteristics. In the observation group, 31 cases were observed (2 cases dropped out) and were treated with modified Suanzaoren formula combined with Wuxing music therapy on the basis of sleep health education. The treatment course for both groups was 4 weeks. The Pittsburgh Sleep Index scale (PSQI), insomnia severity scale (ISI), excessive awakening scale (HAS), and fatigue scale (FS-14) were used to evaluate the efficacy of the two treatments.ResultsAfter treatment, the total effective rate was 90.32% in the observation group and 63.33% in the control group. The PSQI, ISI, HAS, and FS-14 scores of the two groups were better than those before treatment (P< 0.01, P<0.05). The curative effect of the observation group was superior to that of the control group in terms of sleep quality, time to fall asleep, and the degree of physical fatigue (P< 0.01, P<0.05).ConclusionModified Suanzaoren formula combined with Wuxing music is effective in the treatment of primary insomnia, especially with respect to sleep quality, falling asleep time, and physical fatigue, which may be related to the neuroregulatory effect of modified Suanzaoren formula combined with Wuxing music.

      摘要:"Inflammation-cancer transformation" is a general term for the transformation of inflammation-related diseases to the occurrence and development of cancer. Traditional Chinese medicine(TCM)recognizes that inflammation is caused byfire-evil, heat-evil, and toxin-evil .Blocking the process of "inflammation-cancer transformation" can delay the occurrence and development of lung cancer.Based on the theory of "heat polarized toxicity" and the ancient and modern understanding of inflammation and cancer, this study concludes that "heat polarized toxicity" may be the core pathogenicfactor of lung "inflammation-cancer transformation." However, fire-evil and heat-evil should be separated from the manifestation and root cause of the disease . From the point of view of symptoms and reality, the stagnation of evil leads to fire and heat. From the perspective of root cause and deficiency, the deficiency leads to yin fire-evil, which causes accumulation . Subsequently, the inflammation becomes cancer. Furthermore, the differential treatment of lung "inflammation-cancer transformation" has been put forward. On the one hand, we should attach importance to the cause, clearing away heat and detoxification in order to attack external evil.On the other hand, we should cure the disease by seeking root, tonifying soil, and promoting gold to support positive deficiency. In addition, our treatments should be in tune with physical function, and Xuan Tong should be suitable for compliance with the lung. We provide a theoretical basis for TCM intervention in the process of lung "inflammation-cancer transformation" and a useful reference for the prevention and treatment of lung cancer based on TCM.

      摘要:Endometriosis (EM) is characterized by progressive progression. Emotional abnormality is an important factor promoting the progression of EM. Blood stasis is the basic pathogenesis of EM, and qi stagnation is the key link. Liver depression, spleen deficiency, and liver and kidney essence deficiency are involved in it. The liver exhibits rise and dispersion, where too much vitality will break the balance of "birth control inhibition conversion" of kidney essence, which is the driving force of EM progression. The liver "zang qi movement" is deregulated, leading to reduced activity of qi, retrograde blood flow, blood stagnation, and EM progression. When the liver rises too much, this will negatively affect spleen qi, consume liver blood, reduce the kidney essence, and damage the function of multiple organs, providing favorable conditions for EM progression. Upon deficiency of liver and kidney essence, liver yang cannot be nourished, which in turn aggravates mood abnormalities in EM patients. In EM, blood stasis, liver depression, spleen deficiency, and liver and kidney essence deficiency coexist and affect each other. In clinical practice, we should take "relieving depression and activating blood circulation" as the primary treatment and also put emphasis on invigorating both spleen qi and kidney essence to consolidate the foundation and support healthy qi.

      摘要:In traditional Chinese medicine (TCM) terminology, "kidney accumulation" mainly refers to a visible, localized mass of the kidney that does not disappear after onset. The "kidney storing essence" function described in TCM includes the endocrine function of the adrenal gland. Primary aldosteronism is caused by adrenal adenoma, nodules, or hyperplasia leading to the excessive secretion of aldosterone in the cortical glomerular zone, which results in heightened blood pressure and decreased blood potassium levels. Therefore, primary aldosteronism belongs to the disease category of "kidney accumulation" in TCM. The pathogenesis of "kidney accumulation" is based on primary deficiency and superficial excess: deficiency refers to kidney deficiency, mainly kidney yang deficiency; excess refers to phlegm and blood stasis, mainly blood stasis. In the course of treatment, the key treatment principles lie in tonifying deficiency and eliminating excess according to the different stages of disease. Basic treatment involves warming the kidneys and tonifying yang, removing blood stasis, and eliminating accumulation; it should be further supplemented by nourishing kidney yin and eliminating phlegm and dampness.The Guizhi Fuling pill and the Jisheng Shenqi pill were used as the base formulas for treatment in this study, and dosages were adjusted in accordance with individual clinical manifestations of symptoms.

