摘要:"Heart governing the spirit light" is an important statement in the basic theory of traditional Chinese medicine, which was first described in Plain Questions (Suwen). Previous academic research on this statement mainly focused on the location and function of "heart" from the perspective of individuals. Based on the philosophy of human-nature in ancient China, this paper analyzes the theoretical connotation and explanatory dimension of "heart governing the spirit light" . This study aims to further deepen the understanding of the basic theoretical propositions of traditional Chinese medicine. Firstly, with the relevant cases, the authors point out that "the magical changes of nature" is the important semanteme of the "spirit light" in Huangdi Neijing (Huangdi’s Inner Classics), and "heart" is the conceptual carrier that presents changes and trends in individuals. Secondly, this paper discusses the connotation and theoretical extension of "heart governing the spirit light" from the following three perspectives: "experience and observation" , "acknowledging phenomena" and "flowing and moving" in association with body cognition and theory of disease. Finally, based on above viewpoints, the article discusses the "heart-brain relationship" in Huangdi Neijing. It is pointed out that heart has the fundamental and transcendental value orientation in the ideological context of Huangdi Neijing. To a certain extent, heart-brain relationship reflects the dialectical thinking mode of existence and vacuity in the ancient philosophy of life. In the process of theoretical development, the proposition of "heart governing the spirit light" has always been closely related and deeply integrated with the theory of body cognition and disease treatment, which is a typical example of innovative interpretation of the relationship between nature and human in ancient China by traditional Chinese medicine.

摘要:The theory of traditional Chinese medicine holds that "heart governs the spirit light" . The core pathogenesis of heart diseases with cognitive impairment is the dysfunction of the mentality dominated by the heart. Firstly, this paper summarizes the relationship between heart and brain in physiology and pathology from traditional Chinese medicine and modern medicine. Among them, "the meta-mind is in the brain and the awareness-spirit in the heart" is a highly generalization of heart and brain’s role in controlling people’s spiritual consciousness and thinking activities. The heart belongs to the five viscera, and the brain is an organ of extraordinary fu-viscera, so the spirit light of brain should also be regulated by the spirit light of heart. The blood-pumping and endocrine functions of the heart are the physiological basis of the influence of the heart on brain function, and heart and brain diseases also share common pathological basis and risk factors. Then the etiology and pathogenesis of cognitive impairment associated with heart diseases such as heart failure, atrial fibrillation, hypertension and coronary heart disease are then summarized. From the perspective of traditional Chinese medicine, the disorder of "heart governing the spirit light" is the underlying pathogenesis. Heart qi deficiency, or heart blood deficiency, or yang deficiency without warmth, could result in the loss of nourishment of mind, i. e. the disorder of body and mind; Or substantial pathogenic factors blocking the heart could bring about disturbed mind and impaired cognitive function. Modern medicine finds that cardiovascular diseases can lead to the decline of cognitive function by affecting cerebral hemodynamics, nerves and blood vessels to injure brain tissue structure or function. The influence of heart on brain function provides a scientific basis for the theory of "heart governing the spirit light" .

摘要:Psycho-cardiological disease refers to the presence of somatic cardiovascular symptoms associated with negative emotions, with or without cardiovascular disease. Cogntion is the whole process of the brains receiving and processing of external information, from which cognitive psychology is developed and has become an important component of modern psychology research; its core content is to emphasize the negative mood plays a key role in the development of emotional disorders, and provides a new perspective for the understanding of psycho-cardiological disease. The theory of Traditional Chinese medicine holds that the heart is closely related to the cognitive process. The epistemic process is highly summarized in the 8th chapter of Basic state of Spirit (benshen) in Miraculous Pivot (Lingshu) as "perception, memory, mind, volition, thinking, consideration and wisdom" . As the heart is the chief of all the organs, it not only controls the blood vessels and consciousness, but also dominate other organs. The heart leads the sensory organs to perform the function of "perception" , completes the process of "memory, mind, volition, thinking and consideration" by regulating the kidney, spleen and liver, and finally integrates all the information to determine the result of "wisdom" , which makes a systematic connection between the heart, cognition and emotion. Based on this, we put forward the principle of calming the mind as the main treatment, supplemented by invigorating qi and promoting blood circulation, strengthening spleen and reducing phlegm, regulating liver and qi, communicating heart and kidney. This paper adds to the scientific connotation of psycho-cardiological disease and simultaneous heart-brain tretment.

