      摘要:With the pandemic of COVID-19 being under control, more and more COVID-19 patients have entered the rehabilitation stage. Traditional Chinese medicine (TCM) has always attached importance to the treatment of epidemic diseases in the rehabilitation stage so as to further promote the rehabilitation of patients for a better prognosis, and prevent the recurrence of the disease. It's focal to explore how to carry out accurate rehabilitation treatment for COVID-19 patients in the prevention and treatment of COVID-19 for quite some time to come. This paper systematically sorts out the reported clinical manifestations of the post-COVID-19 syndrome and the current guideline for the practices in COVID-19 rehabilitation. Under the background of the collaborative rehabilitation practices with TCM and western medicine, combined with the unique advantages of TCM in disease rehabilitation, this paper discusses the TCM understanding of epidemic diseases in the rehabilitation stage and its prevention and treatment ideas, and puts forward the collaborative rehabilitation strategy with TCM and western medicine for post-COVID-19 syndrome, so as to promote higher-quality rehabilitation of the patients.

      摘要:Under the guidance of the traditional pattern differentiation system of wei, qi, ying and blood, most TCM doctors believe that a critical condition appears when the pathogenic factor enters the blood level in the development of warm diseases, while ignore that it could also occur in a qi level pattern. Based on Professor Jiang Liangduo's new view that " severe warm diseases could occur at qi level" developed based on his experience in the prevention and treatment of Corona Virus Disease 2019 (COVID-19), combined with the theory of " differentiation and treatment based on one's body condition" , this paper elaborates on the theoretical basis, pathogenesis evolution, prevention and treatment method of severe warm diseases at qi level, and points out that the occurrence of severe warm diseases at qi level is closely related to the physiological dysfunction of lung qi. Thus, this paper summarizes the pathogenesis of severe COVID-19 cases at qi level as follows: dampness toxic epidemic pathogenic factors attack the lung, damaging the lung and making it unable to disperse and descend qi and body fluids as normal. The debilitated lung qi can neither promote body fluid circulation nor constrain fluids, which in turn leads to the expiry of lung qi. Correspondingly, the main principle of treatment is to diffuse the lung, replenish lung qi and constrain body fluids.

      摘要:Exogenous febrile disease refers to a group of disorders chiefly characterized by fever due to contraction of exogenous pathogenic factors, with different disease causes and clinical manifestations. It has been a key research scope in traditional Chinese medicine (TCM) since ancient times. It was initially widely discussed in as early as the pre-Qin period and the era of Huangdi Neijing (Huangdi's Internal Classic) and Nan Jing (The Yellow Emperor's Classic of Eighty-one Difficult Issues). Then, at the end of the Eastern Han Dynasty, Zhang Zhongjing established the Six-channel Pattern Differentiation and Treatment System. During the Wei, Jin, Sui, and Tang Dynasties, emphasis was placed on sorting out and enriching the contents of exogenous febrile disease. During the Song, Jin, and Yuan Dynasties, the theory of cold damage was developed, and many new academic viewpoints emerged. The theories of epidemic disease and warm disease were developed and perfected in the Ming and Qing Dynasties. In the Qing Dynasty, doctors attached importance to the combination of cold damage and warm disease in the discussion of exogenous febrile disease. In the modern and contemporary times, doctors advocated the treatment of exogenous febrile diseases with integrated traditional Chinese and western medicine. Currently, at a time when various new types of infectious and febrile diseases frequently occur, it is helpful to clarify the origin, development course and various theories of exogenous febrile diseases in TCM for clinical application and academic innovation.

      摘要:In Wen Yi Lun (Treatise on Widespread Warm Pestilence), Wu Youke has systematically expounded the pathogenesis, patterns, pattern differentiation and treatment, treatment taboos, and self-healing symptoms of yang qi stagnation pattern in warm epidemic diseases, providing important reference for the prevention and treatment of Corona Virus Disease 2019 (COVID-19). Specifically, Wu Youke has proposed the treatment taboos for yang qi stagnation pattern, including avoiding the usage of diaphoresis with warm pungent medicinals, abuse of cold or cool medicinals, and invigorating qi, etc. He has also listed the self-healing symptoms of yang qi stagnation pattern. Besides, he has listed the patterns of yang qi stagnation pattern in warm epidemic diseases, mainly including yang qi stagnation in mo yuan (membrane source), external and internal yang qi stagnation, yang qi stagnation in the stomach, yang qi stagnation due to yin deficiency, and yang qi stagnation in yin collaterals, etc. Correspondingly, the therapeutic method to unblock the stagnant yang in different stages of warm epidemic diseases include dredging, three eliminations, purgation, nourishing yin and dredging collaterals. To sum up, the Theory of Yang Qi Stagnation in Treatise on Widespread Warm Pestilence not only explains the characteristics of the pathogenesis of COVID-19 in various stages, but also provides TCM clinical ideas for its treatment. These ideas are mainly reflected in the principles of whole process treatment of COVID-19, differentiation of various stages, and paying attention to the geographical conditions, with constant emphasis on dispelling pathogenic factors and unblocking yang qi.