      摘要:Micro-lumps in liver is a pathological stage that occurs in the late stage of the development of a variety of chronic liver diseases ; itis equivalent to liver fibrosis and other diseases in western medicine.According to traditional Chinese medicine (TCM), the basic pathogenesis of micro-lumps in liver is deficiency of the original qi, deficiency of the vital qi, and turbidities of phlegm and blood stasis , the foundation is Qi deficiency, and the presentation is phlegm turbidity and blood stasis, which forming the basic pathogenesis of micro-lumps in liver. Besides, the phlegm turbidity and blood stasisamong which the intermingled phlegm and blood stasis isare the key pathological factorsaffecting the whole disease throughout the whole process of the disease.The basic principle of the treatment of this disease is to simultaneously treat phlegm and blood stasis.Studies have shown that liver fibrosis in Wilson disease belongs to the category of micro-lumps in liver.Based on the theory of micro-lumps in liver, this paper discusses the TCM etiology, pathogenesis, clinical outcomes, and intervention strategies of liver fibrosis in Wilson disease.It is concluded that the disease is the accumulation of micro-lumps in liver, the intermingled phlegm and blood stasis is the basic pathological feature of the disease, the deficiency of vital qi is the key to its pathogenesis, and the general treatment is aimed at strengthening body resistance to eliminate pathogenic factors and dispel phlegm and remove blood stasis at the same time.We providea reference for the clinical prevention and treatment of liver fibrosis in Wilson disease.

      摘要:This research analyzes the theoretical connotation of "stasis, " "toxin, " and "stagnation" based on their definition, and it clarifies their implication in "the combination of stasis-toxin-stagnation. "Among them, " stasis " is not only a pathogenic factor and a pathological product, but it also refers to the pathogenesis of blood stasis in acute cardio-cerebrovascular events. "Toxin" mainly refers to endogenous toxin, which has the characteristics of suddenness and severity , and these characteristics are consistent with the clinical manifestations in the acute phase. "Stagnation" mainly includes stagnation of qi at the level of pathogenesis and depression at the level of disease. On this basis, our research also illustrates the pathogenesis of "the combination of stasis-toxin-stagnation" in acute cardio-cerebrovascular events with depression. First, stasis-toxin is the pathological basis of acute cardio-cerebrovascular events. Second, depression exists in the whole process of the pathogenesis of stasis-toxin. Finally, stasis-toxin is the root cause of the sudden aggravation of depression in the acute phase. Bibliometric analysis reveals that "stasis" is the common core pathogenesis element of acute cardio-cerebrovascular events and cardio-cerebrovascular events with depression. "Toxin" is one ofthe most important pathogenesiselement of acute cardio-cerebrovascular events. "Stagnation" is the key pathogenesis element of cardio-cerebrovascular events with depression. These results verify the objectivity and importance of the pathogenesis of "the combination of stasis-toxin-stagnation" in acute cardio-cerebrovascular events with depression and provide a theoretical basis for clinical research.

      摘要:"Primordial qi" and "fire" are the necessary material basis and power of energy metabolism. Their misbehavior is closely related to the pathomechanism of cancer cachexia. Deficiency of primordial qi is the root of the internal friction of cancer cachexia, and the hyperactive fire is the key to the development of cancer cachexia, which is in accordance with the pathogenesis of maladjustment of the relationship between primordial qi and fire proposed by Li Dongyuan: "Yin-pathogenic fire and primordial qi are opposites, one victory leads to one loss." There is no material basis for the essentialwater-food metabolism; because of the deficiency of primordial qi, the hyperactive fire causes the yang qi to fail to diffuse, and these phenomena provide an internal environment and power for abnormal metabolism in cancer cachexia, which is similar to the pathogenesis of cancer cachexia as described by modern medicine. In this way, the method of primordial qitonification and Yin-pathogenic fire purgation is the general principle, with the key point being that the power of wind medicine assists yang, resolves phlegm and stasis. As such, fire and primordial qi are kept in their respective positions to prevent the progression of cancer cachexia, thus providing ideas and a theoretical basis for clinical intervention of tumor cachexia.