摘要:Psycho-cardiological disease is commonly seen in cardiology as coronary heart disease tends to be accompanied by anxiety and depression in clinic. We put forward the psycho-cardiological theory of traditional Chinese medicine based on the physiological functions of "heart governing blood and vessels" and "heart governing spirit light" in traditional Chinese medicine, combining with the idea of "harmony between body and mind" in psychosomatic medicine. In this theory, the heart of the blood vessels and the heart of the spirit are physiologically dependent, and pathologically correlated, resulting in psycho-cardiological disease. Based on clinical observation, we hold that "blood stasis, heat, deficiency, and stagnation" are the primary pathogenesis of psycho-cardiological disease, in which blood stasis is the foundation, heat aggravates the condition, deficiency is the key, and stagnation is the trigger. After reviewing the relevant mechanism studies, it is concluded that "blood stasis and heat" are correlated with the activation of inflammatory pathways, and "deficiency and stagnation" are correlated with "mitochondrial energy metabolism disorder" . In addition, there is a coincidence between inflammatory pathways and mitochondrial energy metabolism disorder. Therefore, from the perspective of the pathogenesis of "blood stasis, heat, deficiency, stagnation" , we can provide certain perspectives for the treatment of psycho-cardiological disease with traditional Chinese medicine, combining inflammatory mechanisms and mitochondrial energy metabolism processes to further explore the roles of "blood stasis" , "heat" , "deficiency" and "stagnation" in the development of coronary heart disease with anxiety and depression.

摘要:Thyroid cancer is the fastest growing type of tumor globally. Most patients have a long survival period. The management of the disease is similar to that of chronic diseases. Traditional Chinese medicine plays an important role and is of great value in the diagnosis and treatment of thyroid cancer. This paper comprehensively summarizes and analyzes the current situation, research progress and existing problems of TCM diagnosis and treatment of thyroid cancer from the following aspects: TCM diagnosis and treatment indications and characteristic advantages of thyroid cancer, randomized controlled clinical research on the treatment of integrated traditional Chinese and Western medicine after surgery, inheritance and innovation of TCM diagnosis and treatment methods, pattern identification and standardization of diagnosis and treatment. Future research and development directions are thus proposed. It is suggested that the efficacy evaluation should combine "evidence-based research" with "real world study" and "evaluation of symptoms, patterns, quality of life and the relapse risk of thyroid cancer" ; We should form an effective TCM diagnosis and treatment protocol and guidance with distinctive TCM characteristics while integrating the new knowledge of modern medicine; Starting from the clinical questions and unmet clinical needs, we should focus on the key points and expand and deepen the research field, in order to provide reference for improving the TCM diagnosis and treatment level and academic development of thyroid cancer.

摘要:In order to explore the approaches to inheritance of legacy in traditional Chinese medicine (TCM), the academic ideas of the three academicians of Chinese Academy of Sciences, Dong Jianhua, Wang Yongyan, and Tian Jinzhou, are analyzed in this paper through literature review. It is discovered that innovation is embedded in each generation of academic heirs. The first-generation academician Dong Jianhua founded the "Unblocking and Descending Theory" , which took "unblocking" as the principle for the treatment of gastrointestinal and other internal diseases. The second-generation academician Wang Yongyan founded the "Collateral Disease Theory" , putting forward the new idea of treating major and refractory diseases in brain, heart and kidney according to the collateral theory. The third-generation academician Tian Jinzhou proposed the "Pattern Cascade Hypothesis" , which provided new theories and methods for the prevention and treatment of encephalopathy and other chronic diseases. The development of traditional Chinese medicine depends on inheritance of academic legacy, and the primary approach is innovation, which could be the inexhaustible source of inheritance.

