      Special Theme: Prevention and treatment of vascular cognitive impairment with traditional Chinese Medicine

      摘要:According to the theory of traditional Chinese medicine(TCM), cognition is derived from the brain marrow, and normal functioning of the brain marrow depends mainly on nourishment of the essence in the kidney and the integrity and smoothness of brain collaterals.When there is a deficiency of essence in the kidney, the essence that nourishes the brain marrow decreases, leading to dizziness, blurred vision, tinnitus, forgetfulness, and other precursory symptoms of vascular dementia.When there is a deficiency of essence in the kidney for a long time, it is not possible to promote the movement of qi, blood, and fluid, resulting in phlegm, static blood, and other pathological products blocking and damaging the brain collaterals.At this time, the essence that nourishes the brain marrow is not transferred to the brain because of the collateral damage, and the brain marrow is disturbed by phlegm and static blood and loses its clear, light, and pure nature.When the brain marrow loses its clarity, the initial symptoms of vascular dementia such as dullness, forgetfulness, and slow thinking will appear.Based on this, we propose that "kidney deficiency, collateral damage, and brain marrow injury" comprise the core pathogenesis of vascular dementia, and we emphasize that intervention at the time of superficial marrow injury can delay the progression of vascular dementia and that early treatment should be based on tonifying the kidney, supplementing the essence, resolving phlegm, and dispelling stasis.The aim of this paper is to provide a theoretical basis based on TCM to prevent and treat vascular dementia and slow the progression of vascular dementia.

      摘要:ObjectiveWe aimed to study the mechanisms underlying the positive effects of Bushen Qizhi formula on cognitive function in ischemic stroke rats by the Keap1/Nrf2/ARE signaling pathway.MethodsIschemic stroke was induced in male SD rats by thread occlusion.The rats were randomly divided into the sham group, the model group, the Bushen Qizhi formula low-dose(BSQZ-L) group(3.6 g/kg), and the Bushen Qizhi formula high-dose(BSQZ-H) group (14.4 g/kg), nine rats in each group. Treatment lasted for 14 days.The Garcia JH method was used to evaluate the degree of neurological deficit.The Morris water maze test was used to evaluate spatial learning and memory.Western blotting was used to detect the protein expression levels of Keap1, Nrf2, and HO-1 in the ischemic hippocampus.The TBA test, the WST-1 assay, and the microplate method were used to determine MDA levels, SOD activity, and GSH levels in the ischemic hippocampus.ELISA was used to detect TNF-α, IL-18, and CXCL10 levels in the ischemic hippocampus.ResultsIn the model group, the neurological score was lower(P<0.01), the time of escape latency was higher(P<0.01), the time in the target quadrant and the time spent crossing the platform were both lower(P<0.05, P<0.01), and the protein expression levels of Keap1, Nrf2, and HO-1, the MDA content, and TNF-α, IL-18, and CXCL10 levels in the hippocampus were all higher(P<0.05, P<0.01), and SOD activity and GSH levels were both lower (P<0.01) compared with the sham group.Compared with the model group, the neurological scores were higher in the BSQZ-H and BSQZ-L groups(P<0.01), the escape latency was reduced in the BSQZ-H group(P<0.01), the time in the target quadrant and the time spent crossing the platform were higher in the BSQZ-H group(P<0.05, P<0.01), the time in the target quadrant was higher in the BSQZ-L group(P<0.05), the protein expression level of Keap1, MDA content, and the TNF-α, IL-18, and CXCL10 levels in the hippocampus were lower in the BSQZ-L and BSQZ-H groups (P<0.05, P<0.01), the protein expression levels of Nrf2, GSH levels, and SOD activity were all higher in the BSQZ-L and BSQZ-H groups(P<0.05, P<0.01), and the protein expression level of HO-1 was higher in the BSQZ-H group(P<0.05). Compared with the BSQZ-L group, the neurological score was higher(P<0.01), the protein expression level of Keap1, MDA content, and TNF-α and IL-18 levels in the hippocampus were all lower(P<0.05, P<0.01), and GSH level and SOD activity were both higher (P<0.05) in the BSQZ-H group.ConclusionThe mechanisms underlying the positive effects of Bushen Qizhi formula on neurological deficit and cognitive impairment in rats after ischemic stroke may be related to the activation of the Keap1/Nrf2/ARE signaling pathway, the reduction of hippocampal oxidative stress, and the inhibition of the inflammatory response.

