      摘要:Traditional Chinese medicine uses the relative balance of yin and yang to define human health, that is, the steady-state of the overall dynamic balance of the entire open complex system. Steady-state disequilibrium means that the human body enters a narrow pre-disease state, where healthy qi and pathogenic factors compete, and there are two evolutionary trends, namely, healing and disease. Once the disturbance is too strong or the intervention is not effective, the system will continue to lose stability, and the mutation will form a stable disease state. As an important practical principle, the theory of "prevention of disease" runs through the clinical process of prevention, diagnosis, treatment, and rehabilitation of traditional Chinese medicine, and has important guiding significance for the screening and early intervention of the pre-disease population. This article focuses on blood glucose instability, which has great physiological significance and extensive pathological effects, elaborates its prevention and treatment significance, screening status, and intervention method , systematically analyzes its critical slowing principle as a pre-disease state of many major diseases, and explains the "viscera-state" mapping model based on system integrity and openness and the critical slowing model based on the dynamic development of the system contained in the disease prevention methodology of traditional Chinese medicine. This paper discusses the identification and intervention path of pre-disease status based on the "fourfold diagnosis combined with participation" of traditional Chinese medicine, with the aim to provide a reference for the diagnostic and treatment research of traditional Chinese medicine in view of complexity science.

      摘要:Mental activity and state, as the external expression of viscera and essence in the process of human life, are the main research subjects of Chinese medicine constitution (CMC) and psychology. As a branch of traditional Chinese medicine, CMC focuses on the essential and overall physical and mental health of humans, while psychology refers to in-depth research on the formation and development of psychological phenomena. There are certain similarities in research purposes, discipline development, theoretical composition, research method, and other aspects between these two disciplines; therefore, they have some intersection in research fields. According to the three key scientific issues of CMC, this essay compares the classification method, formation factors, external manifestations, measurement research method, and clinical applications of the two disciplines from four aspects: constitution classification of constitution classifiable theory and psychological classification, constitution identification of constitution classifiable theory and psychological measurement, constitution-disease correlation theory and physical-mental diseases, and theory of constitution adjustment and psycho-therapy. It is believed that the two disciplines of CMC and psychology can learn from each other and develop new common research fields.

      摘要:From the perspective of body, qi, and spirit in ancient philosophy, this article elaborates on the integrated life concept of body, qi, and spirit in traditional Chinese medicine. The dynamic coordination imbalance among the body, qi, and spirit is the inherent cause of the occurrence of chronic kidney disease, and based on this, the clinical differentiation and treatment concept of chronic kidney disease with the harmony of body, qi, and spirit is elucidated. Taking body adjustment as the first priority aims to regulate the sick organisms, regulating the tangible body to treat the symptoms and signs of chronic kidney disease, regulating the intangible body to nourish the source, cutting off the disease tendency, and reflecting the simple concept of disease differentiation and syndrome differentiation. Regulating qi is essential, in the stages 1-2 of chronic kidney disease, the focus is on regulating the qi transformation of the lung, spleen, liver, and kidney, while in the stages 3-5, the focus is on eliminating turbid toxins to restore qi transformation, containing a unified concept of constant movement and change. The principle of regulating the spirit is to optimize the management of chronic kidney disease through pharmacological method and non-pharmacological method such as baduanjin, and the core is a holistic view of regulating the body and qi. The purpose of the simultaneous regulation of body, qi, and spirit is to explore new clinical treatments for chronic kidney disease, which can help delay the progression of the disease.

      摘要:Nasosinusitis is the main cause of the olfactory disorder, and this kind of olfactory disorder is persistent and difficult to cure, which is in line with the pathogenesis of chronic and prolonged diseases "long" and "persistent" in the theory of "chronic diseases affecting collaterals" . Traditional Chinese medicine(TCM) believes that collateral diseases are caused by the coagulation of phlegm and accumulation of blood stasis. This paper analyzes the evolution of the pathogenesis according to the three stages of pathogenic factors invasion the collateral by the method of cross-referencing between TCM and western medicine, and conclude that in the initial stage of nasosinusitis-related olfactory disorder, external pathogen attacks the collateral and phlegm and turbidity are internally produced; in the middle stage, phlegm retention occurs in the lung collateral and blood flow is blocked due to coagulation; and in the final stage, the binding of phlegm and stasis occurs, and the collateral is injured by turbidity and toxicity. Treatment is based on clearing turbidity and phlegm, promoting blood circulation and removing blood stasis, and eliminating pathogenic factors and resolving toxins. In addition, there are modern studies of biological mechanisms underlying nasosinusitis-related olfactory disorder, such as microcirculation disturbance, increased levels of inflammatory factors, disturbances in lipid metabolism, and induction of apoptosis, which provide a scientific basis for understanding and treating the disease from the viewpoint of phlegm and stasis. In this paper, the purpose of exploring nasosinusitis-related olfactory disorder caused by phlegm and stasis is to provide patients with new ideas of TCM treatment, and to provide a better TCM treatment solution for patients with limited access to western medicine.