摘要:The denotation of traditional Chinese medicine (TCM) terminology is one of the core issues in TCM theoretical studies. Fundamentally, it is about the relationship between "term and matter" . Studying TCM terminology is of significant importance as the denotation of TCM terminology tends to change between "analogy" and "definition" . The denotation could refer to the "definition" in some cases and "analogy" in others, causing confusion and inconsistency. This paper takes terminology of TCM etiology and pathogenesis as examples to analyze the different forms of "definition" and "analogy" of TCM etiology and pathogenesis, then analyzes the transition mechanism of cognition from "definition" to "analogy" and from "analogy" to "definition" based on the similarity of metaphor. After analyzing the rationale of "definition" and "analogy" in the terminology, it is concluded that TCM has no independent system of language and adopts single-focus presentation of metaphorical cognition. These two features explain the underlying rationale. This paper also analyzes whether wind and cold or bacteria and virus are the etiologies of exogenous disease. It is stated that the etiologies should be "something like wind" , "something like cold" and "something like damp" and expounds the significant of studying TCM language from the perspective of metaphorical cognition.

摘要:This paper analyzes the original meaning of Yangming based on the theory of the opening and closing of the three yin and three yang in Huangdi Neijing, and concludes that the connotation of Yangming refers to the state of descending yang qi.In nature, Yangming is the qi of Zaojin. In the human body, Yangming refers to the qi’s right way in the theory of cyclical flow of qi and the general term for the function of qi’s closing and descending.Yangming has a broad and narrow meaning in the human body.In a broad sense, Yangming is a generalization for the function of the qi’s converging and descending in the whole body, including all of the viscera and meridians that are responsible for closing and descending.In a narrow sense, Yangming only refers to stomach and intestines and their meridians.In a broad sense, Yangming’s physiological function is mainly to descend the qi on the right, yet the lungs and gastrointestinal tract are the most important organs.In a narrow sense, the physiological function of Yangming in the human body is that Yangming controls the rise and fall of qi and blood, maintains the descent of qi, and controls the balance of dryness and dampness.Taking a general view on the connotation of Yangming and its physiological functions and pathological characteristics, we conclude that the basic pathological characteristics of Yangming mainly include deficiency of qi and blood, non-descending of qi, and disharmony of dryness and dampness. Furthermore, the article takes wei-syndromeand cough as examples to illustrate the importance of the function of "Yangming" in the treatment of corresponding clinical diseases.
摘要:Circadian rhythm is a common physiological phenomenon in nature with a cycle of nearly 24 hours. It is based on the rhythmic expression of core clock genes such as CLOCK、BMAL1、PER and CRY. A large number of studies have shown that circadian clock genes are closely related to sleep. The suprachiasmatic nucleus (SCN), as the central clock, activates the rhythm and forms a series of transcriptional translation feedback loops (TTFL) expressing clock genes to regulate the body's circadian oscillation, and the body can maintain normal life activities under the co-regulation of sleep homeostasis system and circadian clock system. Traditional Chinese medicine (TCM), however, believes that the circadian rhythm of the human body is mainly related to the ebb and flow of Yin and Yang and the movement of Ying (Nutrient) and Wei (Defense). Interlinked Yin-Yang and its endlessly circulation are the cardinal mechanism of sleep. Through analyzing the mechanism of circadian clock genes regulating circadian rhythm and the relationship of the sleep theory of Yin-Yang, Ying-Wei and circadian rhythms, we can find the interoperability between Yin-Yang changes. Ying-Wei circadian regulation and the feedback loop mechanism of clock genes. Further exploration can provide a new target for TCM treatment of sleep disorders based on biological clock as the underlying mechanism.
摘要:The occurrence and development of severe novel coronavirus pneumonia (COVID-19) follows the pathogenesis characteristics and transmission law of the pestilential diseases, "pathogenic warm attacks the lungs first, and goes to the pericardium reversely" , which belongs to the category of "wrongly-treated pattern" or "unfavorable pattern" . The disease progressed rapidly. At first, the epidemic virus invaded the lungs, then blocked the lungs, leading to the critical stage of inner blocking causing collapse. Modern medicine believes that severe disease involves a series of pathophysiological changes, such as viral damage, immune inflammatory response, endothelial cell damage, thrombosis, and renin-angiotensin-aldosterone system (RAAS) disorders. Regarding the treatment of severe COVID-19, the first proposal is to prevent the occurrence of severe disease, When necessary, it is not limited to wei (defense), qi, ying (nutrient) and blood levels. and prior treatment should be given before development of patterns; This article summarizes the role and value of the methods of detoxification and cooling blood, purging the lungs and opening the bowels, and strengthening spleen and eliminating dampness in the treatment of severe COVID-19. In combination with modern medical research, the possible mechanism of action of the above three treatments was also analyzed.