      摘要:ObjectiveWe aimed to observe the efficacy of Dihuang Yizhi formula in post-stroke cognitive impairment non-dementia(PSCIND) in kidney essence deficiency patients.MethodsA total of 116 patients with PSCIND admitted to Longhua hospital between July 2019 and January 2021 were randomized into the experimental group(n=58) and the control group(n=58). In the experimental group, Dihuang Yizhi formula was used; in the control group, piracetam tablets were used for a course of 6 months.All patients were assessed with the Mini-Mental State Examination(MMSE), the Montreal Cognitive Assessment(MoCA) scale, the Zarit Burden Inventory(ZBI), the Ability to perform Daily Living(ADL) scale, and the traditional Chinese medicine(TCM) syndrome score before, at 3 months, and at 6 months of treatment.ResultsDuring the treatment, five cases fell off in the experimental group and four cases in the control group. Finally, 53 cases in the experimental group and 54 cases in the control group completed this study. Compared with before treatment, patients in both groups had elevated MMSE and MoCA scores after 6 months of treatment(P<0.05), with the increase in MMSE and MoCA scores being correlated with time(P<0.01). The ZBI scores of the patients in the experimental group decreased after 3 months of treatment(P<0.01), and after 6 months of treatment, only the ZBI scores of the patients in the experimental group decreased(P<0.01). The decrease in ZBI was associated with time and group × time interactions(P<0.01). In the MoCA sub-items, after 6 months of treatment, increases in scores of naming(P<0.05), memory and calculation(P<0.01), and language and abstraction(P<0.01) were related to time, whereas increases in scores of delayed recall were related to group × time interactions(P<0.01). After 6 months of treatment, ADL scores decreased in the experimental group(P<0.01), and the decrease in ADL scores was associated with time(P<0.01) and group × time interactions(P<0.05). After 3 months of treatment, the TCM syndrome scores of patients in the experimental group decreased(P<0.01), while after 6 months of treatment, the TCM syndrome scores of patients in both groups decreased(P<0.01). The decrease in TCM syndrome scores was correlated with time and group × time interactions(P<0.01). Safety indicators were normal in all patients before and after treatment.ConclusionBased on the theory of "tonifying the kidney and supplementing the essence, " Dihuang Yizhi formula can improve the cognitive function of PSCIND patients, improve their quality of life, and reduce the burden on their families, exhibiting certain advantages over piracetam.

      摘要:ObjectiveWe aimed to evaluate the clinical efficacy and safety of Jianpi Yishen Huazhuo formula in patients with mild and moderate vascular dementia(VaD) with syndrome of spleen and kidney deficiency and mutual obstruction of phlegm and blood stasis.MethodsA total of 90 patients with mild and moderate VaD with syndrome of spleen and kidney deficiency and mutual obstruction of phlegm and blood stasis treated at Dongfang Hospital of Beijing University of Chinese Medicine, Shanxi Traditional Chinese Medical Hospital, and The Affiliated Hospital to Changchun University of Chinese Medicine between December 2017 and February 2021 were enrolled.The patients were randomly divided into the control group (n=45) and the experimental group(n=45). Patients in the control group were given conventional basic treatment, and patients in the experimental group were additionally given Jianpi Yishen Huazhuo formula.Patients in both groups were treated for 180 days. The changes of Alzheimer’s Disease Assessment Scale-Cognitive section (ADAS-cog) scores and traditional Chinese medicine symptoms were compared between the two groups before and after treatment.ResultsDuring the treatment, four cases fell off in the experimental group and five cases in the control group. Finally, 41 cases in the experimental group and 40 cases in the control group completed this study. The ADAS-cog scores of the experimental group were decreased after treatment compared with those before treatment(P<0.01). The difference in ADAS-cog scores between before and after treatment and the difference in memory and language scores between before and after treatment were larger in the experimental group compared with the control group(P<0.01). The number of patients with depression, misspeaking, and poor speech was lower in the experimental group after treatment compared with that before treatment(P<0.05, P<0.01). The number of patients with poor speech was lower in the experimental group after treatment compared with the control group(P<0.05). The scores of nine peripheral symptoms, i. e., headache, dizziness, fatigue, palpitations, soreness of waist and knee, tiredness and weight of limbs, chilly sensation and cold limbs, frequent nocturnal urination, and residual leach after urination, were lower in the experimental group after treatment compared with those before treatment(P<0.05, P<0.01). The scores of four peripheral symptoms, i. e., sallow complexion, sighing often, chilly sensation and cold limbs, and frequent nocturnal urination, were lower in the experimental group after treatment compared with the control group(P<0.05). Total peripheral symptom scores were lower in the experimental group after treatment compared with those before treatment(P<0.01). The total scores of the experimental group were lower after treatment compared with the control group(P<0.05).ConclusionJianpi Yishen Huazhuo formula can improve cognitive function and related clinical symptoms in patients with mild and moderate VaD with syndrome of spleen and kidney deficiency and mutual obstruction of phlegm and blood stasis.It is safe and has certain advantages over conventional basic treatment.