      摘要:Dryness, as an important concept of etiology and pathogenesis in traditional Chinese medicine, is generally believed to be caused by hot injury and body fluid drying up. The understanding of "dryness due to yang decline" is still insufficient, and the clinical treatments are also limited. The description of dryness in the seven chapters on Yunqi accounts for nearly 90% of the whole Huangdi Neijing; whether dryness is matched with cold-jin or the climate, phenology and diseases affected by dryness are characterized by Yang decline, and whether dryness syndrome is treated with drugs with warm or hot properties, all treatment strategies are based on the gradual decline of Yang qi as the fundamental attribute of dryness. By systematically elucidating the close relationship between dryness and coolness and exploring the treatment of dryness due to yang decline by other researchers, we hope to provide a theoretical basis for subsequent research on dryness.

      摘要:Today, "liver wind disease" in Huangdi Neijing is distinguished from "liver wind" pathogenesis. In this article, we reveal the core pathogenesis of "liver wind disease" in Huangdi Neijing, as well as its treatment principles, methods, and prescriptions, in order to inspire modern clinical diagnosis and treatment of mixed anxiety and depressive disorder. "Liver wind disease" in Huangdi Neijing is a type of emotional and mental illness that is caused by liver deficiency and wind and has a dual pathogenic mechanism of organ deficiency and external evil, with liver deficiency and wind movement as the core pathogenic mechanism, and multiple emotional changes such as sadness, anger, and hatred as the main characteristics. In western medicine, mixed anxiety and depressive disorder is a special category of depression, where the main symptoms of depression and anxiety are seen together, and is complex and variable. This disease is similar to "liver wind disease" , which is characterized by variably emotional manifestations, and can therefore be treated with reference to the key pathogenesis of liver deficiency and wind movement, liver qi stagnation and disorders, and the characteristics of wind. The basic principle is to tonify the liver and treat the wind. Bugan Decoction in Essential Prescriptions Worth a Thousand Gold for Emergencies or Dabugan Decoction in Auxiliary Verse on Drugs and Method for Zangfu Organs can be used accordingly. This article aims to illustrate the core pathogenesis of mixed anxiety and depressive disorder by drawing on the key pathogenesis of liver deficiency and wind movement in "liver wind disease" . Taking the method and decoctions of later generations as a reference, this article offers new ideas for the treatment of similar diseases.

