      摘要:China has made a major commitment to building world-class universities and top-class disciplines(Double First-Class Initiative) in order to enhance the comprehensive strength and international competitiveness of higher education and realize a historical leap from a country with extensive higher education to a powerful country in education. The "double first-class" construction of universities of Chinese medicine should clarify the three key issues of target and tatus, talent construction and platform construction, concern the three evaluation elements of strength, contribution and reputation, and focus on the five "world-class" of teaching team, talent training, scientific research level, cultural inheritance, achievement transformation and social service ability. The universities of Chinese medicine should grasp the historical opportunity to serve the national needs of high-level talents, major scientific and technological strategies and cultural inheritance and innovation, as well as the needs of building a shared community of health for mankind through the internationalization of Chinese medicine under the "Belt and Road Initiative" . By targeting the "world-class" universities, universities of Chinese medicine should open up the road of "double first-class" construction with Chinese characteristics.

      摘要:This paper summarizes and reviews the hotspots and progress of the researches on major depressive disorder (MDD) from the perspectives of traditional Chinese medicine (TCM) and Western medicine. It is considered that certain progress has been made in terms of the epidemiology, pathogenesis, diagnosis and screening, treatment, improving the patient′s quality of life, TCM pattern research and the role of TCM to reduce suicide and recurrence. The future directions of the researches on MDD in TCM and Western medicine are the pathogenesis, continuous updating of clinical diagnostic standards, research and development of new Chinese patent antidepressant medicine, with a focus on deepening the understanding of MDD from the TCM perspective and TCM interpretation and featured treatment of characteristic symptoms of MDD, as well as lowering the risk of suicide and recurrence rate of MDD on the basis of enhanced curative effect.

      Special Theme: Prevention and Treatment of Major Depressive Disorder

      摘要:This paper sorts out the theoretical origin and development of the concept of "yu" in TCM, discusses the difference and connection between "yu" in a broad sense(stagnation) and "yu" in a narrow sense(depressive disorder), and differentiates the meaning of "yu pattern" (stagnation) and "yu disorder" (depressive disorder). Through clarifying the interrelationship between "yu(stagnation) caused by disease" and "disease caused by yu (depressive disorder)" in the occurrence and development of diseases, it′s pointed out that "yu-disease-yu" and "disease-yu-disease" are common malignant cycles in clinic practice. The advantages of "harmony of body and spirit" and "simultaneous treatment of disease and yu" in traditional Chinese Medicine can be brought into play with an understanding of the connotations of "yu(stagnation) caused by disease" and "disease caused by yu (depressive disorder)" , which will provide references for clinical diagnosis and treatment.

      摘要:ObjectiveTo establish a TCM information database for the prevention and treatment of major depressive disorder(MDD) based on cloud data technology, and to evaluate the clinical efficacy of traditional Chinese medicine(TCM) regimen in the prevention and treatment of suicide risk in MDD.MethodsA total of 4 440 patients with MDD who were inpatients or outpatients in 13 hospitals including TCM hospitals, integrated Chinese and Western medicine hospitals and psychiatric hospitals in 7 cities across the country were recruited, among which 2 513 patients without suicidal ideation were included in this cohort study.The data was collected from January 2017 to April 2018.Three groups of cohorts (TCM, Western medicine, and integrated Chinese & Western medicine) were naturally formed.The follow-up interval was 3 months.Taking the appearance of suicidal ideation as the end event and the Montgomery-Asberg Depression Rating Scale(MARDS) as the evaluation tool in the follow-up, we assessed the patients′ dynamic status of depression and the occurrence of the end event.ResultsA TCM cohort(896 patients), a Western medicine cohort(744 patients), and an integrated Chinese and Western medicine cohort(873 patients) were established with MDD patients without suicidal ideation at baseline, and the overall two-year incidence rates of suicidal ideation in the three groups were 34.5%(309/896), 46.9%(349/744), and 23.0%(201/873) respectively.Compared with the Western medicine cohort, the risk of suicidal ideation was reduced in the TCM cohort by 33.9% (HR=0.671, 95% CI(0.575-0.784), P<0.001), and by 59.4% (HR=0.406, 95%CI (0.341-0.483), P<0.001) in the integrated Chinese and Western medicine cohort.In the TCM cohort, compared with low-level TCM exposure time(TCM-ET), the risk of suicidal ideation was reduced by 47.2%(HR=0.528, 95%CI(0.414-0.674), P<0.001), with medium-level TCM-ET, and by 81.6% (HR=0.184, 95%CI(0.127-0.267), P<0.001), with high-level TCM-ET.In the integrated Chinese and Western medicine cohort, compared with the low-level TCM-ET time, the risk of suicidal ideation was reduced by 39.9%(HR=0.601, 95% CI(0.438-0.826), P<0.001) with medium-level exposure time, and by 65.9% (HR=0.341, 95%CI(0.235-0.495), P<0.001) with high-level exposure time, indicating that a higher level of exposure time could reduce the suicidal risk.ConclusionTCM, Western medicine, or integrated TCM and western medicine could all improve the depressive symptoms and suicidal ideation in MDD patients, and the application of TCM treatment seems to better reduce the probability of suicidal ideation in MDD patients than Western medicine treatment alone.The application of TCM treatment for over 6 months (medium-level or above TCM-ET) indicates a greater effect on the prevention of suicide in patients with MDD.