      摘要:The inheritance of illustrious senior traditional Chinese medicine practitioners is a key issue to promote the inheritance and development of traditional Chinese medicine.This study summarizes and defines the holistic elements of illustrious senior traditional Chinese medicine practitioners and proposes that the "Dao-Shu combination" is the essence of the inheritance of illustrious senior traditional Chinese medicine practitioners.We point out that "Dao" not only refers to the path of medicine but also includes the way of being as a person, being a teacher and learning.We discuss inheritance, including inheritance content and inheritance method, and how to integrate high-quality master-apprentice education into college education.Furthermore, we take the research findings of Dao-Shu by Professor Kong Guangyi, obtained by the team’s grounded theoretical work, as an example to illustrate the composition and relationship of the elements of the whole person.Finally, we point out that the inheritance should be the whole person inheritance of Dao-Shu, and that the evaluation of inheritance should consider both aspects of Dao-Shu, rather than being limited to inheritance or medical technology alone. Long-term effects and a dynamic inheritance evaluation system should be established in future studies.

      摘要:The academic exchange and ideological construction of various schools of thought in the pre-Qin period created a cultural atmosphere that promoted the development of the theoretical system of traditional Chinese medicine(TCM). The "law strategy and power-base" based on the works of the Legalists was the direct embodiment of the thought of checks and balances at that time.Through transmutation and convergence, it is found that the ideology of the "law strategy and power-base" is related to syndrome differentiation and tumor treatment in TCM.In the concept of the "law strategy and power-base" , Fa is the law, which includes the meaning of legislation and law enforcement, which is similar to the method of TCM differentiation and treatment with tumor drugs; Shu is the embodiment of the thinking process, the guidance and supplement to the law, and is connected with the thinking process of TCM syndrome differentiation and tumor treatment; Shi is the development process, which is consistent with the advance and retreat trend of TCM syndrome differentiation and tumor treatment.The application of "law strategy and power-base" thinking is the core embodiment of induction, deduction, and logical reasoning in tumor clinical differentiation and treatment.Starting from the "law strategy and power-base" of tumor treatment and relying on the core theoretical system constructed by Legalists, this paper discusses its effects on syndrome treatment strategies, in order to provide new research ideas and perspectives on syndrome differentiation and the treatment of malignant tumors based on the "law strategy and power-base" .

      摘要:Xuanfu was first mentioned in the Huangdi Neijing.Later physicians also published many discussions of Xuanfu, but their opinions were conflicting.Among these physicians, Liu Wansu provided the most detailed discussion about this theory.He claimed that "Everything has Xuanfu, " which expanded the scope of Xuanfu.Opening and closing in order is the physiological normal state of Xuanfu, and closed stagnation is the pathological state of Xuanfu.Liu Wansu proposed that yang-heat depression is the basic pathogenesis of Xuanfu disease, and Xuanfu closure is a complex pathological result of yang-heat generating inner stagnation.Liu Wansu believed that the basic method of opening Xuanfu is to use clearing, diffusion, and purgation together, and he created Fangfeng Tongsheng San.The three methods of clearing, diffusion, and purgation are used together to open the Xuanfu, that is, using cold medicine to clear away heat, and dispelling stagnation with pungent and bitter cold medicines; clearing heat is the main method, and diffusion and purgation serve as auxiliary methods.Combined with the medical records of reputable doctors in various specializations including internal medicine, surgery, gynecology, pediatrics, and otorhinolaryngology, this paper roughly analyzes the basic concept of using Fangfeng Tongsheng San’s three method of clearing, diffusion, and purgation and discusses their applications in various clinical departments, aiming to provide a reference and inspiration for the clinical application of Fangfeng Tongsheng San.