摘要:The four basic principles as the theoretical cornerstones of TCM constitution theory elaborate the formation, characteristics, and changes of the body constitutions, as well as the physiological and pathological characteristics of these constitutions. Allergic constitution is an important biased TCM constitution, which is an underlying condition for various allergic diseases. Symbiotic microbiome is of great significance to the establishment of immune homeostasis and the development and maturation of the immune system, and clearly related to the development of allergic diseases. The lung microbiome could regulate the occurrence and development of allergic asthma directly or indirectly, which is consistent with the basic principles of TCM constitution. The explanation from the perspective of TCM constitution theory provides a view to broadening the ideas of clinical diagnosis and treatment of allergic asthma.
摘要:Spleen failing to transport could result in abnormal distribution of water and food and disorder of body fluid metabolism, thus producing pathological phlegm and dampness accumulation. It is also an important pathogenesis of Chinese medicine constitution. Current understanding of its microscopic mechanism is still limited. The intestinal barrier isolates the internal and external environment of the intestinal tract in different forms, and has the function of preventing invasion of pathogenic agents. The damage of intestinal barrier function will cause inflammation and insulin resistance, which will lead to glucolipid metabolism disorders and phlegm-dampness constitution susceptible to multiple metabolic diseases. From this perspective, it can provide a new scientific explanation for the internal mechanism of spleen failing to transport. In addition, Chinese herbal medicine could improve the intestinal barrier function by regulating intestinal flora, reduce chronic inflammation and insulin resistance, thereby improving metabolic abnormalities, intestinal barrier function can serve as a new target for regulating phlegm-dampness constitution. This study aimed to reveal the microscopic mechanism of spleen’s failing to transport in phlegm-dampness constitution from the perspective of impaired intestinal barrier function. It could provide scientific basis for rectifying phlegm-dampness constitution which could be of profound significance for the clinical practice of Chinese medicine.