      摘要:ObjectiveTo evaluate the efficacy of a comprehensive TCM regimen on reducing the 2-year recurrence rate of major depressive disorder(MDD).MethodsThis large-scale multi-centered, prospective cohort study was conducted by recruiting MDD patients from 13 sub-centers nationwide from January 2017 to April 2018 with assistance of the Boshi Medical Cloud Data Management Platform .A comprehensive TCM regimen was used in the intervention of MDD patients. Three cohorts(TCM cohort, integrated Chinese and western medicine cohort, and western medicine cohort) were formed to observe the differences in the recurrence rate of MDD. According to the exposure time of the comprehensive TCM regimen intervention, the TCM cohort and the integrated Chinese and western medicine cohort were further divided into three groups with low, medium and high exposure levels respectively, and difference in the recurrence rate among those groups of different exposure levels was observed, so as to evaluate the long-term efficacy of the comprehensive TCM regimen in reducing the recurrence rate of MDD.ResultsA total of 4 440 patients with MDD were included in the TCM cohort(1 484 patients), western medicine cohort(1 412 patients) and integrated Chinese and Western medicine cohort(1 544 patients). The two-year recurrence rate was 25.5% in the TCM cohort, 35.4% in the western medicine cohort, and 19.3% in the integrated Chinese and western medicine cohort. Compared with the recurrence risk of the low-level exposure group in the Chinese medicine cohort, it was 41.6% lower in the medium-level exposure group(HR=0.584, 95%CI(0.467~0.731), P<0.001), and 71.3% lower in the high-level exposure group(HR=0.287, 95%CI(0.214~0.384), P<0.001). Compared with the recurrence risk of the low-level exposure group in the integrated Chinese and western medicine cohort, it was 54.3% lower in the medium-level exposure group(HR=0.457, 95%CI(0.352~0.692), P<0.001) and 75.2% lower in the high-level exposure group(HR=0.248, 95%CI(0.182~0.337), P<0.001).ConclusionThe comprehensive TCM regimen seems to reduce the 2-year recurrence rate of MDD. There is probably a close correlation between the exposure level of TCM intervention and the recurrence rate, and a longer exposure duration could possibly lead to a lower recurrence rate.