      摘要:Cancer is a kind of "deficiency pathogen" that represents the combination of qi deficiency and arrogated evil.The deficiency pathogen is the driving force of carcinogenesis that summarizes the "ebbs and flow" and "deficiency or excess" between healthy qi and cancerous toxin. T lymphocytes are tumor-specific killer cells.Therefore, tumor antigen recognition by T lymphocytes lies at the core of cancer immunotherapy.The tumor cells hide by lowering their antigenicity, hampering T cell function.Abnormal angiogenesis and stromal reprogramming in the tumor microenvironment are the basis of blood stasis, phlegm coagulation, and cancer growth.The reprogrammed microenvironment induces T cell exclusion by excluding effector T cells from the vicinity of cancer cells.Furthermore, continuous tumor antigen stimulation results in a cancer-host "stalemate." The loss of effector T cell function can be defined as T cell exhaustion.Healthy qi deficiency and cancerous toxins reinforce each other to form the "deficiency pathogen, " which is difficult to eliminate.Therefore, tumor treatment should focus on the root of the disease, and clinical treatment should be guided by a combination of T cell phenotype and syndrome differentiation.This may serve as an important basis for precise tumor treatment with traditional Chinese medicine.

      摘要:ObjectiveWe aimed to explore the effects of Coptidis Rhizoma-Rheum Officinale on hepatic insulin resistance in type 2 diabetes mellitus(T2DM).MethodsWistar rats with T2DM were randomly divided into the following five groups: the model group, the metformin group(0.10 g/kg), the Coptidis Rhizoma-Rheum Officinale low-dose group(1.18 g/kg), the Coptidis Rhizoma-Rheum Officinale mid-dose group(2.36 g/kg), and the Coptidis Rhizoma-Rheum Officinale high-dose group(4.72 g/kg), with six rats in each group.Moreover, a normal group was included(n=6). The intervention lasted for 8 weeks.Fasting blood glucose(FBG) was detected in rats by a biochemical method, and ELISA was used to detect fasting serum insulin(FINS) and liver IL-1β, IL-18, and IL-10 levels.The insulin resistance index(HOMA-IR) was calculated.HE staining was used to observe the pathological changes of rat livers.Oil red O staining was used to observe ectopic fat deposition in the liver.The protein expression levels of NLR family pyrin-domain containing protein 3(NLRP3), apoptosis-associated speck-like protein containing a CARD(ASC), cysteinyl aspartate-specific proteinase 1(Caspase 1), insulin receptor substrate 2(IRS2), phosphorylated IRS2(p-IRS2Ser731), protein kinase B(Akt), and p-Akt in liver tissues were detected by Western blotting.ResultsFBG and HOMA-IR levels were lower in the metformin group and the mid-dose and high-dose groups(P<0.05, P<0.01) and the FINS level was higher in the metformin group and the mid-dose group(P<0.05, P<0.01) compared with the model group.The FBG level was higher(P<0.01) and the FINS level was lower(P<0.05) in the low-dose group compared with the metformin group.HE staining showed that hepatic cords were arranged relatively neatly, and few inflammatory cells were observed in the metformin group. In the low-dose group, the hepatic cord arrangement was not regular, and inflammatory cell infiltration and hepatic cell vacuolation were observed.The hepatic cords of the rats in the mid-dose group were arranged neatly, and infiltration of focal inflammatory cells was observed.The hepatic cords of rats in the high-dose group were arranged relatively neatly, and few infiltrated inflammatory cells and vacuolated hepatic cells were observed.Oil red O staining showed that the average density was lower in the metformin and the mid-dose and high-dose groups compared with the model group(P<0.05, P<0.01). The levels of IL-1β and IL-18 were lower in the metformin and the mid-dose and high-dose groups compared with the model group(P<0.05, P<0.01). The protein expression levels of NLRP3, ASC and Caspase 1 in the liver were lower in the metformin group(P<0.05), and the protein expression levels of NLRP3 and Caspase 1 were lower in the mid-dose and high-dose groups(P<0.05) compared with the model group.The ASC protein expression level was higher in the low-dose group compared with the metformin group(P<0.05). The p-IRS2Ser731/IRS2 ratio was lower(P<0.05) and the p-Akt/Akt ratio was higher(P<0.05) in the metformin and the mid-dose and high-dose groups compared with the model group.ConclusionCoptidis Rhizoma-Rheum Officinale can effectively improve hepatic insulin resistance, inhibit the NLRP3 inflammatory vesicle activation-mediated inflammatory response, reduce ectopic fat deposition, and thus improve insulin signaling in T2DM rats.