      摘要:Healthy qi plays an extremely important role in the occurrence, development and changes of epidemics. The battle between exogenous pathogens and healthy qi determines the occurrence and outcome of epidemics. As a tonic medicinal, Renshen (ginseng, Radix et Rhizoma Ginseng) has played an important role in the prevention and treatment of epidemic diseases. However, it needs to be carefully considered the timing of using tonic medicinals in treatment and how to maximize the merits of tonic medicinals while minimizing its demerits. In Wen Yi Lun (Treatise on Warm Pestilence), Wu Youke has made a detailed exposition on the method of tonification and the application of Renshen (ginseng, Radix et Rhizoma Ginseng) in the treatment of epidemics, indicating that the method of tonification should be used with caution rather than prohibited, and it should be used urgently but not excessively; besides, in the application of Renshen (ginseng, Radix et Rhizoma Ginseng), the following items are discussed: the differentiation of deficiency and excess patterns, the true or false nature of the pathogenesis, the suitable patterns for tonification, as well as the timing and taboos of using it, thus providing reference and inspiration for the application of tonifying method and the use of Renshen (ginseng, Radix et Rhizoma Ginseng) in the treatment of epidemic diseases in later generations.

      摘要:" The spleen storing yi (overall mental activities) and governing si (thoughts)" is one of the theoretical guiding ideologies in TCM treatment of emotional diseases. Although there is a wealth of clinical experience to support its feasibility, its connotation is still ambiguous with a lacking in objective indicators, resulting in difficulty in wide clinical application. In this paper, by comparing this theory with corresponding knowledge in modern emotional psychology, the commonality and individuality are compared. Based on that, the theory is further interpreted from the three levels of cognition, physiology and expression. Then combined with modern immune cytokine theory, the relationship between emotion and spleen is analyzed, and it's found that emotion is closely related to the spleen with bidirectional adjustment. By adopting such a thinking mode to combine the spleen and emotion, a better clinical efficacy may be achieved.

      摘要:San Pin classification method recorded in Shen Nong Ben Cao Jing (Shennong's Classic on Materia Medica) is the earliest existing classification method for TCM Materia Medica, and it seems to be the simplest classification method at the first sight. If we deeply analyze the classification in light of the characteristics of Chinese civilization, however, we can realize that this classification method is formed under strong influence of Chinese civilization, thus reflecting the unique way of thinking of the Chinese people. The San Pin classification method highlights people-oriented view of life, contains the preventive view of treating diseases, embodies the three-element concept based on holistic thinking, and demonstrates the strategic view of "a supreme doctor even has the wisdom to govern a country" . It also carries the profound historical origin and rich philosophical connotation for the realization of the goal of building a healthy China.