      摘要:ObjectiveTo observe the changes in the integrity of white matter fiber bundle and local cerebral perfusion in relevant brain areas of depressed rats with magnetic resonance technique, and to verify the positive regulatory effect of Yinao Jieyu (Brain-benefiting Depression-resolving, YNJY) Formula on depression from the perspective of neuroimaging in a non-isolated experiment with depressed rats.MethodsBased on the result of behavioral assessment, male Wistar rats were randomly divided into normal group, model group, YNJY group and fluoxetine hydrochloride group(FH group). And the depression model was established by chronic unpredictable mild stimulation(CUMS). Then the YNJY group was gavaged with YNJY decoction, the FH group with fluoxetine hydrochloride capsule, and the other two groups with distilled water for four weeks. After that, magnetic resonance diffusion tensor imaging(DTI) and arterial spin labeling(ASL) were performed to measure the fractional anisotropy(FA), apparent diffusion coefficient(ADC) and cerebral blood flow(CBF) values of each group, so as to obtain the nerve fiber structure and local cerebral perfusion level of the corresponding areas in rats.ResultsDTI showed that in bilateral hippocampus and prefrontal cortex in the model group, the FA values were lower(P<0.05, P<0.01), and the ADC values were significantly higher compared with those of the normal group (P<0.05, P<0.01). Compared with the model group, FA values in the YNJY and FH groups were significantly higher in the right hippocampus and bilateral prefrontal cortex (P<0. 05, P<0.01), and ADC values were lower in the left prefrontal cortex(P<0.05, P<0.01). ② The result of ASL imaging showed that the CBF values in the left hippocampus and bilateral prefrontal cortex regions of the brain were lower in the model group compared with those of the normal group(P<0.05, P<0.01), and the CBF values in the bilateral prefrontal cortex were significantly higher in the YNJY and FH groups compared with those of the model group(P<0.05).ConclusionFiber bundle damage of varying degrees and microcirculatory changes in hippocampus and prefrontal cortex could be observed in depressed rats. Besides, YNJY Formula seems to alleviate or block depressive symptoms by improving neural microcirculation and metabolism, repairing damaged fiber bundles, restoring neuronal integrity, improving neuronal activity and remodeling ability. The above findings suggest that the application of MRI omics technology could be used to reflect the structural and functional changes in the development of depression, and provide reference for the evaluation of the curative effect.

      摘要:ObjectiveTo observe the changes in brain function and serum tumor necrosis factor alpha(TNF-α) in MDD patients with liver qi stagnation or kidney yang deficiency patterns, and to preliminarily discuss the possible targets of brain region in the TCM treatment of different patterns of MDD through the correlation between TNF-α and brain functions.MethodsThree groups of participants(patients with moderate MDD with kidney yang deficiency pattern, patients with moderate MDD with liver qi stagnation pattern, and healthy participants)(n=17) were recruited from October 2017 to October 2020. The former two groups were recruited from the Third Affiliated Hospital of Beijing University of Chinese Medicine, Xuanwu Hospital and Anding Hospital, and the healthy control group was recruited from students and teachers at Beijing University of Chinese Medicine and staffs at the Third Affiliated Hospital of Beijing University of Chinese Medicine. The participants in each group were with matched gender, age and years of education. All participants completed resting-state fMRI and serum collection on the same day. Serum TNF-α level was detected by ELISA; MRI data was processed by SPM and DPASF for brain function assessment and correlation analysis.ResultsThe level of TNF-α in patients with either pattern was higher than that in the control group, and it was higher in the liver qi stagnation group than that in the kidney yang deficiency group(P<0.01). Compared with the control group, the left precuneus and the left superior parietal lobe were abnormally activated in patients with kidney yang deficiency pattern; while abnormal activation was found in the right superior temporal gyrus, right insula, left inferior occipital gyrus, left inferior temporal gyrus, left cuneus, right precuneus, and left precuneus in patients with liver qi stagnation pattern. The serum TNF-α in patients with kidney yang deficiency was positively correlated with the fALFF values of right dorsolateral superior frontal gyrus, right middle frontal gyrus, left middle occipital gyrus, left precentral gyrus, and left rectal gyrus(P<0.05), while it was negatively correlated with the fALFF values of the left precuneus, right inferior temporal gyrus, right middle occipital gyrus and right cerebellum(P<0.05). In patients with liver qi stagnation, only the fALFF value of cerebellum was positively correlated with serum TNF-α(P<0.05).ConclusionThere seem to be differences in brain function and serum TNF-α levels in MDD patients with liver qi stagnation pattern and kidney yang deficiency pattern. Patients with kidney yang deficiency may have more abnormalities in attention-related brain regions and stronger correlation with depression-related inflammatory responses, mainly related to abnormalities of the dorsolateral prefrontal lobe. However, patients with liver qi stagnation may have more abnormalities in vision-related brain regions, and the correlation between inflammation and brain regions is probably weaker than that of patients with kidney yang deficiency.