      摘要:ObjectiveWe aimed to explore the therapeutic effect of Modified Banxia Xiexin Decoction (BXXX) in the treatment of polycystic ovary syndrome (PCOS) complicated with insulin resistance (IR) through regulating the PI3K/Akt/GLUT4 signaling pathway.MethodsLetrozole combined with a high-fat diet was used to construct a PCOS and IR rat model. Model rats were divided into 6 groups, namely the control group, the model group, the metformin group[0.157 5 g/(kg·d)], and the BXXX high-, medium-, low-dose groups[26.46、13.23、6.615 g(kg·d )], with 6 rats per group, and the corresponding medication was given by gavage for 2 weeks. 6 normal rats were taken as the control group. Fasting blood glucose(FBG) levels were measured. Pathological changes in ovarian tissues were observed via HE staining. Serum sex hormone levels, glucose levels, and lipid metabolism indexes were measured by ELISA. The expression levels of PI3K/Akt/GLUT4 signaling pathway-related factors in ovarian tissues of PCOS and IR rats were determined by RT-PCR, IHC staining, and Western blotting.ResultsCompared with the control group, the structure of the follicles in the model group was abnormal, the number of granular cell layers was reduced, and more cystic dilated follicles were observed. T levels, HOMA-IR levels, and TC and TG levels were increased (P<0.05); and the mRNA levels of Akt1 and the protein expression levels of PI3K, p-PI3K, Akt1, p-Akt1, and GLUT4 were decreased (P<0.05). The corpus luteum and follicles were observed at different developmental stages. Compared with the model group, in the metformin group, BXXX high-dose group, BXXX medium-dose group, and BXXX low-dose group, the number of granulosa cell layers was increased and the number of cystic dilated follicles was decreased or even zero. The T levels in the metformin group and BXXX high-dose group and the TC and TG levels in BXXX high-dose group were decreased (P<0.05); and the mRNA levels of PI3K and Akt1 in the metformin group, BXXX high-, medium-, low-dose group were increased (P > 0.05). The GLUT4 mRNA level in the metformin group, BXXX high-, medium-dose group was increased (P > 0.05). The expression levels of p-PI3K and GLUT4 in the metformin group, BXXX high-, medium-, low-dose group were significantly increased (P<0.05).ConclusionModified Banxia Xiexin Decoction can improve IR and reproductive endocrine levels in PCOS and IR rats to treat PCOS. Its mechanism is related to the regulation of PI3K/Akt/GLUT4 signaling pathway-related factors.

      摘要:ObjectiveTo investigate the molecular mechanism of Zishen Huoxue Formula alleviating hippocampal oxidative stress injury in rats with vascular dementia (VD) by regulating N-methyl-D-aspartate receptor (NMDAR) to promote mitophagy.MethodsThe VD rat model was established by two-vessel occlusion. According to the random number table method , 60 male SD rats were randomly divided into the sham group, the model group, the donepezil group (0.45 mg/kg), the Zishen Huoxue Formula low-dose group (8.9 g/kg), the Zishen Huoxue Formula medium-dose group (17.8 g/kg), and the Zishen Huoxue Formula high-dose group (35.6 g/kg) (n = 10 rats per group). The drug was given by intragastric administration for 14 days. The learning and memory ability of rats in each group was evaluated by the water maze test. According to the random number table method , 50 male SD rats were randomly divided into the sham group, the model group, the Zishen Huoxue Formula high-dose group (35.6 g/kg), the MK-801 (an NMDAR inhibitor)group (1 mg/kg), and the Zishen Huoxue Formula high-dose combined with MK-801 group (35.6 g/kg + 1 mg/kg) (n=10 rats per group). Rats in the MK-801 group were injected intraperitoneally for 7 consecutive days after modeling, while rats in the Zishen Huoxue Formula high-dose combined with MK-801 group were treated with Zishen Huoxue Formula by gavage combined with intraperitoneal injection of MK-801. The drug was given by intragastric administration for 14 days. Western blotting was used to determine the expression levels of proteins involved in the PTEN-induced kinase 1 (PINK1)/Parkinson protein (Parkin) signaling pathway[PINK1, Parkin, prohibitin 2 (PHB2), LC3B, and ubiquitin-binding protein p62]. The mean fluorescence intensity of PINK1 and neuron-specific enolase (NSE) in the hippocampus was measured by immunofluorescence. The contents of malondialdehyde (MDA) and superoxide dismutase (SOD) in the hippocampus were measured by enzyme-linked immunosorbent assay. The mRNA expression levels of NR2A and NR2B and the protein level of NR2B in the hippocampus were detected by real-time PCR and Western blotting.ResultsThe result of the water maze test showed that compared with the sham group, the escaping incubation period was prolonged (P<0.05) and the times of crossing platform were reduced (P<0.05) in the model group. Compared with the model group, the escaping incubation period was shortened in the donepezil and Zishen Huoxue Formula low-, medium-, and high-dose groups (P<0.05, P<0.01), and the times of crossing platform were increased in the donepezil and Zishen Huoxue Formula medium- and high-dose groups (P<0.05, P<0.01). Further research showed that: compared with the model group, the protein expression levels of Parkin, PINK1, PHB2, and LC3Ⅱ/Ⅰ in the hippocampus of rats in the Zishen Huoxue Formula high-dose group were increased (P<0.01), the protein expression of p62 was decreased (P<0.01), the mean fluorescence intensity of PINK1 and NSE was increased (P<0.05, P<0.01), the content of SOD was increased (P<0.01), and the content of MDA was decreased (P<0.01); the protein expression levels of Parkin, PINK1, PHB2, and LC3Ⅱ/Ⅰ in the hippocampus of the MK-801 group were decreased (P<0.01), the protein expression of p62 was increased (P<0.01), and the mean fluorescence intensity of PINK1 and NSE was decreased (P<0.05, P<0.01), the content of SOD was decreased (P<0.05), and the content of MDA was increased (P<0.01). Compared with the MK-801 group, the protein expression levels of Parkin, PINK1, PHB2, and LC3Ⅱ/Ⅰ in the hippocampus of the Zishen Huoxue Formula high-dose combined with MK-801 group were increased (P<0.01), the protein expression of p62 was decreased (P<0.01), the mean fluorescence intensity of PINK1 and NSE was increased (P<0.05, P<0.01), the content of SOD was increased (P<0.01), the content of MDA was decreased (P<0.01), and the mRNA and protein expression levels of NR2B were increased (P<0.01).ConclusionZishen Huoxue Formula can improve oxidative stress injury of hippocampal neurons in VD rats by regulating mitophagy through the NR2B-mediated PINK1/Parkin signaling pathway.