      摘要:ObjectiveWe aimed to observe the effects of Buyang Huanwu Decoction(BYHWD) on optic tract injury of cerebral ischemia rats and to explore the effective components, core targets, and regulatory mechanism of BYHWD on cerebral ischemia rats’ optic tract lesions.MethodsA suture occlusion method was adopted to construct a rat model of cerebral ischemia induced by right middle cerebral artery embolization.In total, 24 male SD rats were randomly divided into the sham group, the model group, and the BYHWD group, eight rats in each group. The rats in BYHWD group were intragastrically injected with BYHWD for 30 days(16.1 g/kg). Optic tract damage was detected by T2 mapping, and changes in the optical tract microstructure were analyzed by diffusion tensor imaging(DTI) and diffusion tensor tractography(DTT). The active components and targets of BYHWD were collected from the TCMSP, PubMed, and CNKI databases, and the targets of ischemic optic tract injury were screened from the GeneCards, OMIM, DisGeNET, and DrugBank databases.Cytoscape was used to construct an active component-therapeutic target network.A protein-protein interaction(PPI) network was constructed, and protein cluster analysis was performed with the STRING platform.KEGG pathway analysis of core protein clusters was performed based on the Metascape database.ResultsT2 mapping showed that the rT2 value in the optic tract region was higher in the model group compared with the sham group(P<0.01). The rT2 value in the optic tract region of the BYHWD group was lower compared with the model group(P<0.01). The DTI result showed that the rFA value in the optic tract region of both the model group and the BYHWD group were lower(P<0.01) and the rAD and rRD were higher compared with the sham group(P<0.01). The rFA value was higher(P<0.01) and rAD and rRD were lower (P<0.01) in the BYHWD group compared with the model group.DTT analysis showed that the relative average length and density of nerve fibers in the optic tract region of the model group and BYHWD group were lower(P<0.05, P<0.01) compared with the sham group, while the relative average length and density of nerve fibers in the optic tract region of the BYHWD group were higher(P<0.05, P<0.01) compared with the model group.In total, 56 active components and 400 potential effective targets related to ischemic optic tract injury were screened from BYHWD.The main active components were Astragaloside Ⅳ, paeoniflorin, and ferulic acid.PPI core genes included AKT1, TNF, TP53, CASP3, and JUN.Nine related signaling pathways were obtained by KEGG pathway enrichment analysis of PPI core protein clusters.They regulate a variety of biological processes and molecular functions in neurons, astrocytes, microglia, and oligodendrocytes, mainly involving the PI3K-Akt signaling pathway, Toll-like receptor signaling pathway the apoptosis pathway, energy metabolism, and inflammatory factor-related pathways.ConclusionBYHWD can alleviate optic tract nerve injury and promote remodeling of the optic tract ultrastructure after cerebral ischemia in rats.Its effects on ischemic optic tract injury may be mediated by multiple signaling pathways, including the PI3K-Akt signaling pathway, the MAPK signaling pathway, and the TNF signaling pathway, affecting neuronal and glial cell functions.

      摘要:ObjectiveWe aimed to explore the properties of Ashitaba to lay a theoretical foundation for its clinical and industrial application in China.MethodsIn this study, the CNKI, Wanfang, VIP, CiNii, Web of Science, PubMed, and Scopus databases were selected for the retrieval of relevant studies.The medicinal properties of Ashitaba were analyzed based on the theory of traditional Chinese medicine.ResultsAshitaba (Angelica keiskei Koidzmi) belongs to the Umbelliferae.The stems and leaves of Angelica L. are used in traditional Chinese medicine.It can be used both dry and fresh. Ashitaba is mainly distributed in Japan and South Korea, and also introduced in Shandong and other places in China. Ashitaba leaves are sweet, salty, and slightly bitter in flavor and cool in property and pass through the spleen, kidney, and liver channels.It can remove turbidity and lower blood lipids, tonify the liver and kidney, and alleviate a hangover. Ashitaba is used for treating obesity, hyperlipidemia, and diabetes and in people with decreased bone mineral density, low immunity, and excessive drinking habits.It can be taken orally as a water decoction(3~10 g of dry medicine, 15~50 g of fresh herbs). It can also be applied at appropriate amounts to injuries for external use.ConclusionWe report the medicinal properties of Ashitaba in order to lay a theoretical foundation for further experiments and clinical application, which is of great significance to the globalization of traditional Chinese medicine resources.