      摘要:ObjectiveTo investigate the effect of Yangxin Shengmai (Heart-nourishing Pulse-engendering) Granule (YXSM Granule) on rats with coronary heart disease complicated with anxiety and depression through BDNF/TrκB pathway.Methods124 SD rats were randomly divided into the blank group (n=14) and the model group (n=110). Except for the blank group, the depression model was established with Chronic Unpredictable Mild Stress (CUMS) for 28 days. On the 15th day, the myocardial infarction reperfusion model was established by ligating the left anterior descending coronary artery. Among the rat models, 14 rats were operated but not ligated, and they were set as the sham operation group; the others were randomly divided into model group and low-, medium- and high-dose (2.1, 4.2, 8.4 g/kg) YXSM Granule groups and 0.29 g/kg Compound Danshen tablet + Fluoxetine (CDST+ Flu) group. After intragastric administration for 2 weeks, the rats in each group undergone behavioral tests. The myocardial injury, left ventricle diameter (LVD) and left ventricular wall thickness (LVWT) were observed by HE staining, the protein expression levels of BDNF, TrκB and CD34 in myocardial tissue were detected by immunohistochemistry, and the protein expression levels of BDNF, TrκB and p-TrκB in hippocampus were detected by Western blotting.ResultsCompared with the sham group, the frequency of entering the central area of the open field and the total time spent and total distance travelled in the movement track in the model group were significantly reduced (P<0.01), and the frequency and proportion of time spent in the open arm of the elevated cross maze were significantly reduced. Compared with the model group, the frequency of entering the central area of the open field and the total time spent and distance travelled in the movement track in all YXSM Granule groups and CDST+ Flu group were significantly increased (P<0.01), and the frequency and proportion of time spent in the open arm of the elevated cross maze were significantly increased, but with no significant difference. Compared with the sham operation group, in the model group, the degree of myocardial injury and LVD were significantly increased (P<0.01), the LVWT and the protein expression levels of BDNF, TrκB and CD34 in myocardial tissue and BDNF and p-trkb/TrκB in hippocampus decreased significantly (P<0.05, P<0.01). Compared with the model group, the degree of myocardial injury was significantly improved, the LVD was significantly reduced (P<0.01), the LVWT and the protein expression levels of BDNF, TrκB and CD34 in myocardial tissue and BDNF and p-TrκB/TrκB in hippocampus were significantly increased (P<0.05, P<0.01) in all the other groups.ConclusionYXSM Granule seems to improve the cardiac function and alleviate depressive symptoms in rats with coronary heart disease with anxiety and depression, which may play a cardioprotective and antidepressant role by activating BDNF/TrκB signal pathway.

      摘要:ObjectiveTo observe the effect of Bushen Tiaojing (Kidney-tonifying Menstruation-regulating) Formula (BSTJF) on quality and apoptosis of blastocyst in mice induced by repeated controlled ovarian stimulation (COS).Methods160 female ICR mice were randomly divided into normal group, model group, low-dose BSTJF group and high-dose BSTJF group (25.6, 51.2 g/mL). Except for the normal group, in the other three groups repeated COS model was established (COS three times). The mice in the normal group and model group were gavaged with distilled water, and the mice in the low-dose and high-dose BSTJF groups were gavaged with low- and high-dose BSTJF at 9 a. m. every day for 10 consecutive days. The model group, low- and high-dose BSTJF groups were intraperitoneally injected with pregnant mare serum gonadotropin (PMSG) 10 IU at 4 p. m. on the 11th day, and human chorionic gonadotropin (hCG) 10 IU 48 hours later. The normal group was intraperitoneally injected with saline 0.1 mL at 4 p. m. on the 11th day. There were 3 consecutive cycles with an interval of 4 days between every two cycles. Mice in all groups were caged with male mice of the same strain immediately after the third intraperitoneal injection of hCG (for the model group, low- and high-dose BSTJF groups) or normal saline (for the normal group). It was recorded as pregnancy if a vaginal plug was identified or sperm was observed on the vaginal smear in the following morning. Pregnant mice were killed 96 hours after the last intraperitoneal injection to obtain blastocysts. The quality of blastocysts was observed and number of blastocysts were counted under inverted microscope, and the rate of high quality blastocysts was calculated. The apoptosis rate of blastocysts was detected by TUNEL, and the levels of mRNA expression of Bcl-2 and Bax were detected by real-time fluorescence quantitative PCR. The levels of expression of apoptosis-related factors (Bcl-2, Bax and cleaved caspase-3) were detected with Simple Western, and the expression of cytochrome c was detected with immunofluorescence.ResultsCompared with the normal group, in the model group the number of blastocysts obtained was increased(P<0.05), while the rate of high-quality blastocysts was decreased(P<0.05); the total number of cells in blastocysts decreased, and the apoptosis rate increased(P<0.05); the RNA and protein ratio of Bcl-2 and Bcl-2/Baxm decreased(P<0.05), while the mRNA and protein expression levels of Bax increased(P<0.05), and the protein expression level of cleaved caspase-3 increased(P<0.05); cytochrome C was released from mitochondria and co-localization coefficients M1 and M2 decreased(P<0.05). Compared with the model group, there was no significant difference in the number of blastocysts obtained in the low- and high-dose BSTJF groups(P>0.05), but the rate of high-quality blastocysts increased significantly(P<0.05); the total number of cells in blastocysts increased and the apoptosis rate decreased(P<0.05); the mRNA and protein ratio of Bcl2 and Bcl-2/Bax increased(P<0.05), while the mRNA protein expression level of Bax decreased(P<0.05), and protein expression level of cleaved caspase-3 decreased(P<0.05); there was no significant release of cytochrome C, but the co-localization coefficients M1 and M2 increased(P<0.05).ConclusionBushen Tiaojing Formula has the potential to inhibit blastocyst cell apoptosis induced by repeated COS and improve the quality of early embryos, and its mechanism may be related to mitochondrial apoptosis pathway.