      摘要:ObjectiveWe aimed to explore the effect of Shoutai Pills on the epithelial-mesenchymal transition (EMT) of trophoblasts and the embryo loss rate in embryos of mice with recurrent spontaneous abortion (RSA).MethodsNinety CBA/J female mice aged 6-8 weeks old were randomly divided into the normal group, the model group, the dydrogesterone group, the Shoutai Pills low-dose group, the Shoutai Pills medium-dose group and the Shoutai Pills high-dose group with 15 mice in each group. The normal pregnancy mouse was established with the mice in normal group × BALB/c male mice, and the RSA mice model was established with the mice in other groups × DBA/2 male mice. Female mice were administered doses 14 days before mating. The Shoutai Pills low-dose, medium-dose and high-dose groups were given 7.58, 15.17, 30.33 g/kg intragastric administration respectively, and dydrogesterone group was given dydrogesterone 0.003 g/kg intragastric administration, the normal group and the model group were given the same volume of distilled water intragastric administration. After continuous dosing to the 6th day of pregnancy, nine pregnant mice were selected from each group randomly and sacrificed under anesthesia, and then the embryos were harvested. Pathological changes of trophoblast cells at the maternal-fetal interface were observed by hematoxylin-eosin staining. Immunofluorescence staining was used to determine the protein expression levels, and real-time fluorescence quantitative PCR was used to determine the mRNA expression levels of EMT markers (E-cadherin, Vimentin) and β-catenin of embryonic trophoblasts in each group. The remaining pregnant mice in each group were administered doses until the 14th day of pregnancy, and after mice were sacrificed under anesthesia, embryo loss in each group was observed.ResultsCompared with the normal group, the protein and mRNA expression levels of the epithelial cell marker E-cadherin in the embryonic trophoblasts of the model group were increased (P<0.05), the expression levels of the mesenchymal cell marker Vimentin in the embryonic trophoblasts of the model group were decreased(P<0.05), the expression levels of β-catenin in the embryonic trophoblasts were decreased (P<0.05), and the embryo loss rate of the model group was increased (P<0.05). Compared with the model group, the expression levels of E-cadherin in the embryonic trophoblasts were decreased (P<0.05), the expression levels of Vimentin in the embryonic trophoblasts were increased, and the expression levels of β-catenin in the embryonic trophoblasts were increased of each dose Shoutai Pills group and dydrogesterone group(P<0.05), all of which were most significant in the Shoutai Pills medium-dose and high-dose groups and the dydrogesterone group, the embryo loss rate in the Shoutai Pills low-dose group was decreased (P>0.05), and the embryo loss rate in the Shoutai Pills medium-dose and high-dose groups and the dydrogesterone group was decreased significantly (P<0.05) .ConclusionShoutai Pills can reduce the rate of embryo loss in RSA mice, possibly by promoting EMT by activating β-catenin signaling molecules in trophoblasts and improving the migration and invasion ability of embryonic trophoblasts. These effects are conducive to the implantation of the embryo in the uterine cavity and play a role in protecting the fetus.