摘要:ObjectiveTo observe the effects of modified Erbu Zhuyu (Double-tonifying Fertility-promoting) Formula on pinopodes, microvilli, estrogen receptor α (ERα) and leukemia inhibitory factor (LIF) expression of endometrium in aged mice.MethodsPregnant mice aged 8~9 months were randomly divided into aged model group, low-, mid- and high-dose modified Erbu Zhuyu Formula groups, progynova group and aspirin group (n=8 in each group). Another 8 pregnant mice aged 6~8 weeks were selected as childbearing age blank group. The mice in childbearing age blank group and aged model group were perfused with 0.5% CMC solution every day, and the mice in the other groups were perfused with corresponding interventional medicines every day for 5 days. The expression of pinopodes and microvilli in endometrium was observed with scanning electron microscope. The protein localization of ERα and LIF in endometrium was evaluated with immunohistochemistry. The expression of ERα and LIF mRNA and protein in endometrium were also measured.ResultsScanning electron microscope showed that mice in the childbearing age blank group presented with smooth endometrial surface, short and dense microvilli, and high-expressed pinopodes. In aged model group, the endometrial surface appeared uneven and seriously damaged, the microvilli were sparse, and pinopodes was almost invisible. The expression of microvilli and pinopodes in each treatment group was better than that in the aged model group. Compared with the childbearing age blank group, the expression of ERα mRNA and protein in the endometrium of aged model group increased (P<0.05), and the expression of LIF mRNA and protein decreased (P<0.05). Compared with aged model group, the expression of ERα mRNA and protein in endometrium of each treatment group decreased (P<0.05), and the expression of LIF mRNA and protein in endometrium of each treatment group increased (P<0.05).ConclusionModified Erbu Zhuyu Formula can enrich the expression of microvilli and pinopodes, reduce the expression of ERα mRNA and protein, and increase the expression of LIF mRNA and protein, so as to improve endometrial receptivity.
摘要:ObjectiveThe oxidative stress model was established by adding H2O2 to the culture medium in vitro to explore the effect of Bushen Tiaojing Formula (Kidney-supplementing Menstruation-regulating formula, BSTJF) on the quality and antioxidant capacity of oocytes.MethodsEighty SPF female Kunming mice aged 3-4 weeks (25±3 g) were selected. Each mouse was injected with serum gonadotrophin (10IU). After 48 hours, the ovaries were isolated under a stereo microscope to obtain GV-phase oocytes. The cells were randomly divided into normal group, model group, high-dose BSTJF group, low-dose BSTJF group, and incubated in IVM for 14 hours. MⅡ stage oocytes were then collected. The first polar body (PB1) extrusion rate of the oocytes was observed under an inverted microscope while the ultrastructure of mitochondria observed under a transmission electron microscope. The distribution of mitochondria and the level of reactive oxygen species (ROS) were evaluated with the Mito-Tracker Red CMXRos and DCHF-DA kits. Chemistry Colorimetric method and RT-qPCR method for detection of superoxide dismutase (SOD) and glutathione peroxidase (GSH-px) activity and mRNA expression.ResultsCompared with the normal group, the PB1 excretion rate of oocytes in the model group was significantly reduced; the integrity of the mitochondrial structure was destroyed with abnormal distribution, and the level of ROS increased. The antioxidant enzymes SOD and GSH-px activity and mRNA expression were all decreased; Compared with the model group, the mitochondrial structure of the high- and low- dose groups of BSTJF was relatively complete, with only part of the mitochondrial cristae (MC) destroyed. Mitochondria were uniformly distributed, the level of ROS decreased, and the PB1 extrusion rate, antioxidant enzyme SOD, GSH-px activity and mRNA expression were all increased (P<0.05).ConclusionBushen Tiaojing Formula can improve the quality of oocytes, and its mechanism may be related to protecting the structure and function of mitochondria, regulating the activity of antioxidant enzymes, and reducing oxidative damage.
摘要:ObjectiveTo investigate the effect of oligosaccharide esters (OE) of polygala tenuifolion (Yuanzhi) on β-Amyloid protein (Aβ25~35) induced SH-SY5Y cell injury and AKT/CREB/BDNF signal pathway.MethodsThe cultured SH-SY5Y cells were divided into control group, Aβ25~35 injury model group, OE (50, 100, 200 and 400 mg/L) groups, and Aβ25~35 injury + OE (50, 100, 200 and 400 mg/L) group. Cells in the control group and OE group were incubated with equal volume of solvent and different concentrations of OE 24 hours, respectively. Cells in the Aβ25~35 injury model group and Aβ25~35 injury + OE group were incubated with 25 μmol/L Aβ25~35 for 24 hours after the addition of equal volume of solvent and different concentrations of OE for 2 h. The survival rate of SH-SY5Y cells was detected by MTT assay; Hoechst 33258 staining was used to observe apoptosis; The expressions of pAkt/Akt, pCREB/CREB and BDNF proteins in SH-SY5Y cells were detected by Western blot.ResultsCompared with the control group, OE (100, 200 and 400 mg/L) significantly increased the survival rate of SH-SY5Y cells; Aβ25~35 significantly decreased the survival rate of SH-SY5Y cells, increased the fluorescence intensity of Hoechst 33258 staining, and decreased the protein expression of pAkt/Akt, pCREB/CREB and BDNF in cells (P<0.01). Compared with the model group, OE (100, 200 and 400 mg/L) treatment significantly enhanced the cell survival rate, decreased the fluorescence intensity of Hoechst 33258 staining, reduced apoptosis, and increased the expression of BDNF, pAkt/Akt, pCREB/CREB in cells (P<0.05, P<0.01).ConclusionOE has protective effects against β-Amyloid protein induced SH-SY5Y cell injury, which is related to activating Akt/CREB phosphorylation and promoting the expression of BDNF.

