El objetivo es investigar el efecto y mecanismo de la combinación de cimicifugósido epóxido y cisplatino sobre la proliferación y apoptosis de células de cáncer de pulmón A549. Se seleccionaron las líneas celulares A549 y BEAS-2B como objetos de estudio. Se utilizó el método CCK-8 para detectar el efecto del cimicifugósido epóxido y cisplatino sobre la viabilidad de células A549, así como el efecto del cimicifugósido epóxido en células BEAS-2B, calculando la concentración inhibitoria media (IC50) y la concentración inhibitoria al 20 % (IC20); se llevaron a cabo experimentos posteriores con IC20 de cimicifugósido epóxido y cisplatino en células A549. Las células A549 se dividieron en grupo control, grupo cisplatino (5,09 μmol/L), grupo cimicifugósido epóxido (4,39 μmol/L) y grupo cisplatino + cimicifugósido epóxido (5,09 μmol/L + 4,39 μmol/L). Después de 48 horas de tratamiento, se detectó la tasa de células positivas con la sonda fluorescente EdU-488; se evaluó la apoptosis con tinción de anexina V/yoduro de propidio; se observó la formación de gotas de lípidos con tinción Bodipy 493/503; se detectó la expresión de proteínas Akt, p-Akt, mTOR, SREBP1, FASN, ACC1, ACLY, ciclina E1, CDK2, Bcl-2, Bax y caspasa-3 clivada mediante western blot; se midió la intensidad media de fluorescencia de SREBP1 mediante inmunofluorescencia confocal. Los valores IC50 de cimicifugósido epóxido para células A549 fueron (22,80±0,93) μmol/L y IC20 (4,39±0,73) μmol/L; para cisplatino, IC50 (31,57±1,53) μmol/L e IC20 (5,09±0,78) μmol/L. Para células BEAS-2B, IC50 de cimicifugósido epóxido fue (26,60±1,41) μmol/L. En comparación con el grupo control, los grupos cisplatino, cimicifugósido epóxido y la combinación mostraron disminución en la tasa de células positivas EdU, aumento en la necrosis celular y apoptosis total, disminución en la formación de gotas lipídicas, reducción en la expresión proteica de p-Akt, mTOR, SREBP1, FASN, ACC1, ACLY, ciclina E1, CDK2, Bcl-2, aumento en la expresión de Bax y caspasa-3 clivada y disminución en la intensidad media de fluorescencia de SREBP1 (p<0,05). En comparación con el grupo cisplatino, el grupo cimicifugósido epóxido presentó mayor necrosis celular, menor formación de gotas lipídicas, menor expresión de proteínas SREBP1, ciclina E1, Bax y caspasa-3 clivada, y mayor expresión de CDK2 y Bcl-2 (p<0,05); el grupo combinación mostró disminución en necrosis celular, aumento en apoptosis total, reducción en formación de gotas lipídicas, disminución en expresión proteica de p-Akt, mTOR, SREBP1, FASN, ACC1, ACLY, ciclina E1, CDK2, Bcl-2, aumento en expresión de Bax y caspasa-3 clivada, y disminución en intensidad media de fluorescencia de SREBP1 (p<0,05). En comparación con el grupo cimicifugósido epóxido, el grupo combinación tuvo menor necrosis celular, mayor apoptosis total, menor formación de gotas lipídicas, disminución en expresión proteica mencionada, aumento en expresión de Bax y caspasa-3 clivada y disminución en intensidad media de fluorescencia de SREBP1 (p<0,05). Conclusión: La combinación de cimicifugósido epóxido y cisplatino puede inhibir la proliferación de células A549 mediante la inhibición de la vía de señalización PI3K/Akt/mTOR, regulando la expresión de SREBP1 y afectando la vía del metabolismo lipídico.