      摘要:ObjectiveTo explore the active components, targets and possible mechanism of Yishen Tongluo (Kidney-boosting Collateral-unblocking) Formula (YSTLF) in the treatment of diabetic kidney disease (DKD) based on network pharmacology.MethodsThe effective components and related targets of YXTLF were screened from the Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform (TCMSP), The Encyclopedia of Traditional Chinese Medicine (ETCM) and A Bioinformatics Analysis Tool for Molecular mechanism of Tradition Chinese Medicine (BATMAN-TCM), and further supplemented with literature research. Then these components and targets were associated with the target information related to DKD in GeneCards, OMIM, TTD and DrugBank databases. Based on the protein-protein interaction (PPI) network constructed by String11.0 software, the key targets corresponding to the main active components in YSTLF were obtained. The " component-disease-target" regulatory network diagram with TCM compounds was constructed by cytoscape 3.8.0, and the pharmacodynamic mechanism of YSTLF in the treatment of DKD was preliminarily predicted. At the same time, gene ontology (GO) enrichment analysis and Kyoto Encyclopedia of Gene and Genome (KEGG) enrichment analysis were carried out using R language BioConductor and pathview software to reveal the functions of related target genes and pathways. With the mesangial cells (SV40-MES-13) of mice induced by high glucose as the main research object, the effects of YSTLF on the expression of key target genes and the regulation of related pathways were verified by luciferase reporter gene detection, RT-qPCR and western blot.Results108 active components of YSTLF with potential therapeutic effect of DKD and 417 corresponding targets were screened. The " component-disease-target" network diagram suggested that kaempferol and astragaloside were the main active components in YSTLF in the treatment of DKD, and Akt and IL-6 were the key targets of the above-mentioned chemical components. The result of GO functional annotation and KEGG pathway enrichment showed that the selected key targets were mainly involved in a series of biological reactions, such as drug immune response, oxidative stress response, inflammatory response, and signal transduction, etc., and were mainly involved in the regulation of AGE-RAGE and PI3K/AKT signal pathways. The result showed that YSTLF could effectively improve the activation of inflammation related indexes TNF-α and IL-6 and fibrosis related indexes TGF-β1, α-SMA and Col-Ⅰ in SV40-MES-13 cells in mice induced by high glucose.ConclusionYishen Tongluo Formula seems to be multi-component, multi-target and multi-pathway in the treatment of DKD. It may play a role in the treatment of DKD by regulating AGE-RAGE and PI3K/AKT signal pathways, thus inhibiting inflammatory reaction and renal tissue fibrosis.

      摘要:ObjectiveTo establish a model for assessing the degree of coronary artery obstruction based on features of their pulse-graph parameters, so as to explore the clinical diagnostic value of pulse diagnosis.MethodsSmart TCM-I electropulsograph was used to collect pulse samples. According to degrees of coronary artery occlusion based on coronary angiography (CAG) result , 531 patients with coronary heart disease (CHD) or suspicious CHD were divided into three groups: non-occlusion group, mild occlusion group and moderate/severe occlusion group. The time-domain features and MSE features of pulse samples were extracted by time-domain analysis and Multiscale entropy (MSE), and the differences in pulse features among different groups were compared with nonparametric test. Based on the features of pulse-graph parameters, the model for assessing the degree of coronary artery occlusion was established with the random forest (RF) machine leaning method .ResultsCompared with the non-occlusion group, h2/h1 and h3/h1 among time-domain features of pulse-graph were significantly increased in mild occlusion group and moderate/severe occlusion group (P<0.05). Compared with the non-occlusion group and mild occlusion group, MSE1, MSE2, MSE3, MSE4 and MSE5 were significantly decreased in moderate/severe occlusion group (P<0.01). Based on the characteristics of pulse-graph parameters of 531 cases, a model for assessing degree of coronary artery occlusion was established with a 86.79% average accuracy rate.ConclusionPulse detection technology has potential application value in the assessment of coronary risk events.