      摘要:Zhu Danxi, a Yuan Dynasty physician, founded the school of nourishing yin by combining the strengths of various schools and his own clinical experience.Zhu Danxi advocated the therapeutic role of nourishing yin and making the fire descend in his clinical practice, which had a profound influence on later generations.This article discusses the core elements of Danxi’s academic theory, the "theory of yang being sufficient but yin being deficient" and the "theory of ministerial fire" , in the etiology and pathogenesis of women’s syndromes occurring around menopause and their application in clinical treatment.The occurrence of syndromes around menopause is based on the deficiency of essence and blood in the woman’s body, and the external manifestations of the disease are delusional movement of ministerial fire, which does not keep its position, and loss of the monarch’s wisdom, which result in disorder of the monarch and minister.In terms of disease treatment, the principle is to replenish essence and blood, nourish yin, and make the fire descend, so that the discrete ministerial fire can return to its place, and patients can be treated with the Dabuyin pill, the dosage of which can be adjusted according to the degree of delusional movement of the ministerial fire in the internal organs.Moreover, the heart and kidney communicate, so that the water and fire, and the yin and yang of the whole body are in harmony; then, the monarch is holy, the ministers are virtuous, and the internal organs are in coordination.From the perspective of health and longevity, Danxi emphasizes the cultivation of the mind and restraint of delusional thoughts, so that the ministerial fire does not move; the diet should also be light, including much grain, beans, vegetables, and fruits, which are less thick and nourishing, for women with syndromes occurring around menopause.Danxi’s theory has significance in the clinical treatment and healthcare for women with syndromes occurring around menopause and can serve as a reference for later generations of physicians.

      摘要:Recurrent hyperthyroidism is a special type of hyperthyroidism that is difficult to treat clinically.It is characterized by repeated recurrence after the cure of hyperthyroidism, causing serious physiological and psychological problems to patients.The theory of traditional Chinese medicine assumes that recurrent hyperthyroidism is closely related to emotional disorder.Sovereign fire and ministerial fire participate in the regulation of emotional activities.On the contrary, emotional disorder can lead to the imbalance between sovereign fire and ministerial fire.Zhu Danxi proposed that "disorderly heart is ministerial fire" . The heart is easily disturbed by external factors, and anxiety leads to hyperactivity, resulting in reckless action of the ministerial fire.We believe that this is the core pathogenesis of recurrent hyperthyroidism.In addition, the course of recurrent hyperthyroidism is long, which is based on yin deficiency in the liver and kidney, while the ministerial fire is nourished by the yin of the liver and kidney, and the deficiency of yin fluid makes the ministerial fire more likely to act rashly.In terms of treatment, firstly, we should start with regulating emotions using psychological therapy and traditional skill method.Secondly, we should clear the sovereign and ministerial fire in order to restore balance.Yin should be nourished from beginning to end.The specific treatment should be based on medicines which have soft and quiet properties, so as to inhibit yang hyperactivity. According to the different affected viscera, the medicines that belong to the meridians should be selected. According to the combination of nature and flavor, fire should be reduced and yin should be protected.Finally, we must supplement qi in order to strengthen health.