      摘要:In order to clarify the connotation of the discipline and thinking of traditional Chinese medicine(TCM), this paper systematically expounds the formulation of the core concept of Huangdi Neijing(Huangdi′s Internal Classic, hereinafter referred to as Neijing), its connotation and its influences on the development of TCM theory. It is concluded that under the influence of the need to define the connotation of TCM discipline and the trend to explain the discipline from the perspective of TCM thinking, the connotation of TCM thinking is gradually made clear, and summarized as the core concept of Neijing. Such a concept serves as the core to understand and explain the phenomenon of life from the point of view of function, movement and change and holism on the basis of the theories of image, yin and yang, five elements and essence and qi combined with the object of medical researches; and the thought of harmony and balance is frequently emphasized in the concept. It is the most pivotal and representative part of the TCM thinking mode, and the key to understanding the theory, knowledge, culture and wisdom of TCM. Besides, it also ensures the curative effects and guarantees that the TCM theory can be inherited and innovated continuously, where the connotation remains unchanged.

      摘要:The inheritance and innovation of the experience of prestigious senior traditional Chinese medicine(TCM) experts is beneficial to the training and career development of TCM practitioners. In recent years, the relevant work has attracted attention and made some progress, but it is still faced with many problems in the process, which may lead to inheritance bias or attenuation, and eventually affect the inheritance quality. Through research on the inheritance content, team, management, mode and other aspects, this paper analyzes the present situation and weakness in the inheritance of the experience of prestigious senior TCM experts in detail. Based on that, this paper puts forward not only the concept of equal emphasis on Dao and Shu(doctrines and techniques) in inheritance, but also the idea of integrating multiple method to carry out the research, so as to make the inheritance work more comprehensive, specific and feasible. With the goal of fully understanding their roles as people, doctors, teachers, and scholars, this paper might provide new ideas and approaches for the inheritance and innovation of TCM.

      摘要:Collateral theory originates from Huangdi Neijing(Huangdi’s Internal Classic). The corresponding therapeutic method originates from Shanghan Zabing Lun(Treatise on Cold Damage and Miscellaneous Diseases). And its pattern differentiation system is initially formed in Linzheng Zhinan Yi’an(Case Records as a Guide to Clinical Practice). The theory, widely adopted in various disciplines, still needs improvement in dermatology. According to the collateral theory, collaterals of the skin consist of qi collaterals, fluid collaterals and blood collaterals. Among them, the blood collaterals govern blood circulation in the vessels, the qi collaterals govern the defense against external pathogenic factors, and the fluid collaterals govern the filling and moistening of the skin and interstices. The cutaneous blood collaterals(vessels of the skin and interstices) play a critical role in the transmission of disease from internal organs to skin. The harmonious functioning of cutaneous blood collaterals will bring harmony to the other two types of collaterals. Otherwise, the cutaneous blood collaterals may become a necessary junction in the transmission and development of pathological changes in the cutaneous collaterals, which are also regarded as the precondition of skin diseases. Moreover, the mechanism of chronic skin diseases is also characterized by the latent pathogenic factors in cutaneous blood collaterals. This paper proposes the theory of cutaneous blood collaterals based on the origination and development of the collateral theory with an analysis of its connotation and significance in both theoretical and clinical aspects, which might provide insights into the application of collateral theory in the treatment of skin diseases.

      摘要:The elderly has the physiological characteristics of insufficient essence and qi, declining original qi, and overall deficiency of the four channels and five zang organs, as well as the pathological characteristics of deficiency in the root and excess at the tip, the deficiency-excess complexity, and lingering pathogenic factors due to deficient healthy qi. The kidney, mingmen and triple jiao are physiologically interrelated and complement each other in function. Together with the five zang organs and six fu organs, they play the role of qi transformation as a whole. The deficiency of kidney essence and the decline of mingmen fire are the root causes of age-related debility, while the failure of qi transformation of the triple jiao is the key for transformation between deficiency and excess patterns. From the perspective of "kidney-mingmen-triple jiao qi transformation" , we analyze the TCM pathogenesis and symptoms of debility. Specifically, as the kidney fails to store essence, kidney deficiency may occur; as the kidney is not in charge of the external, pathogenic factors tend to invade the body; when the kidney fails to produce marrow, the spirit may not gather; simultaneously, the triple jiao fails to support qi transformation normally. As a result , a deficiency pattern tends to be complicated with an excess one with lingering pathogenic factors. Such an interpretation may contribute to the deep understanding of the linkage of "essence-qi-spirit-physique" in age-related debility and the establishment of a correlative dynamic pathogenesis for the condition.