      摘要:Motor neuron disease (MND), a clinical refractory disease, can be classified as "a disease of flaccidity and spasticity" in traditional Chinese medicine (TCM). Many doctors have their own ideas regarding the treatment of syndrome differentiation, but there is no clear consensus. The concept of toxins damaging collaterals was developed from the theory of "toxins damaging brain collaterals" put forward by academicia Wang Yongyan, which marks the pathological process of toxicity accumulation and toxicity to collaterals, causing the spread of toxicity, directly damaging the shape and quality of the zang-fu organs and resulting in rapid deterioration of the disease. Combined with the theory of TCM and modern research, the pathological changes of toxins damaging collaterals are consistent with the characteristics of occult, extensive, and intractable lesions of MND. It is suggested that toxins damaging collaterals may be the core pathogenesis of the occurrence and development of MND, namely, the accumulation of toxins from external contraction and internal damage in the medullary collaterals leads to damage to the medullary collaterals, imbalance of qi and blood, metabolic imbalance, and subsequent brain and spinal cord injuries, leading to MND. Moreover, qi deficiency and internal wind-related pathogenesis are extended from toxins damaging collaterals. Qi deficiency and toxins damaging collaterals influence each other, leading to the worsening of disease. Wind images of MND muscles may be induced by toxins damaging collaterals. Therefore, the treatment principle of detoxifying and regulating collaterals to remove evil, replenishing qi, and nourishing yin to solidify the root is put forward, emphasizing supplementing qi and detoxifying, nourishing yin and dispelling wind, and regulating collaterals, in order to expand the options for MND treatment and further improve the clinical effect.

      摘要:Fatigue is a common symptom of ankylosing spondylitis (AS), and an important factor of unsatisfactory treatment outcomes, decreased quality of life, dysfunction, and even disability in AS patients. According to traditional Chinese medicine, essence, qi and spirit are the three major elements of life. Essence is the basic substance for the transformation and maintenance of life. Qi is the fundamental driving force for life activities. Spirit is the master and the supreme commander of all life activities, such as mind, volition, esthesia and movement. Together, essence, qi, and spirit maintain the normal life activities of the human body. In this article, based on an understanding of the concept and interaction of essence, qi, and spirit, combined with the pathogenic characteristics of AS, such as deficiency of the kidney essence, obstruction of dampness and heat, and cold coagulation due to yang deficiency, it is found that AS-related fatigue is closely related to the spleen and kidney, with essence deficiency, qi stagnation, and lack of spirit as its core pathogenesis. Deficiency of kidney essence, lack of essence to nourish the mind are the roots, spleen deficiency, dampness resistance, qi stagnation, and fatigue of the mind are the main syndromes. The main treatment goal is to tonify the kidney and supplement essence, and strengthen the spleen and qi in order to cultivate its roots, dispel dampness and clear away heat, regulate qi and spirit, and restore its normal function. In clinical practice, it is necessary to determine the urgency of the manifestation and root cause, and the severity of deficiency and excess and to add or subtract, in order to provide a reference for the clinical treatment of AS-related fatigue.

      摘要:Recurrent urinary tract infection (RUTI) is a common clinical condition mostly seen in the elderly, post-menopausal women, and people with immunodeficiency. It is characterized by its intractable nature and a high recurrence rate, making it a clinical treatment challenge. We discuss the core pathogenesis and treatment of RUTI based on the theory of "Zhongqi Deficiency Leading to Urine and Defecation Change" in the Yellow Emperor’s Classic of Internal Medicine and the pathological mechanism of modern medicine. On this basis, it is proposed that Zhongqi deficiency, dampness and turbidity, and lingering in the yin orifice are the core pathogenic mechanisms of RUTI. Zhongqi deficiency and loss of activity of the spleen are the basis of the disease. Spleen deficiency, water dampness, and dampness and turbidity in the yin orifice induce RUTI. Zhongqi deficiency impedes the dispelling of evil, so the dampness lies in the lower jiao and lingers on, becoming a vague evil, waiting for an opportunity to flare up, which is the root cause of recurrent RUTI. In terms of treatment, we attach importance to the characteristics of the patient and focus on the Zhongqi, and harmonize the middle qi throughout. During the attacking period, both attack and tonification are applied, and during the remission period, the middle energy is nourished and righteousness is supported. The aim is to enrich the theoretical basis of the treatment of RUTI from the Zhongqi and to provide new ideas for clinical research and treatment.