      摘要:ObjectiveWe aimed to observe the clinical effects of wax-mud moxibustion on qi deficiency and blood stasis syndrome of shoulder-hand syndrome after stroke.MethodsFrom January 2019 to December 2020, a total of 105 patients with post-stroke shoulder-hand syndrome who visited the outpatient department and ward of Henan Hospital of Traditional Chinese Medicine were selected and randomly divided into the control group(n=52) and the experimental group(n=53). Both groups were treated with conventional drug therapy and rehabilitation training, and patients in the experimental group were treated with wax-mud moxibustion therapy.The two groups were treated for 4 weeks.Clinical efficacy, the traditional Chinese medicine(TCM)syndrome score, the shoulder-hand syndrome score(SHSS), the visual analogue scale(VAS) score, the hand drainage volume difference, the Fugl-Meyer score, and the shoulder range of motion(L-ROM) were compared between the two groups.ResultsDuring the treatment, eight cases fell off in the experimental group and seven cases in the control group. Finally, 45 cases in the experimental group and 45 cases in the control group completed this study. The total effective rate was 66.67%(30/45) in the control group and 86.67%(39/45) in the experimental group, and the total effective rate of the experimental group was higher than that of the control group(P<0.05). Compared with pre-treatment data, the TCM syndrome score, SHSS, VAS score, and hand drainage volume difference were decreased(P<0.01) and the Fugl-Meyer score and L-ROM were increased(P<0.01) in the experimental group after treatment.In the control group, the VAS score and the drainage volume difference were decreased(P<0.05, P<0.01) and the Fugl-Meyer score was increased(P<0.05) after treatment.In the experimental group, the TCM syndrome score, SHSS, VAS score, and hand drainage volume difference were lower and the Fugl-Meyer score and L-ROM were higher after treatment compared with the control group(P<0.01).ConclusionWax-mud moxibustion can relieve joint pain, swelling and improve range of motion, athletic ability of patients with shoulder-hand syndrome after stroke by replenishing qi and activating blood, warming the channels, freeing the network vessels, removing dampness, and dredging channels, which has certain advantages over conventional treatment.

      摘要:ObjectiveWe aimed to investigate the anti-inflammatory mechanism of heat-reinforcing needling on rheumatoid arthritis(RA) with cold syndrome.MethodsThe RA with cold syndrome model was established by injecting ovalbumin dry powder and Freund’s complete adjuvant combined with cold freezing.Fifty rabbits(50% male and 50% female) were randomly divided into the normal group, the model group, the resatorvid(TAK-242) group, the heat-reinforcing needling group, and the lipopolysaccharide(LPS) combined with heat-reinforcing needling group, 10 rabbits in each group.Rabbits in the TAK-242 group were intraperitoneally injected with the TLR4 inhibitor TAK-242(0.75 mg/kg); rabbits in the heat-reinforcing needling group were treated with "Zusanli" (ST 36) with heat-reinforcing needling; and rabbits in the LPS combined with heat-reinforcing needling group were injected intraperitoneally with the TLR4 agonist LPS(0.25 mg/kg) on the basis of heat-reinforcing needling. Each group received continuous treatment for 14 days. The circumference and skin temperature of the knee joint of the rabbits were recorded; the histomorphology of the synovial membrane of the rabbit knee joints was observed by HE staining; the expression levels of TNF-α, IL-1β, and IL-6 in synovial tissues were detected by immunohistochemistry; the protein expression levels of TLR4, MyD88, and ERK1/2 in synovial tissues were detected by Western blotting; and the mRNA levels of mTOR and p70S6K were detected by real-time PCR.ResultsThe circumference of the knee joint was lower and the skin temperature of the knee joint was higher(P<0.01) in all intervention groups compared with the model group. The HE staining of the TAK-242 group and the heat-reinforcing needling group showed that the inner mucosa of the joint capsule was basically flat, the proliferation of pannus was reduced, and there was a small amount of inflammatory cell infiltration, while the LPS combined with the heat-reinforcing needling group showed that the inner mucosa of the joint capsule was rough, the pannus proliferated, and the inflammatory cell infiltration and aggregation were obvious. The expression of TNF-α, IL-1β, and IL-6(P<0.01), the protein levels of TLR4, MyD88, and ERK1/2, and the mRNA levels of mTOR and p70S6K were lower(P<0.01) in all intervention groups compared with the model group.The circumference of the knee joint was lower and the skin temperature of the knee joint was higher(P<0.01) in the heat-reinforcing needling group and the TAK-242 group compared with the LPS combined with heat-reinforcing needling group; the levels of TNF-α, IL-1β, and IL-6(P<0.01), the protein levels of TLR4, MyD88, and ERK1/2, and the mRNA levels of mTOR and p70S6K were lower(P<0.01) in the heat-reinforcing needling group and the TAK-242 group compared with the LPS combined with the heat-reinforcing needling group.ConclusionHeat-reinforcing needling can effectively reduce knee swelling and inflammatory infiltration in RA with cold syndrome model rabbits.The possible mechanism is related to the inhibition of the TLR4-MyD88-ERK1/2 pathway, antagonism of cell proliferation activity, and a decrease of TNF-α, IL-1β, and IL-6 secretion by heat-reinforcing needling.