摘要:Prestigious experienced traditional Chinese medicine practitioners have rich experience in the treatment of hematological disorders. In order to better summarize and inherit their academic thoughts, the Chinese Society of Traditional Chinese Medicine has summarized the academic thoughts of nine professors including Zhou Aixiang, which have been welcomed by TCM practitioners. Therefore, this is one of the series to summarize the academic thoughts of six professors Li Yinglin, Jiao Zhonghua, Fu Rulin, Tang Youjun, Yang Wenhua and Zhou Yuhong. This paper intends to help practitioners learn and master the clinical experience of famous traditional Chinese medicine and treat patients even better. Among them there are Professor Li Yinglin's unique treatment principles for aplastic anemia, hemorrhagic diseases and iron-deficiency anemia; Professor Jiao Zhonghua's academic thoughts on strengthening the spleen and kidney, preventing disease recurrence and extramedullary infiltration, and treating blood diseases with the combination of disease differentiation and pattern identification; Professor Fu Rulin’s clinical experience in diagnosis and treatment of blood diseases focusing on spleen and kidney, nourishing liver and kidney, strengthening Yin fluid, promoting blood circulation and removing blood stasis, clearing heat and detoxification; Professor Tang Youjun’s dialectical thought of pulse-taking and treating leukemia and blood stasis blocking bone marrow; Professor Yang Wenhua’s original new therapy for leukemia. Professor Zhou Yuhong has unique academic views on the treatment of hematological diseases with integrated traditional Chinese and Western medicine, focusing on the treatment of "phlegm and blood stasis" and body constitution differentiation. By sorting out and summarizing the clinical experience and academic ideas of famous traditional Chinese medicine, it is expected to be used for reference to traditional Chinese medicine, doctors of integrated traditional Chinese and Western medicine and colleagues who are committed to traditional Chinese medicine.

摘要:Diabetic foot is one of the main causes of disability and death in diabetic patients. The Wagner and TEXAS classifications focus on the assessment of the severity of the diabetic foot and cannot be used to guide wound treatment. Based on this, we propose STAGE principles to guide the treatment of diabetic foot ulcers. This principle emphasizes that the incision and drainage are based on the anatomy of the foot and blood supply to control the infection. In the process of incision and drainage and treatment of diabetic foot wounds at different stages, the corresponding principles, methods, prescriptions, medicines and techniques with Traditional Chinese Medicine characteristics are proposed, and the objective and subjective indicators for the evaluation of clinical efficacy in each treatment stage are summarized, focusing on the whole body. Symptoms and changes in local wound and peri-wound symptoms. During the incision and drainage stage, attention should be paid to the subsidence of local redness and swelling. Favorable signs are relief of body temperature, local swelling and pain, which indicates that the infection is under control. Otherwise increased heat, swelling and pain indicate that the incision and drainage layer is not in place or there is an unincised abscess cavity. In the hierarchical treatment stage, the blood supply of the wound needs to be assessed in a timely manner, and the treatment of bone (joint) and tendon is related to healing and functional recovery, and needs to be cleverly designed. Granulation tissue covering the wound is the key to epithelialization, and the quality of granulation tissue needs to be assessed frequently. Through the integration of STAGE principle and external treatment of Traditional Chinese Medicine, a systematic solution for wound repair of diabetic foot is proposed.