      摘要:ObjectiveTo develop the knowledge-attitude-practice(KAP) scale for preventive treatment of disease through constitution of Chinese medicine(KAP-PTDCCM) and test its reliability and validity, in order to provide an assessing tool for health management in preventive treatment through constitution of Chinese medicine.MethodsThe KAP-PTDCCM was initially developed by referring to literature review combined with expert opinion based on the KAP Theory. A convenience sample of 922 students from three universities in Xinjiang and Beijing were investigated. Reliabilities of KAP-PTDCCM were tested by internal consistency reliability and split-half reliability. Validities of KAP-PTDCCM were tested by construct validity, convergent validity and discriminant validity.ResultsThe KAP-PTDCCM consisted of 24 items belonging to three dimensions: Knowledge(8 items), Belief/Attitude(9 items), and Practice(7 items). The total Cronbach′s α value of KAP-PTDCCM was 0.925(>0.9) and varied from 0.887 to 0.943(>0.8) for its three dimensions. Split-half reliability was 0.960. Results of exploratory factor analysis were in accordance with KAP theory, and the cumulative contribution rate of the top three factors was 66.33%. The indexes of confirmatory factor analysis showed a good fitness of the model. Convergent validity showed that correlation coefficients ranged from 0.538 to 0.838(P<0.05), and discriminant validity showed that correlation coefficients ranged from 0.096 to 0.541(P<0.05). The successful rate was 100% according to the convergent validity and discriminant validity tests.ConclusionThe 24-item scale of KAP-PTDCCM seems to show good reliability and validity for the application in evaluating the knowledge, attitude, practice, and health management effect of preventive treatment of disease through constitution of Chinese medicine.

      摘要:ObjectiveTo explore the correlation between TCM constitution types and mental health, so as to provide reference for the further enrichment and development of the TCM constitution theory.MethodsWang Qi’s Questionnaire of Nine Body Constitutions in TCM and Symptom Checklist 90(SCL-90) were used to evaluate the body constitution and mental health of college students from Beijing University of Chinese Medicine. The correlation between TCM constitution and SCL-90 items was explored with Pearson correlation analysis. Then the correlation between the 9 types of constitution and SCL-90 item scores was analyzed with multiple linear regression analysis with the confounding factors(age and sex) controlled.ResultsA total of 900 subjects were recruited. The result of Pearson correlation analysis showed that the scores of SCL-90 items were negatively correlated with the scores of balanced constitution, but positively correlated with those of the other eight kinds of biased TCM constitution, with the constitutions of qi stagnation(r=-0.432~-0.602, P<0.001), qi deficiency(r=-0.385~-0.544, P<0.001) and blood stasis(r=-0.326~-0.505, P<0.001) being the top three. Multivariate linear regression analysis with the confounding factors(age and sex) controlled showed that the scores of qi deficiency, phlegm dampness, blood stasis, and qi stagnation constitutions were risk factors for somatization(P<0.05). Scores of blood stasis and qi stagnation constitutions were the risk factors for obsessive-compulsive symptoms(P<0.01), while the scores of balanced and yang deficiency constitutions were the protective factors for such symptoms(P<0.01). The scores of damp heat and qi stagnation constitutions were both the risk factors for interpersonal sensitivity and paranoid ideation(P<0.01), while the scores of balanced and yang deficiency constitutions were the protective factors for them(P<0.01). The scores of blood stasis and qi stagnation constitutions were the risk factors for depression(P<0.05), while the scores of balanced and yang deficiency constitutions were the protective factors for it(P<0.05). The scores of qi deficiency, blood stasis and qi stagnation constitutions were all risk factors for anxiety(P<0.05), while the scores of balanced and yang deficiency constitutions were protective factors for it(P<0.05). The score of qi stagnation constitution was a risk factor for hostility(P<0.01), while the score of yang deficiency constitution was a protective factor for it(P<0.01). The score of qi stagnation constitution was a risk factor for phobic anxiety(P<0.05), while the score of balanced constitution was a protective factor for it(P<0.05). The scores of blood stasis and qi stagnation constitutions were the risk factors for psychoticism(P<0.01), while the scores of balanced and yang deficiency constitutions were the protective factors(P<0.05). The score of qi stagnation constitution was a risk factor for the others score(P<0.001), while the scores of balanced and yang deficiency constitutions were the protective factors(P<0.001). Male was a risk factor for all SCL-90 factors, and age was a protective factor for the 8 factors of SCL-90 except for somatization and anxiety.ConclusionThe mental health degree according to SCL-90 seems to have certain correlation with the nine kinds of TCM constitutions, with the balanced constitution as the protective factor for mental health, and the blood stasis and qi stagnation constitutions as the risk factors.