      摘要:Pulmonary nodules are common in clinical practice. Early screening of high-risk nodules and intervention are the top priority in the management of this disease. At present, the pathogenesis of pulmonary nodules in the field of traditional Chinese medicine (TCM) is still under discussion. Based on the theory of collateral diseases in TCM, we suppose that with the effect of internal and external pathogens, the basic pathological basis of pulmonary nodules is qi deficiency accompanied by phlegm and blood stasis in collateral vessels. The key pathogenesis of pulmonary nodules is yang deficiency and yin aggregation in pulmonary collateral vessels. Tumor toxin accumulation is the central cause of pulmonary nodule deterioration. Combined with recent studies of western medicine, we discuss the diagnosis and treatment of medium- to low-risk and high-risk pulmonary nodules. The basic treatment method is reinforcing yang and repressing yin, while dissipating phlegm and blood stasis from the pulmonary collateral vessels based on syndrome differentiations, and common treatments include tonifying qi, warming yang, resolving phlegm and accumulation, and detoxifying to dissipate nodules. Through all of these methods, the qi and blood will be harmonized, balancing yin and yang, and then dispersing the pulmonary nodules.

      摘要:Primary immune thrombocytopenia (ITP) is the most common hemorrhagic disease. In traditional Chinese medicine (TCM), it belongs to the category of "blood syndrome" or "purpura." Ancient and modern physicians mostly treat it focusing on the spleen or spleen and kidney (TCM terms, not completely the same as the anatomical concepts), and there are also a few doctors who treat it based on the heart or liver (see above). In clinical practice, we found that treatment of ITP based on the lung often achieved better clinical efficacy. Based on ancient medical books and modern literature, we systematically put forward a new viewpoint of regulating blood through "lung qi aiding spleen to control blood" and "lung qi helping heart to transport blood" based on the theory of "the lung is a blood-zang which can transform the blood" and "Jin-shui theoretical and mutual transformation between essence and blood." We also summarize three etiology and pathogenesis theories, lung qi affected by evil, lung qi deficiency, and yang deficiency with external cold manifestation. The four principles of replenishing and restoring lung qi, clearing heat and purging lung, nourishing yin and clearing lung, and warming lung for dispelling cold are proposed to treat ITP and we recommend classical prescriptions. This article broadens the scope of TCM treatment of ITP.

      摘要:ObjectiveTo explore the regulatory effect of qigong abdominal breathing exercise on the integrated structure of the spine and the Governor Vessel, we analyzed the temperature changes of the acupoints of the Governor Vessel and the morphological changes of the spine.MethodsIn total, 68 healthy college students were included as the subjects, and divided into the experimental group and the control group according to the ratio of 1∶1 by randomized block. The experimental group adopted the three-round standing stake exercise with abdominal breathing exercise, and the control group adopted only the three-round standing stake exercise. Both groups were trained for 8 weeks. The temperature of each acupoint of the Governor Vessel [Dazhui (GV14), Taodao (GV13), Shenzhu (GV12), Shendao (GV11), Lingtai (GV10), Zhiyang (GV9), Jinsuo (GV8), Zhongshu (GV7), Jizhong (GV6), Xuanshu (GV5), Mingmen (GV4), Yaoyangguan (GV3)] was measured by an far-infrared thermal imager, the intervertebral angles of the spine (T1-S1) were measured by a spine electronic measuring instrument, and the changes of the two groups before and after intervention were compared.ResultsFar-infrared thermal imaging result showed that, compared with the control group, the temperatures of Dazhui (GV14), Shenzhu (GV12), Shendao (GV11), and Mingmen (GV4) in the experimental group were increased after intervention, and the differences were statistically significant (P<0.05). The spinal morphology measurement result showed that, compared with the control group, the intervertebral angles of T10/11 and L4/5 in the experimental group were smaller after intervention, and the differences were statistically significant (P<0.01).ConclusionQigong abdominal breathing exercise has a benign regulatory effect on the circulation of qi and blood of the Governor Vessel and spine stability.