      摘要:ObjectiveTo provide a basis for determining the appropriate growth year and scientifically setting the specification grade of Rheum Tanguticum in its artificial cultivation.MethodsThe 3-year-old to 6-year-old cultivated Rheum Tanguticum with the same place of growth and harvest season were selected as the research material. The specifications were divided into rhizome and root segment. The root segment was further divided into three grades according to the diameter: >5 cm , 3-5 cm and<3 cm. The weight proportion and active component content of Rheum Tanguticum selected of different specification grades and growth years were measured.ResultsThere was a significant difference in the weight proportion of root segments with the diameter of 3 ~ 5 cm and <3 cm Rheum Tanguticum with different growth years (P<0.01). The weight proportion of root segments with the diameter of 3 ~ 5 cm would gradually increase with the growth year, while the weight proportion of root segments with a diameter <3 cm would gradually decrease with the growth year. The growth year and the specification grade had a significant influence on the contents of three kinds of active components of Rheum Tanguticum: Dianthrone, free anthraquinone and tannin. The growth year and specification grade had similar impact on the content of Dianthrone, with the highest content found in 6-year-old roots with diameter > 3 cm. The content of free anthraquinone was more affected by the specification grade, with the highest content found in the roots with diameter >5 cm. The content of tannin was more affected by growth year, with the highest content found in 6-year-old Rheum Tanguticum.ConclusionThe influence of specification grade on the quality seems to stem from the difference of growth year, so the growth year should be regulated before matching the specification grade (according to diameter of root) of Rheum Tanguticum with its quality. For the evaluation of the quality of Materia Medica, multi-dimensional evaluation based on the active components could be considered for a more accurate evaluation of the quality of Materia Medica.

      摘要:The theory of "heart housing shen (mind)" elaborated on in Huangdi Neijing (Huangdi's Internal Classic) has important guiding significance in clinical practice. In view of the prevalence and morbidity of coronary heart disease with anxiety and depression, which is considered to be the disorder of heart's function, the heart failing to house shen could be used in explanation of its pathogenesis. This paper explains the theoretical connotation of "heart housing shen" and the intrinsic connection between such a theory and the coronary heart disease with anxiety and depression. Based on that, it is stated that "treatment to restore the heart's dual functions" and "simultaneous treatment of the body and mind" are the fundamental treatment principles for that disorder. Besides, as the onset of the coronary heart disease with anxiety and depression is also closely related to the liver, both the heart and liver should be addressed by nourishing the heart and calming the shen, promoting blood circulation and removing blood stasis, and regulating the liver qi to relieve qi stagnation.

      摘要:Patellofemoral osteoarthritis is one of the common clinical knee-joint diseases. Based on the theory of "Tendon Constraining Bone" , this paper analyzes the meaning of tendon and bone and their relationships in TCM. Moreover, the causes of patellofemoral osteoarthritis and pain are explained in light of tendon. As "the knee is the house of tendons" , tendons play a leading role in the pathogenesis and treatment of patellofemoral osteoarthritis. Therefore, the theory of "Tendon Constraining Bone" plays an essential guiding role in TCM understanding and treatment of osteoarthropathy. Tendons can move joints and maintain joint stability. Through analyzing the theory of "Tendon Constraining Bone" and the relationship between tendon and patellofemoral osteoarthritis, this paper further discusses the rationale of treating patellofemoral osteoarthritis with tendon-regulating and kneecap-moving manipulation, which could provide reference for deepening the application of the theory to guide clinical practice and relevant basic researches.

      摘要:Sjogren's syndrome is a refractory disease for which TCM demonstrates certain advantages in its treatment. " Prevalent dryness affecting the liver" is a classical theory from Huangdi Neijing (Huangdi's Internal Classic), which has important guiding significance for the treatment of Sjogren's syndrome with TCM. In this paper, the connotation and development of the theory of " prevalent dryness affecting the liver" is comprehensively analyzed in combination with clinical practice. Sjogren's syndrome is closely related to the liver itself, its functions, and the coordination between the liver and other viscera. The liver itself pertains to yin, and it nourishes blood and moistens dryness. The liver's functions pertain to yang, and they help to move body fluids without any sign of dryness. It's also of great significance that the liver and other viscera are well coordinated. In ancient books and modern studies, many clinical manifestations of Sjogren's syndrome, including anxiety and depression, are closely related to the liver. Therefore, the treatment methods based on regulating and soothing the liver are summarized, such as soothing the liver qi, clearing the liver heat and nourishing the liver yin, taking into account all the five zang organs, and dredging the channels and collaterals. Taking the theory of " prevalent dryness affecting the liver" as the breakthrough point, this paper systematically sorts out the onset, pathogenesis, pattern differentiation, and the significance of addressing the liver in the treatment of Sjogren's syndrome to provide reference for clinical practice.