      摘要:ObjectiveTo construct a phlegm-dampness constitution knowledge map based on the Neo4j graph database, so as to provide a preliminary basis for the follow-up research on constitution-related question and answer, intelligent auxiliary diagnosis and treatment, and health plan recommendation.MethodsThe related data of phlegm-dampness constitution were comprehensively collected by searching CNKI, Wanfang Data and VIP. The entities related to phlegm-dampness constitution were extracted by manual labeling, and the relationships among them were further explored. The knowledge ontology of phlegm-dampness constitution was constructed from the aspects of forming factors, disease susceptibility, characteristics of the constitution and corresponding regulation method with reference to the TCM clinical terminology and semantic network standards.ResultsIn the Neo4j graph database, a total of 19 types of node labels and 6 types of relationships were created, including 383 nodes and 844 connections, which realized the preliminary construction and visualization of the phlegm-dampness constitution knowledge map.ConclusionThe phlegm-dampness constitution knowledge map constructed with the Neo4j graph database seems to be intuitive and efficient, which lays the foundation for the establishment of a knowledge sharing platform of TCM constitution.

      摘要:Concurrent body constitution is quite common, while its identification poses great challenges to the current TCM constitution research because of various and complicated combinations. In order to optimize the existing constitution questionnaire and developing the auxiliary analysis scale with the concurrent constitutions as the research focus, this paper elaborates the development of concurrent body constitution questionnaire under the condition of "conflict restriction" from the aspects of theoretical framework and questionnaire formation and evaluation, in order to provide reference for the identification of concurrent body constitution to facilitate its clinical and public health applications.

      摘要:Based on the "Five-Phase Evolution" , the occurrence and development of tumor is discussed from three aspects in this paper. Specifically, deficient qi and yang fails to diffuse due to stagnation and blockage, leading to the formation of tumors; the toxicity of drugs further damages yang and result in static toxin, leading to the progression of tumors; pathogenic factors are blocked internally due to qi collapse, then the stagnated yang toxin leads to the metastasis of tumors. This paper innovatively proposes that "qi-yang stagnation and blockage" is the core of the occurrence and development of tumor. The therapeutic principles for different phases of tumors are proposed in this paper, including reinforcing qi and warming yang, and reinforcing healthy qi and balancing yin and yang; regulating qi to activate yang, and relieving stagnation and dissolving lumps; removing blood stasis and detoxifying, and strengthening yang and resolving stagnation, etc. This paper may provide theoretical basis for treating tumors by regulating qi and yang and serve as references for the clinical practice.

      摘要:Epidermal growth factor receptor inhibitors(EGFRIs) improve the survival rate of patients with EGFR-mutated non-small cell lung cancer significantly, but may simutaneously cause a series of adverse dermatological events. In traditional Chinese medicine(TCM) theory, EGFRIs could be classified as of dryness-heat nature that injures yin, therefore the adverse dermatological events are caused by heat-toxin of EGFRIs. Heat underpins the whole process of EGFR-mutated non-small cell lung cancer, and the disease location is the lung. The whole course of the disease can be divided into three stages, including wind-heat pattern combined with dampness-heat at the initial stage, yin deficiency and blood dryness pattern at the middle stage, and pattern of dual yin-yang deficiency at the advanced stage. Therefore, in the treatment of EGFRIs-related adverse dermatological events, on the basis of clearing heat, the therapeutic method differ in different stages, i. e. dispelling wind and eliminating dampness, nourishing yin and blood, as well as nourishing the spleen and kidney at the initial, middle, and advanced stages respectively. The formulae include Zhiyang Pingfu(Itch-relieving and Skin-soothing) Formula, Zizao Yangrong Tang(Dryness-relieving and Skin-Nourishing Decoction), Jingui Shenqi Wan(Golden Cabinet’s Kidney Qi Pill) and so on.
